This Community-Based Social Services Resource Guide (Community Resource Guide) was
developed by Anthem Blue Cross (Anthem) to help health care providers connect Medi-Cal
Managed Care (Medi-Cal) members to culturally competent resources and services. Some
services are available to Medi-Cal members at no cost. We hope that you will find this guide to
be a helpful tool in connecting your patients with resources to help them improve or maintain
their health.

This Community Resource Guide is divided into three sections: Southern Region, Central
Region, and Northern Region. Under each geographical region, you will first find resources

One-on-one tutoring for youth ages 6 to 18 years of age. Services offered include after-school
tutoring workshops, help for English language learners and assistance with student publications.
Other services include daily writing prompts, help completing homework, help reading silently
and aloud. Volunteers help with field trips, assist with the yearly Young Authors’ Book Project

Adult Day Health Care of Fresno and Clovis counties
Adult Day Health Care offers occupational and speech therapy as well as dietary and nursing
services. A registered nurse is present daily to evaluate and provide medical and nursing care.
Social work services are available to help participants locate needed services, and counseling is
provided to participants and their families. Serves individuals 18 years of age and older with a
physical or mental impairment. Hours of operation are Monday to Friday, 8 a.m. to 2 p.m.
Agency on Aging — Family Caregiver Support Program
The Agency on Aging offers services through their Family Caregiver Support Program. This
program provides caregivers with practical tools to alleviate stress and avoid burnout,
networking opportunities to decrease isolation and share concerns, and stopgap measures to
stabilize the provision of care during times of extreme difficulty. Services include support
groups, classes and case management. Serves Fresno and Madera counties; accepts Medi-Cal

Alcoholics Anonymous (AA)
AA offers a self-help alcoholism recovery program utilizing the 12-step approach. Classes are
offered in English and Spanish at no cost individuals/families. Note, the center is only open for
meetings. Hours of operation are Monday through Friday from 10:30 a.m. to 4:15 p.m. and
Saturday from 9 a.m. to 1 p.m.
Adult Day Centers
OASIS and People Always Learn Something (PALS) Adult Day Centers specialize in Alzheimer
and dementia care — offering therapeutic activities for seniors, and allowing caregivers a
much-needed break. PALS is a program of OASIS for people 40 and older with mild cognitive
impairment in the early stages of dementia. OASIS and PALS provide a full day of stimulating,
recreational and therapeutic activities for people in the earliest to the most advanced stages of
dementia tailored to each participant's personal interests, needs, strengths, cognitive and physical
abilities to nurture self-esteem and dignity. Caregiver support is also provided. Medi-Cal will
not cover the cost of the programs, but financial assistance is available through private donations
and a grant through the Fresno-Madera Area on Aging. Offering attendance options to fit in

At the Asian Pacific Islander Clinic, case management teams provide comprehensive, culturally
and linguistically appropriate outpatient and community-based specialty mental health services
to transition individuals who meet medical necessity. Services are provided using a wellness and

The CCLS provides a range of services from advice to court representation for victims of abuse.
They assist with naturalization, legal permanent resident card renewals and U-Visa/VAWA
applications. CCLS employs attorneys, paralegals and support staff to provide civil legal

representation for those having difficulty accessing health care services) and housing assistance

