• Evidence based high blood pressure management program that utilizes a tracker to empower patients to take ownership of their cardiovascular health.

• Incorporates the concepts of remote monitoring, mentoring, tracking as key features to improve HBP management, physical activity and weight reduction.

• Four month education sessions are recommended along with incentives for participation.

• Encourage participants to take weekly readings or 8 readings at least once/month over 4 months.

• Target Audience: Individuals with high blood pressure• Focus groups: Corporate and School-site Employees, patients in

clinics focusing on HBP control, community sites with opportunity to reach individuals on a continuous basis

Check. Change. Control.® was founded on successful evidence-based practices from the American Heart Association pilot program, Check It, Change It. The Check It, Change It program proved to be especially effective among the target population of African Americans (Thomas et al. (2012). Check It, Change It: A Community-Based Intervention to Improve Blood Pressure Control).

Feel free to provide monitoring and feedback such as:

• Remind the participant to take their own blood pressure readings each week for a minimum of four months.

• Identify and note to the participant which of the standard blood pressure measurement categories that the participant’s weekly readings fall into.

• Identify and note to the participant whether the participant’s blood pressure readings have increased, decreased, or remained stable.

And here are the “Don’t”s:

Volunteers are not to make any medical diagnoses about, or prescribe treatment for, the participant.

Yes. The tool will provide an initial score. As users

collect their other vital measures, they can add their

new information and get an updated score. For the

most accurate heart score, complete vitals are

needed.

What time commitment does My Life Check require of

its users?

The initial assessment is very brief, only four to five

minutes. Regular updates, that allow users to track their

progress, can be made in just a couple of minutes through

the easy-to-use online format. The tool also allows users to

accept and commit to taking additional simple steps to achieve their goals and work to improve their heart health.

How is health data entered into My Life Check protected?

My Life Check uses the highest level of security to protect

individual users’ information from data breaches. Data is

stored on a secure database managed by a highly-rated,

HIPPA compliant external vendor, and companies do not

receive any employees’ personal health information. The

system does generate reports and leaderboards that show

overall trends, but it does not include information on specific employees.

What is a Heart Health Score, and what can it tell

individuals about their health?

My Life Check uses the answers to the health assessment to generate a

Heart Health Score on a scale of 0-10. The score is grounded in the

science of AHA’s Life Simple 7 and based on users’ answers to the My Life

Check health assessment that relate to smoking status, healthy diet,

physical activity, healthy weight, blood pressure, cholesterol and blood

glucose. A score of 10 indicates ideal heart health, and a lower score

indicates there are lifestyle changes which should be made to move

individuals toward ideal cardiovascular health.

Research shows that individuals with five or more Life Simple 7 metrics in

the ideal range (a score greater than 7.14) have approximately 88 percent

lower cardiovascular mortality compared with participants who have one or no metrics in the ideal range (a score of less than 1.43).
