HOW TO REGISTER FOR PATIENT PORTAL
A STEP-BY-STEP GUIDE
Before registering, please make sure you have:
- Your Student ID Number- Your UNCP email address
1.
If you have not set up an account, click “Register”

Fill out registration information.
It may help you remember, if you choose the same
username you use for Bravemail

Click submit.
You will then be asked to check your email to validate/confirm your account. Log into
your UNCP email.

Click the link provided in the
email to log into your NEW Patient
Portal account.

Enter your new username and password you created.
Log in

Congratulations! You are now registered.
Make sure to write down your user name and
password.

Use the top tabs to
navigate the Portal.

Entering Immunizations
If you enter your immunization information
in online, don’t forget to send a hard copy to
Student Health Services for verification.
Enter your immunization history. Don’t forget to hit “submit” in order to save.
After submitting, see what you’re missing.

Record
Click the “Upload” tab, and upload your
immunization record

Health History
Click the “Forms” tab to complete your UNCP Health History
Make sure it says “Completed”
