out palliative care as opposed to aggressive

medical care, If they have a chaplain spiritual

provider then if they do not.

What Patients want at End of Life:

• Warm relationship with their care providers• To be listened to.• To have someone to share their fears and concerns.• To have someone with them when they are dying.• To be able to pray and have others pray for them.• To have a chance to say “goodbye” to loved ones

GOALOffer Proactive End of Life Care to Families and Patients prior to death.

A. Increased referral for end of life care including removal of care and terminal discussion. B. Content concerning end of life ministry objectives.

Proposals• Hospital wide discussion on End of Life Care. • Create a protocol for calling spiritual care for all End of Life Events. • Increase visibility of “No One Dies Alone”.• Provide Resources for Patients and Families.
