Open a New Form in Google Drive

Name It

Select Question Format

Add Questions

Make the Answer Required

Format Form

Select Who Can Respond (Paid Accounts Only)

Click the 3 Dots to Select a Destination

Response Sheet on Drive

Create a Link to Insert in Email

Compose New Email and Hit Insert Tab

Hit Hyperlink Icon

Name the Link, Paste URL and Click OK
