Incident Management
February 2020

What is incident management?
• What are the risks associated with doing the work?
• How do you go about determining the risks and managing them?
• Need for prioritization to address risks

Definitions
JOB HAZARD ANALYSIS Process of examining each step of a job to identify the hazards, severity, probabilities, risk, and controls
HAZARDSomething that produces an undesired outcome
ROOT CAUSE FACTORSAll factors contributing to the occurrence of an incident, accident, near miss (near hit)
CORRECTIVE ACTIONSSteps taken to mitigate the causal factors that contributed to the occurrence of an incident, accident, near miss
JOB SAFETY OBSERVATIONSProcess of observing a worker to determine whether work is being performed safely
SeverityThe degree of
seriousness of the undesired outcome
ProbabilityThe likelihood that
the undesired outcome will occur
RiskSeverity multiplied by
the probability

• Need a systematic approach to Incident Management
• Do you have a system?
• What systems are available to help you manage incidents?

Prepare for the possibilities
ADD A FOOTER
PRE-PLANNING
• Analyze the operation step by step
• Identify hazards
• Assign severity and probability at each step
• Calculate Risk

PRE-PLANNING
• What controls are in place/needed?
• Engineering
• Administrative (Written Programs,
Equipment Inspections, Training,
Performance Evaluations, Job Safety
Observations, etc.)
• PPE
Controls
PRE-PLANNING
• How do you establish priorities?
• Risk based approach (Highest Risk First)
• Reactive vs Proactive Approach?
• Factors affecting prioritization
• Available resources
• Degree of impact
• Timelines
• Government oversight

PRE-PLANNING
• Develop documented Job Safety
Observations
• Based off Job Safety Analysis
• Objective is to observe workers and ensure
tasks are done safely
• Incident Management Systems must
incorporate the job safety observations
• Can be conducted prior to and after an
incident
Job Safety Observation

PRE-PLANNING
• SDS must be maintained in a systematic
manner per OSHA
• Incident Management System can
incorporate SDS
• Pros – Readily available, systematically
organized, paperwork reduction, don’t lose,
updated and current

PRE-PLANNING
• Needs based and customizable
• Searchable
• Multi-user accessible from kiosk
• Print secondary labels
• Generate Reports

Incident
Even with all the preplanning - mistakes happen
Injuries
• Using an Incident Management System to manage a serious forklift accident
• Circumstances
•Untrained Forklift Operator
•Injury to hand
