The intent of the changes to F.S. 316.066 is to combat Insurance fraud place emphasis on injury and CMV crashes capture more data with the inclusion of short

forms

Prepared by law enforcement must be submitted to the department and may be maintained by the Law enforcement officers agency.

Must be submitted within 10 days after an investigations is completed by the law enforcement officer who investigates a motor vehicle that:

◦ Resulted in death◦ Personal injury◦ s.316.061(1) Hit & Run◦ s. 316.193 DUI◦ Rendered a Vehicle inoperable ◦ Involved a Commercial Motor Vehicle

FMCSA: any truck with a GVWR/GVCWR greater than

10,000 lbs eligible to be classified as a CMV. a motor vehicle designed to transport 9 or

more people (including the driver’s seat)

Florida traffic statutesHas a gross vehicle weight rating of 26,001 pounds or more;Is designed to transport more than 15 persons, including the driver; orIs transporting hazardous materials and is required to be placarded.A vehicle that occasionally transports personal property to and from a closed-course motorsport facility, as defined in s. 549.09(1)(a), is not a commercial motor vehicle if the use is not for profit and corporate sponsorship is not involved.

• Name and address of parties involved including drivers and passengers

• Names and address of all witnesses

• These two forms are the same with the exception of the Narrative/Diagram which are not required for short form

• Driver Exchange form (90011S) is kept by local agency

• No more optional short form on Driver Exchange

• HSMV number is required for both short & long forms

1. If not a Hit and Run owner information is required. (“Same as Driver” is not acceptable)

2. A Parked Car cannot have a driver 3. If a person has a first name, they must have a

last name4. For a Non Motorist these are required fields

(most, but not all, vendor applications already require this):