Fresno Barrios Unidos is an organization that transforms communities by empowering youth and
families through advocacy, education and wellness. Programs offered include, El Joven Noble
(“The Noble Youth”) a youth development and support program and Teen Success, a program
that empowers teen mothers.
Fresno County Department of Public Health Community Health Division
The Fresno County Department of Public Health Community Health Division offers standard
hepatitis C and HIV testing, but they also offer rapid oral and blood HIV testing. Additionally,
hepatitis prevention/education, HIV test counseling, HIV/AIDS prevention education, partner
notification, and treatment information is provided. Hours for HIV testing are
Monday to Thursday from 8:30 a.m. to 11:30 a.m.
Fresno LGBT Community Center
The Fresno LGBT Community Center is open to the public. They offer access to information
and local and national LGBTQ resources including local publications such as News Link and the
CCA Newsletter. They offer reference materials relating to the LGBTQ civil rights movement as
well as printouts of recent court decisions affecting the LGBTQ community. Hours of operation
are Tuesday through Saturday from noon to 5 p.m.
Fresno Rescue Mission
The Fresno Rescue Mission offers an emergency family shelter that provides 14 to 30 days of
temporary shelter to families as well as food, clothing and personal care; referral to other
agencies that can assist them; and love/hope through spiritual support. To assist them in starting
over in an apartment, families are given food packages and other needed supplies when they
leave the shelter. Monday through Friday from 8 a.m. to 5 p.m.
Gay and Lesbian Center — Bakersfield
The Gay and Lesbian Center seeks to improve the well-being of the LGBTQ community with
cultural and educational programs and an extensive referral network. They offer community
building socials and other activities like picnics, dinner outings and camping trips. They also
offer a program that brings together lesbian, gay, bisexual, transgender and ally professionals

from Bakersfield and Kern counties together to celebrate and promote equality in the workplace
and 2030+Q-, a peer support and networking group for individuals under 40 years of age who
identify as LGBTQ or straight allies. Serves Bakersfield and Kern counties. Services are offered
at no cost. Hours are Tuesday through Thursday from 5 p.m. to 7 p.m., Friday from 4 p.m. to 7
p.m. and Saturday from 3 p.m. to 6 p.m.
Guardian Angel Home Health Care-Rehabilitation Service
Guardian Angel Home Care provides quality home health services which meet the needs and
expectations of those injured, ill, disabled, or challenged. Guardian Angel Home Health Care is
comprised of multi-disciplinary professionals who have expertise in skilled nursing, physical
therapy, occupational therapy, speech therapy, medical social services, and home health aide
assistance. Serves California residents. Hours of operation are Monday to Friday,
8:00 a.m. to 5:00 p.m.
Heritage Adult Day Health Care
Heritage Adult Day Health Care offers therapeutic services to seniors. Transportation to and
from the facility is offered at no cost, and breakfast and lunch are provided. Patients typically
stay at the center for five to six hours. Nurses are on staff to monitor each patient’s health, social
workers provide 1:1 sessions, and nutritionists provide consultation. Every week, patients are
taken on field trips to various stores to purchase hygiene products. Serves Fresno County
residents; accepts Medi-Cal members. Hours of operation are 7:30 a.m. to 4:30 p.m.
Homeless Veterans Taskforce by The American Legion
To help our struggling homeless veterans, the Legion's Homeless Veterans Task Force, works to
ensure local services and resources are available to homeless veterans and their families. The
Task Force, which has chairpersons in each department, collaborates with government agencies,
homeless service providers and veterans service organizations to develop and implement
initiatives that will help homeless veterans. Additionally, Homeless Veterans Task Force
chairpersons can provide direct aid to veterans and their families who reach out to them. A
veteran who is homeless or facing possible homelessness can contact a chairperson, who will
provide guidance and assistance to the veteran and his or her family. Serves California
Residents. Hours of Operation are Monday to Friday, 8 a.m. to 5 p.m.
Kings View’s Blue Sky Wellness Center
The Kings View’s Blue Sky Wellness Center offers mental health services to adults by focusing
on key mental health recovery principles including dispensing hope, freedom of choice,
modeling self-determination and personal responsibility, and emphasizing that people can and
do heal from a mental health diagnosis. They offer self-help and support groups as well as a
variety of activities such as computer access, library time, a weekly clothes closet and a gym for
personal use. Serves Fresno County residents. Hours of operation are Monday to Saturday from
8:30 a.m. to 4:30 p.m.

