HYGIENE EXPERIENCE
The Department of Health, Baluchistan has only 4 health
education officers in the Province with one in Quetta. Health
education officers are assigned tasks which are not related
to health education. Health Education Officer in Quetta is
supervising CDD but doing no health education. Materials from
National Institute of Health are not distributed. Health
education officers have no job descriptions in this Province,
no audio-visual facilities, artist or transport.
BIAD has 12 mobile teams each consisting of a Community
Development Officer, two female para-medics — Lady Health
Visitor (LHV) and Medical Technician (MT) — responsible for
community health promotion, one sanitarian and one lady
teacher. Health and hygiene promotion are a major component
of the team's work, delivered one-on-one to mothers by the
female team members. One concrete result has been the
reported construction of 1886 latrines, covering about 38V. of
families in the project areas.
UNICEF has spent some of its resources on developing a series
of health educational materials: pinch and scoop, sugar salt
mixture, and BC6 vaccination have been produced. Others under
development include hygiene, including hand washing correct
latrine utilization, nutrition, including breast-feeding; and
TT vaccinations. None of their visuals include any words and
minimal use of tribal characteristics to permit as wide a use
as possible. Each of these materials have taken a year or so
to produce, reflecting the time consuming cycle of design,
test and revision.
As in other provinces, Baluchistan uses its vaccinators to
promote Oral Rehydration Solution (ORS). Reflecting the
scattered population of the province, it has the lowest
immunization rate and ORS usage rate for diarrhoeal cases of
any province. '
The Malaria Control Programme is organised in a two, week crash
"Spray Programme" to cover all houses in each target area.
One team includes 5 to 6 people, mainly spray labourers, and
a supervisor who visits house to house motivating families and
educating them with the help of posters and explaining the
linkages between drainage and malaria. Teams include only
male workers but do not seem to face much problem contacting
rural women as the /programme is one of the oldest and has
worked in. the same areas for more than two decades. The
District Health Education Officer is responsible for these
teams and monitors them. The Chief of this programme would
agree to integrate education on other water related diseases
(e.g. diarrhoea prevention through hygiene education) in this
programme. The Malaria Control programme appears to be useful
at least for hygiene education for rural male population.
Malaria Control Programme workers may also be useful in
dissemination hygiene education among school children, youth
groups etc.
The Population Welfare Cell, Baluchistan has 53 Family Welfare
Centres (7 urban + 46 rural). In theory, each FWC has one
LHV, 1 Female Health Worker (FHW) and one male worker who Are
also responsible for population control, motivation and
education among rural population. In reality, most of the LHV
posts are vacant. Although PWC, with the help of UNICEF and
CIDA (a federal Government project), trained 250 LHVs for the
province, it is understood that most did not stay and returned
to their home province. Those who stay do not leave Quetta
2,
and avoid the rural centres. There is no integration between
FWCs and other Health programmes. However, PWC does
collaborate with various NGOs interested in population control
and other development activities, by offering FWCs and their
staff. The 46 rural FWCs could be used for organizing
training activities for rural health workers, or organizing
hygiene education for rural women who come to these centres.
FHWs of the Family Welfare Centres may be more useful as
Master trainers to train TBAs or other community health
workers than LHVs.
Within the Expanded Programme on Immunization (EPI) a new
Provincial WHO Representative has organized training courses
for medical officers and Para-medical staff in all but 3
districts. For this training, new manuals in urdu have been
used, however, the importance of face to face communications
still appears to be under—stressed.
The AICF (L'Action Internationale Centre la Faim) Afghan
Refugee Health Programme has organized TBA training in camps.
They were trained by 1 Pakistani LHV, 2 Afghan educated women
from the camps and 1 Afghan nurse. Training was for 2 to 3
months. Only those TBAs were selected who were well
recognized by the community and who were actively practising.
After training, each TBA was responsible for Health and
Hygiene education of 10 families in the camp. Among other
things, the Female Community Health Worker (FCHW) training
curriculum includes: causes, spread and prevention of
disease; demonstration of hand-washing; clean water and
protection of wells, faeces and the importance of latrines;
how to remove rubbish; diarrhoea and ORS; and malaria.
Programme staff judged that TBAs are definitely the most
useful village based health workers but LHVs are not
interested nor sufficient in number in the Province to reach
and train the TBAs. Hence, more flexibility and relaxation
is needed for admission in Public Health Schools in
Baluchistan so that more Baluchi girls can join the
profession. L'AICF would like to work in rural areas of
Pakistan after the Afghan refugees leave. One possibility
would be to establish a school for the training of female
health workers outside Quetta, at a district level.
WASA's Sanitation Project is located in two katchi abadis near
Quetta city. As a pilot phase, WASA is trying a new approach
with emphasis on community involvement. WASA female workers
visit women individually in their homes (though in one abadi,
they also meet women in the female Vocational Skill Training
Centre). The Quran and Hadtith are used to convince women for
the hygienic use of latrines (using purity and pollution
concepts), and the importance of anal cleansing.
The Women's Cell office in P & D (Women's Division) has only
limited experience in rural areas. Women's Cell interventions
in health have included! providing female wards, extra rooms
for females in hospitals in urban areas. one mobile Women's
Mobile Health Unit, and a Mother and Child Health Centre in
Kalat. The Cell is also a funding agency to Social Welfare
Department and NGOs for establishing Female education centres,
adult literacy classes, income generation centres, which could
be used for organizing hygiene education.
Most LHVs are from Punjab and only came to do the LHV course
in Quetta and then leave after serving their bond period.
Every year Department of Health has to use new LHVs to replace
those who have left, yet still there &re many LHVs posts
vacant. Integrating hygiene education in their work may be
difficult as they are to fill several different registers for

various vertical health programmes.
The Nutrition Cell started in late 1988 and is only at pilot
stage. UNICEF has donated one vehicle, 2 typewriters and one
photocopier and is also helping to arrange Training Courses
for Female Health Workers. The programme is covering 18 urban
and sub-urban areas. The programme has trained 18 LHVs in
procedures to identify malnourishment among children, and
growth monitoring.
