The Canadian Volunteer Fire Services Association annually honors a volunteer firefighter or fire officer whose outstanding achievements in the fire service and community exemplify the duties of a firefighter. Nominated by their peers, the recipients are awarded a plaque and a place in history within the Canadian fire fighting community.
Nominations must be received by June 7th, 2019. To nominate a volunteer firefighter for the CVFSA Firefighter Lifetime Achievement Award, fill out the form below and attach the nominee's resumé or background sheet, letters of recommendation, and any other supplemental material that will be helpful during the selection process. You may email this completed PDF, or a printed and scanned copy, to Mark.P@cvfsa.ca.
