2. Detached and signed bottom portion of Prohibited Items List
3. Completed Player Biography Sheet
4. Detached and signed bottom portion of Camp Code of Conduct form
5. Completed and signed waiver and release of liability form
6. Non-refundable deposit of $40.00 (balance due minimum two (2) weeks prior to start of camp
If paying by personal cheque, the balance is required a minimum of four (4) weeks prior to start of camp

This year we are interested in offering a busservice, however the cost without sufficient
participation, can be prohibitive.Bus service based on a minimum of 20campers taking the bus would be an additional$21 per day ($105 per week). The price wouldbe indexed lower with more students.

Essential Items ListThe purpose of the Essential Items list is to maximize the participation of the learning opportunitywhile campers are attending the training. A locker will be provided for each camperEach camper should put together a day pack or gymbag to contain the following items to bebrought each day:
1. Refillable water bottle, canteen, camelback(drinking water is provided at camp)
2. Insect repellant
3. Sun screen
4. Rain wear(minimum top, bottoms optional)
5. Baseball or wide brim hatfor sun protection
6. Notepad and pen or pencil
7. Shorts, t-shirt and casual footwear (running shoes, sandals)
to change into at the end of the day
8. Garbage bag(s)to place dirty, muddy, sweaty paintball clothes and boots for transport home
Paintball Gear:To be worn every day to camp -
- long sleeve shirt- long pants- boots or sturdy shoes
- gloves (optional)- bandana, baseball cap (optional to keep out bugs and paint)
Coveralls are available for use, but can become warm to wear during thesummer months. Army surplus uniforms are affordable and stand up well.

Prohibited Items List
The purpose of the Prohibited Items list is to maximize the learning opportunity while campers areattending the training and to minimize the risk of loss of personal property.The following items are deemed to be detrimental in sustaining an ideal learning environment.
A. Weapons of any kind including but not limited to:Firearms, Pellet guns, Fixed or folding blade knives longer than 15 cm or 6, sling shots, archeryequipment, axes, hatchets, nor machetes.
B. Electronic gaming devices: PSP, Gameboy etc.
C. Portable music devices: iPod, MP3 player, Walkman, Boom box etc.
D. Cellular telephones, Blackberry or PDA (the camp is equipped with a phone and can receiveinbound calls and offer a message centre for campers.)
E. Alcohol or non-prescription drugs, prescription drugs outside of the prescription bottle, over thecounter medications without prior notice to the camp staff
F. Sexually explicit material
G. Fireworks, Fire Crackers or similar pyrotechnic devices
H. Paintballs purchased outside of Georgina Paintball

Camp Code of Conduct
PreambleThe purpose of the Code of Conduct is to outline the limits of conduct considered in agreementwith the goals and the well-being of the Camp community, and to define the procedures to befollowed when students fail to meet the accepted standards of behaviour.
Campers have a responsibility to familiarize themselves with the Camp rules and regulations andto conduct themselves accordingly while attending the Georgina Paintball Camp.
The Camp reserves the right to take necessary and appropriate action to protect the safety, andwelfare of the Camp and its staff.
When Camp rules and regulations are violated the responsibility and final decisions regarding
possible disciplinary action rests with the Director of Georgina Paintball Camp.
Scope
This code applies to unacceptable conduct by campers while on Georgina Paintball premises.Campers are expected to be responsible for their actions whether acting individually or in a group.
Georgina Paintball Camp reserves the right to take necessary and appropriate action to protect thesafety and welfare of the camp. This may include taking necessary and appropriate action in caseswhere a Camper is accused of serious conduct.
Codes of Conduct apply to:(a) human rights offences (including sexual harassment and discrimination)(b) dishonesty(c) discipline issues in regards to recognition of authority
Procedure for Handling MisconductThe preferred method for resolving misconduct is through informal resolution, especially with minoroffences. More serious misconduct can result in suspension from participation without financialremuneration. Misconduct of the most serious nature will result in immediate dismissal from theprogram, and if warranted the contacting of the relevant authorities.
The primary objective of this Code is not to be punitive but rather to be a guideline for appropriatebehaviour in an educational environment.
Dress Code & Use of ProfanityCampers are to refrain from excessive use of profanity.Campers are expected to refrain from wearing inappropriate attire, or in a manner that wouldcompromise the safety of the individual or of others around them, as designated by the Camp staff
