ePDP includes the elements of a PDP

as well as a full templated discharge

summary.

Start this document early during the

admission to ensure smooth discharges from the hospital.

Select the unit.

Select the patient.

Select Discharge Process.

The Summary tab displays all

information that has been entered

so far.

Update the Discharge

Attending and Hospital service by searching from the drop-down menu.

Remember you can save anytime by

hitting Save at the bottom of each

section.

These 3 fields will serve as the discharge summary. If completed

here no additional summary needs to be written!

Select “DELETE ALL DATA on 3 TABS” to indicate discharge summary already

dictated.Otherwise Select “KEEP Data” to

marked as these are the discharge summary

You must enter the principle diagnosis (primary reason for

admission) before moving to the Status at DC tab

Enter the PRINCIPAL reason for admission with a description of the hospital course

Enter all other diagnoses with

associated hospital course. There are 14

fields available.

search for a provider if not defaulted in.

If PCP within DPH/UCSF system, an email will be generated advising PCP that dc documents available for review. Additional physicians can be selected in the Additional Distributions. Alternatively free text the

name of the physician if not in the DPH/UCSF system. No email will be generated to these providers.

Click the Save button located at

the bottom of each tab at any point while editing the

ePDP to save your work.

After clicking the Save button, you

will be required to enter your

password to save your work.

You or any other provider may then continue working

on the saved version at any point in time.

Review the ePDP Summary.

You can edit any section of the ePDP by clicking on the

field and then hitting modify below or go directly to the relevant tab above.

To FINALIZE the ePDP go to the

Summary tab and click Finalize

below.

IMPORTANT: Your signature on the FINAL

ePDP indicates that you are accountable for the entire content (even if multiple

providers contributed to the document over time).

ePDP is complete!

Med Rec and Discharge Instructions must also be completed

for the patient to be discharged.

To make edits to an already finalized ePDP,

go to Enter/Update PDP. Click Continue to make

changes to the document.
