Vessel Type of Injury Description of Injury
Troms Sirius LTI AB. Fractured his right hand as it became pinched between the lifting sling and pipe during cargo operations.
Helmer Hanssen MTC CE. Bruised his right elbow during maintenance work. Prescription medication was given onboard the vessel.
Troms Pollux MTC Trainee. Sustain small rift in finger during maintenance work. Failed to comply with instructions from Chief Officer and rift got infected. Antibiotics prescribed by onshore doctor.
Troms Capella MTC AB. Hit knee on hatch during inspection. Received prescription pain killers from onshore doctor.
Lance MTC While climbing a ladder after a routine inspection of a cargo hold, an AB scraped his wrist on the ladder and suffered a small wound. Wound later became infected and IP was treated with antibiotics by onshore doctor.

In an effort to prevent fatalities and serious injury, collision, property damage and environmental events, Tidewater began to track Significant Incidents in FY 2013. A Significant Incident is defined as an incident where the outcome either was, or could have been, a severe, debilitating injury, a fatality, or an extensive damage or pollution event. All Significant Incidents, whether recordable, first aid, near miss, environmental, collision or property damage, will be fully investigated with causes and corrective actions presented to Tidewater and Troms Offshore personnel.
Significant Incidents
What is a Significant Incident? An accident or high potential event where:
• someone was or could have been severely injured or worse.
• extensive damage happened or could have happened.
• significant pollution was sustained or could have been sustained.
• your first thought when looking at a high potential event is: “we just dodged a serious bullet!”
Then the Incident is Significant.