Legal Services for Immigrants by Mil Mujeres
Mil Mujeres providse legal services, community education and empowerment to low-income
Hispanic immigrants and their families in the United States. They offer immigration legal
services to the low-income individuals in the Latino community, specializing in immigration
benefits for survivors of violent crimes in the United States. Our main focus are survivors of
gender related crimes. However, we also provide services to other survivors. Offering help in the
following areas: U-visa (for victims of specific crimes), Violence Against Women Act, Deferred
Action for Childhood Arrivals, T-visa (for victims of trafficking), and Special Immigrant
Juvenile Status.
Madera Rescue Mission
The Madera Rescue Mission is committed to the needy, destitute and homeless. They not only
meet temporary needs but also offer long-term help and solutions to those who truly desire a
change in their life. Serves Madera County residents. Hours of operation are Monday to Friday
from 8 a.m. to 5 p.m.
Marjaree Mason Center
The Marjaree Mason Center offers individual and group therapy for adults and children affected
by domestic violence. They provide emergency and long-term safe housing, legal and financial
assistance services, support services, and children's services to victims of domestic violence as
well as community education. Serves Fresno County residents. Hours of operation are Monday
through Friday from 8:30 a.m. to 5 p.m. 24-Hour Crisis Hotline available at 1-559-233-4357.
Noami’s House
Noami’s House provides overnight shelter to homeless women 365 days a year. They also offer
services at no cost through the combined resources of the Collaborative including hot meals,
medical and dental care, a day shelter, laundry and clothing assistance, case management,
storage, transportation, health and personal living education, and information and referrals.
Assessments are offered Monday to Friday at 3:30 p.m. and Saturday and Sunday at 12:45 p.m.
or 5:45 p.m.
Safe Houses — Fresno Emergency and Transitional Living Safe House
The Safe Houses program assists survivors of domestic violence by providing them with
transitional housing, case management and educational opportunities. This program helps people
develop independent living skills and a means of income to help in attaining the necessary
resources to support their move into permanent housing free of domestic violence. Serves Fresno
County Residents. Hours of operation are Monday to Thursday, 8 a.m. to 5 p.m.
Substance Abuse by Promesa Behavioral Health
This Substance Abuse program offers individual and group drug and alcohol counseling in an
outpatient setting to individuals who struggle with addiction. A wide range of addiction
treatment services are available, including: admission screening, crisis sessions, drug testing,
peer support and mentoring, relapse prevention, new mothers groups, and more. Serving Fresno

Alameda County Public Health Department (ACPHD) Office of AIDS Administration
(OAA)
The mission of the OAA is to reduce the rates of new HIV infections, offer HIV testing and link
individuals who are HIV-positive with care. The OAA provides leadership, resources and
guidance in collaboration with community members and organizations to coordinate the delivery
of HIV/AIDS health services for the Alameda County Public Health Department. Serves
Alameda County residents with a verified HIV-positive status. Residents must provide proof of
income (300% of federal guidelines or lower) to receive services as no cost. Hours are Monday

American Civil Liberties Union (ACLU) of Northern California
The ACLU works to preserve and guarantee the protections of the Bill of Rights. Their aim is to
extend freedoms to segments of populations who have traditionally been denied their rights

health patients; prisoners; people with disabilities; and the poor. Call intake at

Asian Pacific Self-Development and Residential Association (APSARA)
The APSARA is an educational, problem solving organization that provides leadership for
youth. Programs include after-school tutoring, volunteer opportunities for high school students
to assist elementary students, Head Start, the Care program (after-school recreational programs
for teens), free food program for seniors, youth leadership development, dancing groups, sports,
on-the-job training, mental health case management and outreach, health access and outreach,
and translation services and interpretation. Hours of operation are Monday through Friday from
8 a.m. to 5 p.m.
Alameda Recreation and Parks Department (ARPD)
The ARPD offers the following summer activities for youth:
For youth 3 years of age through prekindergarten, summer activities include Introduction

transportation; mentoring youth who have disabilities; offering fee-based consultation services
to train private employers in how to accommodate people with disabilities; and providing

