Radon Warning Statement
Every buyer of any interest in residential real property is notified that the property may present exposure todangerous levels of indoor radon gas that may place the occupants at risk of developing radon-induced lung cancer.Radon, a Class-A human carcinogen, is the leading cause of lung cancer in non-smokers and the second leadingcause overall. The seller of any interest in residential real property is required to provide the buyer with anyinformation on radon test results of the dwelling showing elevated levels of radon in the seller’s possession.
The Illinois Emergency Management Agency (IEMA) strongly recommends ALL homebuyers have an indoor radontest performed prior to purchase or taking occupancy, and mitigated if elevated levels are found. Elevated radonconcentrations can easily be reduced by a qualified, licensed radon mitigator.

Lead Warning StatementEvery purchaser of any interest in residential real property on which a residential dwelling was built prior to 1978 is notified thatsuch property may present exposure to lead from lead-based paint that may place young children at risk of developing lead poisoning. Lead poisoning in young children may produce permanent neurological damage, including learning disabilities, reduced intelligence quotient, behavioral problems, and impaired memory. Lead poisoning also poses a particular risk to pregnant women. The seller of any interest in residential real property is required to provide the buyer with any information onlead-based paint hazards from risk assessments or inspections in the seller’s possession and notify the buyer of any known lead-based paint hazards. A risk assessment or inspection for possible lead-based paint hazards is recommended prior to purchase.

KEYBOX AUTHORIZATION ADDENDUM
The undersigned (Seller(s)) having entered into an Exclusive Marketing agreement with
(BROKER), (Company Name and Address)
pertaining to the sale of (PROPERTY), hereby authorizes BROKER to use a keybox in the marketing of Seller(s) Property. Seller(s) acknowledges: 1. A keybox is designed as a repository of a key to the above Property, permitting access to the interior of the
Property by Participants or Subscribers of the Multiple Listing Services (MLS), and their authorized licensees. 2. Broker is authorized to affix a keybox to the Property and, provided the owner is absent, any MLS Participant or
Subscriber associated with the Multiple Listing Service(s) whether acting as Buyer’s agent or otherwise shall have the right, through use of said keybox, to show the Property at any reasonable time.
3. It is not a requirement of the Multiple Listing Service or Broker that a Seller allow use of a keybox. 4. Seller acknowledges that neither listing nor selling Broker, the Mainstreet Organization of REALTORS®
