Framework for CF Care

“I was shocked to see the variation in outcomes across centers. I knew this was important information that we would want to share with our committees.  More importantly, it was obvious that something needed to be done, but at the time I wasn’t sure what we should do.”

E-mail message to CFFFrom: xxxx xxxxx [mailto:xxxxxxxxx@yahoo.com]Sent: Monday, August 23, 2004 1:21 AMSubject: CFF accredited centers with best patient outcomes

Our infant son was recently diagnosed with cystic fibrosis. We have been researching the disease and have come across several articles stressing differences in patient outcomes even among the CFF accredited treatment centers. However, we have been unable to find information that shows which centers have the best patient outcomes. Could you give us this information or let us know how we could find it? We want to make sure our son receives the best possible care. Your help is greatly appreciated.

Aha Moment!

Partnering with our patients and families is important if we wish to accelerate improvement and…..

partners share information freely!

Input from Key Partners

• “We have an amazing group of people in our community… high level of commitment and passion for improvements for this disease”

• “Need to celebrate how far we’ve come……phenomenal improvement”

• “We are leading the charge and doing great things…… exciting”

Input From Three Wise Men

• Pursue a dual strategy: – Pull up those who are lagging & push forward the top

• Patient/family as “captain of the ship”– Health care as a series of decisions made in

conjunction with trusted counselors

• Patient/family experience of care:– Powerful way to explore dimensions off the radar