Alameda County Behavioral Health Care Services Crisis Response Program (ACBHCS
Crisis Response Program)
The purpose of the Crisis Response Program is to reduce unnecessary hospitalizations for adults
with serious mental health diagnoses, to provide short-term voluntary therapeutic treatment and
case management to adults in crisis, and to link adults to appropriate long-term mental health
services. Serves ethnically diverse adults in Alameda County who have a serious and persistent
mental health diagnosis. Hours are Monday through Friday from 8:30 a.m. to 5 p.m.
Bay Area Community Services (BACS)
The Bay Area Community Services Center offers cost-effective community-based services to
disabled adults and seniors as well as their families. They provide mental and physical
stimulating activities, nutritious meals/snacks and door-to-door transportation. They also provide
counseling, education, support groups and daytime respite to caregivers while still ensuring
excellent care and supervision for loved ones. Serves Alameda, Contra Costa and Santa Clara
counties. Hours are Monday through Friday from 9 a.m. to 5 p.m.

Behavioral and Community Services (BCS) by Terra Nova Counseling
Behavioral and Community Services provide a broad range of counseling services, BCS staff offer group counseling in anger management, parenting and co-parenting skills. BCS houses most
of the outpatient mental health services of the agency that are outside of the Children's Services. For clients seeking therapy or counseling, therapists meet with new clients to complete an intake assessment to determine a treatment plan. The length of services depends upon the specific
program or the needs of the client. BCS is able to accept some clients on a sliding scale, we are able to offer reduced fees based on your proof of income. Talk with the person who schedules your appointment about whether you qualify. Serves Sacramento County Residents. Hours of

Berkeley Free Clinic
The Berkeley Free Clinic offers services like acute medical care, tuberculosis testing, dental
services, and sexual transmitted infection (STI) and HIV testing. Accepts Medi-Cal members.
Hours of operation are Monday, Wednesday and Friday from 7 p.m. to 9 p.m. (same-day
appointments only); Saturday from 11 a.m. to 2 p.m. (drop-in, women and transinclusive clients
only); Saturday from 3 p.m. to 5 p.m. (drop-in and HIV testing only); and Sunday from 4 p.m. to
7 p.m. (drop-in, men's STI testing and HIV testing for all).
Bridge to Healthy Families by ACC Senior Services
Bridge to Healthy Families (BHF) is a program that seeks to improve access to comprehensive
support services for family caregivers and their care receivers. The program offers care
assessments and home safety checks, care consultations and caregiver support, support groups
and educational programs. Also offering information and referral for respite services, home
modification, medical alert, and more. Hours of operation are Monday-Friday 8 a.m.-5 p.m.
Serves Sacramento County Residents.
Catholic Charities of the East Bay — Immigration Project (Oakland)
Catholic Charities of the East Bay provides immigration and naturalization services and
programs for immigrants in the East Bay area. Services include green card application and
renewal assistance, family base petitions, Nicaraguan Adjustment and Central American Relief
Act form assistance, a violence against women program, adjustment of status assistance, and
help in completing other forms. There is a $40 fee for legal counseling appointments. Hours are
Monday through Friday from 9 a.m. to 5 p.m.; closed from 1 p.m. to 2 p.m.

Child Abuse Treatment Program (CHAT) by Child and Family Institute (CFI)
With funding awarded by the Federal Victims of Crime Act, Grant AT13101111, through the California Governor's Office of Emergency Services (Cal OES), CFI is able to offer
psychotherapy services to children and youth victims of crimes. CFI is able to serve children and youth victims who do not have access to such services due to a lack of resources, lack of insurance or Medi-Cal ineligibility. Treatment is offered to victims of physical, sexual, and
emotional abuse, parental substance abuse, child abduction, bullying and domestic violence. Serves El Dorado, Nevada, Placer, Sacramento, Sutter, Yolo, and Yuba County Residents.
Community-Based Adult Services by California Department of Aging
Community-Based Adult Services is a day health program for older adults and adults with certain chronic medical, cognitive or mental health conditions, or disabilities who are at risk of needing institutional care. This program used to be called Adult Day Health Care (ADHC). CRIL offers independent living services to those with disabilities. Services include housing

