NCA SubAwardee Reporting Guide

Instruction Guide and Forms

This brief guide explains the required forms, their location, reporting order, and submission date.

Remember these guide relates to your NCA SubAwardee report for the period of January 1, 2012-June 30, 2012.

Submission Date

Please submit your NCA SubAwardee Report for the period of Jan 1st – June 30th, 2012 WHAT:

Sub-Awardee Fiscal Summary, Report 1 Narrative Report Form Fiscal Documentation

WHEN: July 3rd, no later than 4:30PM WHERE & HOW: E-mail report to:

Report Documents

Required Documents: Sub-Awardee Fiscal Summary, Report 1 Narrative Report Form Fiscal Documentation

All or No Expenditures this period? No Expenditures. Fill out all of the gray areas of the

excel sheet. If you are not submitting for reimbursement in July, enter zeros in the “Expenditures per category for report 1” column. Be sure to fill out Awarded Amount and Requested Budget numbers, etc, - All gray areas and attach confirmation of submitting the Narrative Reporting form.

Entire Expenditure. Fill out all gray areas of the excel sheet. If the full $10,000 has been expended,

column D row 20 will automatically calculate total requested amount as $10,000. Be sure to fill out Awarded Amount and Requested Budget numbers, etc, - All gray areas and attach Narrative Report email confirmation.

Complete and submit form on-line. Make a copy of the email confirming your

submission. Attach confirmation email behind Fiscal Report

Form.

Sub-Awardee Fiscal Summary, Report1 Fiscal Documentation- Do they align with your

stated goals and objectives? Check your Application & Cooperative Agreement.

Signature. This name/signature should correspond with the one listed on your Cooperative Agreement.

Allowable/Unallowable costs list and the new GSA rates – There are changes.

*Formatting (Times New Roman; 12) matters!

Fiscal Documentation

Compile all fiscal documentation as a single .PDF file 

Name the file “Fiscal Documentation_Report 1_your award number.” See next page for your award number.

Attach fiscal documentation order as listed in the report table.

If you have two or more documents for a line item, attach together and cross-reference them.