charge. Hours of operation are Monday through Friday from 9 a.m. to 5 p.m.; closed for lunch
from 12:30 p.m. to 1:30 p.m.
Del Oro Caregiver Resource Center
The Del Oro Caregiver Resource Center is a nonprofit organization that serves individuals and
families who are providing care to brain-impaired adults and the frail elderly. Services include
information and referral, family consultation, family counseling, respite care, legal/financial
consultation, education and training, and support groups. Serves Alpine, Amador, Calaveras,
Colusa, El Dorado, Nevada, Placer, Sacramento, San Joaquin, Sierra, Sutter, Yolo and Yuba
counties. Most services are offered at no cost; some services require a small copayment. Hours
of operation are Monday to Friday from 8 a.m. to 4:30 p.m.
Emergency Housing-Family Shelter Program by Next Move
Family Shelter provides short-term emergency housing, life skills coaching, alcohol and drug recovery support and education, on-site SETA head start, laundry services, housing relocation assistance, case management for one year, assistance with clothing, shoes and housewares to homeless families up to thirty days. This program serves two parent families, single fathers, and sons over the age of 14. Serves Sacramento County Residents.

Emergency Youth Shelter by Wind Youth Services
Wind Youth Services provides supportive services and opportunities to youth experiencing homelessness as they pursue self-determined lives of stability and independence. Through our programs and services, Wind Youth Services provides homeless and at-risk youth between the ages of 12 and 24 with basic safety net and survival services and a safe place to stay. The Emergency Youth Shelter provides a temporary, low barrier, emergency overnight shelter for youth ages 12-17 (18 and in high school is okay). This LGBTQ-friendly, trans-affirming, pet-friendly emergency shelter is the only of its kind in the greater Sacramento area, and providing a safe, non-judgmental, and supportive community environment. Services include, hot meals, counseling, case management and more.
Fountain Recovery Drug and Alcohol Treatment
The Fountain Recovery Drug and Alcohol Treatment program offers help to individuals and
families affected by alcohol and drug addiction. The program is 12 step-oriented as well as
abstinence- and evidence-based and has a foundation of encouragement, love, practical guidance
and personal attention. Their small client-to-staff ratio allows for a client-centered approach that
offers individualized attention to meet each client’s specific needs. The program has four
different levels of care: residential treatment, day treatment, intensive outpatient treatment and a
sober living environment. Follow-up to treatment includes a free Aftercare Group for clients
who have successfully completed treatment. They accept most insurance. Hours are Monday
through Friday from 9 a.m. to 5 p.m.
Golden Rule Services
Golden Rule Services offers a variety of no cost, confidential HIV, STD and hepatitis C
education and prevention services that are accessible, affordable, and culturally and
linguistically appropriate. Services include rapid HIV and HCV testing, STD screening, needle
exchange, harm reduction services, HIV/STD risk reduction support groups, case management
services, individual peer-based risk reduction counseling, latex and polyurethane condom and
lubricant distribution, limited transportation to medical appointments, and information and

Health by Women’s Empowerment
Women’s Empowerment's mission is to educate and empower women who are homeless with the skills and confidence necessary to get a job, maintain a healthy lifestyle, and regain a home for themselves and their children. Hong Fook Adult Day Health Care Center
The Hong Fook Adult Day Health Care Center provides individualized health care services to
frail adults. They enable individuals to remain in the community and prevent premature
institutionalization. Nursing services include health monitoring, medication assistance, personal
care, health education and liaison services with their personal physician. Rehabilitation services
include dietary consultation, nutritional education and daily hot meals. Recreational activities
include field trips, music programs, and birthday and holiday celebrations. Serves Alameda and

Magnolia Women's Recovery Program, Inc.
Magnolia Women's Recovery Program, Inc. offers individual and group counseling for women
with substance abuse problems. They provide parenting classes, anger management, relapse
prevention and education classes (topics include Hepatitis C, HIV, domestic violence and
trauma). Serves Oakland County residents who are pregnant or have up to two children from
birth to 7 years of age; accepts Medi-Cal members. Program operates on a 24/7 schedule.
Mexican Consulate by Consulado de Mexico en Sacramento
The Mexican Consulate in Sacramento is appointed by the Mexican government to protect and promote the welfare and interests of Mexican immigrants living in the United States. The consulate offers a broad range of services including keeping the population up to date on relevant laws and regulation, providing a financial mailbox for Mexicans abroad that allows them to send and receive funds internationally, financial advice on problems with a financial institution in Mexico, assistance obtaining a Mexican passport. Opening Doors
Opening Doors provides tools and support for survivors of human trafficking, refugees,
immigrants and other underserved residents. Services include refugee programs to support
refugees resettle and acclimate to a new country, a new workplace environment and new health
care and financial systems; a Prosperity Project to help residents improve their economic
situation by starting or expanding small businesses and to help them take control of their
personal finances by educating them about the U.S. financial system; a Survivors of Human
Trafficking program that provides case management to counsel victims of human trafficking;
and immigration legal services offering immigration services for nominal and affordable fees.

Rehabilitation Services Program by Woodside Healthcare Center
Rehabilitation Services Program provides post-hospital care to those recovering from strokes, orthopedic interventions including fractures and joint replacements, cardio-pulmonary conditions and more. Patients suffering from varying stages of Dementia and Alzheimer's Disease are given specialized care and recreational programs in a secure environment. Long-term care provides professional nursing and therapy for chronically ill or permanently disabled individuals. Residents at this level can often take care of many or all of their routine activities, yet typically require assistance with medical treatments, medication and maintenance therapy programs. The RYSE Center offers youth programs and LGBTQ support. Programs include media, arts and
culture programs that promote community healing and social justice by facilitating young
people’s innovative creation of political, personal and expressive media and art; education and

management, academic counseling, transcript review, STEM programs, GED referrals and

Sacramento County In-Home Supportive Services (IHSS) Public Authority
The IHSS program helps pay for services to help residents remain safely in their home. Services
offered through IHSS include cleaning, meal preparation, laundry, grocery shopping, personal
care services (such as bowel and bladder care, bathing, grooming and paramedical services),
accompaniment to medical appointments and protective supervision for the mentally impaired.
To be eligible for IHSS, individuals must reside in California, have Medi-Cal eligibility
determination, live at home or an abode of his/her own choosing, and submit a completed Health
Care Certification form.
Sacramento Covered
Sacramento Covered is a community-based nonprofit organization with a focus on improving
the overall health of Sacramento county residents. Services connect individuals and families to
medical coverage, primary and preventative care, social services, and health care resources. The
main goal is to increase awareness and understanding of health care issues for uninsured and
underserved people; help the community overcome the challenges preventing them from getting
coverage; educate residents about their health care options; and offer in-person and
in-language assistance as well as provide onsite assistance for patients in local ER departments.

The Sacramento County Mental Health Treatment Center is a locked, adult psychiatric health
facility providing acute inpatient services to indigent residents and Medi-Cal members. The
treatment center operates 24/7 and offers information regarding resources to individuals
experiencing a mental health crisis. Serves Sacramento County residents.
Second Chance
Second Chance offers an emergency shelter for individuals and families. The maximum stay is
two weeks for individuals and three weeks for families. Drug and alcohol programs are also

The treatment program is certified by the Commission on the Accreditation of Rehabilitation Facilities (CARF) and has helped countless patients achieve a healthier and more productive lifestyle. MedMark Treatment Centers takes a multi-modal approach to substance abuse treatment. Services include medication-assisted treatment, substance abuse counseling, physical evaluation, coordinated pregnancy treatment plans, relapse prevention and more. The Billy DeFrank LGBTQ Community Center offers no-cost rapid HIV testing. The center also

support groups, bisexuals and friends groups, crystal meth anonymous, Latino men's groups,
