Our February meeting started off with an
update from the president of the Oregon Dental
Association, Joni Young, DMD. The new
liaison for Marion Polk Dental Society is Mark
Mutschler, DDS.
The 2016 Legislative session will be short
but Dr. Young went over some of the ODA’s
top priorities.
#1 Medicaid Loan reimbursement-this is to
help get dentists in underserved areas.
#2 Dental Board – after 10 years, a clinician
can apply to have their name taken off for
minor infractions (non-clinical infractions).
She mentioned the board is against it because
they say it will take too much money to update
the website.
#3 Mid-level provider-pilot program approved
by the Oregon Health Authority. Services will
be on tribal lands.
#4 Tele-dentistry-put out by OHSU. This is
still in approval process but dentist will make
the diagnosis and walk hygienist through
treatment.
2015 update:
#1 Senate bill that makes sure the dental
director, is always a funded position. With a
state dental director we will have more
successful dental programs.
We then got to hear from Dr. Priya Kansal
about cardiology issues in dentistry. Dr. Kansal
works for NW Permanente at the Salem
Hospital. She started out talking about
dual-antiplatelet therapy. Dual anti-platelet
therapy is Aspirin along with one of the
following agents: Plavix, Effient or Brilinta.
Dual anti-platelet therapy is mandatory for one
year after placement of a drug-eluting stent,
regardless of whether post-MI or elective.
Select individuals may require dual
anti-platelet therapy for life based on their risk.
Risk factors can include the location of the
stent or prior history of stent thrombosis. A
study that looked at extractions in patients on
antiplatelet agents found that most dental
extractions in coronary stenting patients can be
carried out safely without stopping multiple
antiplatelet agents.
Dr. Kansal then talked about systemic
anticoagulation. Indications for anticoagula-
tion include atrial fibrillation with moderate to
high risk of stroke, left ventricular thrombus,
mechanical heart valves, deep vein thrombosis
and pulmonary embolism. The anticoagulant
that we hear about the most is Coumadin
(Warfarin) but the new ones on the market are
Pradaxa, Xarelto and Eliquis. These newer
drugs have a shorter half-life and no
monitoring is required. A study showed that
most procedures can be performed on Warfarin
without increased bleeding risk.
To bridge or not to bridge, that is the
question? Dr. Kansal talked about bridging
patients on Warfarin with a low molecular
weight heparin.

Studies showed that bridging patients who are at low risk can actually increase bleeding.
Bridging should be reserved for the highest risk patients such as those with mechanical valves,
patients with atrial fibrillation and prior history of stroke and recent thrombotic event within 3
months.
We got a nice review on the requirements for antibiotic prophylaxis. Indications for
antibiotic prophylaxis are for those patients with prior endocarditis, prosthetic heart valves,
cyanotic heart disease and cardiac transplant with valvular abnormality. The recommended
regimen for prophylaxis is Amoxicillin 2 grams or if allergic to penicillin, Cephalexin 2 grams.
The recommendations for treating a patient with heart failure include prevention of blood
pressure and heart rate fluctuation which may lead to decompensated heart failure. Also, try to
prevent arrhythmias by using anesthetic without epinephrine.
When treating patients with cardiac devices such as pacemakers and defibrillators be aware
of electromagnetic interferences from certain dental equipment. If equipment is greater than
5cm from device there should be no interference. No studies have shown any permanent
damage from dental equipment when tested near cardiac devices. Antibiotic prophylaxis is not
needed for these patients.
Patients with high blood pressure often have oral side effects from their medication. Some
of these side effects are xerostomia, gingival hyperplasia, lichenoid reaction and dysgeusia. A
normal blood pressure range is 100-130 systolic/60-80 diastolic. Hypertension is defined as
greater than 140/90 or 135/85 in the presence of diabetes or chronic kidney disease. Local
anesthetics with vasoconstrictor should be avoided or used in low doses in patients taking
nonselective beta-blockers (propranolol or carvedilol), uncontrolled hypertension, refractory
arrhythmias, MI or stroke less than 6 months, unstable angina, coronary artery bypass graft
under 3 months and congestive heart failure. The maximum does of epinephrine in a patient
with cardiac risk is about two cartridges of LA with 1:100,000 epi or 4 cartridges with
1:200,000 epi. No procedures should be done on a patient with a blood pressure of 180/110 or
higher.
When in doubt contact your patient’s cardiologist before starting any treatment.

New Smart Mouthguard Detects
Head Injuries.
SportTechie (1/20, Turner) reports that Force Impact
Technologies has launched the FITGuard, a head injury
awareness mouthguard that will assist athletes with their timing
of returning to play when there is a possibility of head injury.
The mouthguard has color-coded LEDs embedded in it to
indicate how much impact force an athlete receives.

program originated in St. Louis,
Missouri in 2002. Throughout the
country approximately 10,000 dentists,
30,000 dental team members and other
community volunteers will be providing
free oral health care services including
exams, cleanings, fluoride treatments,
sealants, fillings, crowns, and
extractions, to more than 300,000
children from low-income families.
New Toothpaste Fights Tooth Decay By Turning Plaque Green.
The Wall Street Journal (2/8, Johannes, Subscription Publication) reviewed a new toothpaste
that aims to improve oral hygiene by binding to plaque and showing it as green, revealing areas
a person missed while brushing. The article stated that researchers at the University of Illinois at
Chicago conducted a short-term study of Plaque HD, finding plaque was reduced by 51.3%
after people brushed with the toothpaste for up to 10 days. American Dental
Association spokeswoman Dr. Mary Hayes has not tried the toothpaste, but said it
could serve as a consistent reminder to patients. Dr. Hayes added that areas between
the teeth and at the gum line are common areas patients miss.
Michigan Dentist Office To Offer Cheek
Swabs For Bone Marrow Donor Registry.
The AP (1/26) reports that a Michigan dentist office
will begin offering patients the opportunity to help
fight blood cancer when they get their teeth cleaned.
According to the article, the dentist office, in
collaboration with the organization Delete Blood
Cancer DKMS, will ask patients if they
would like to submit a cheek swab to register
as a potential bone marrow donor in the
national registry.

NHL Increasingly Focused
On Players’ Dental Health.
Many sources are carrying an AP
(2/10, Lage) story reporting that
although the National Hockey
League is scrutinizing player safety
more than ever, many NHL players
still lose teeth as part of the game.
Still, there is increasing interest in
helping players keep their original
teeth, and when players do get hit in
the mouth with a stick or puck
during a game, they are moments
away from getting professional
treatment from a team dentist and
perhaps an oral surgeon. Several
teams, including the Nashville
Predators and Los Angeles Kings,
are equipped with a dental chair in
their arenas. The article adds, Dental
care is one of the progressive moves
made by the NHL, according to
Predators general manager, David
Poile.

New Dental Clinic Now Open In Des
Moines High School. KCCI-TV Des Moines, IA
(1/27, Victor) reported on its website and during a
broadcast that a new dental clinic has opened inside
Scavo High School. According to the article, The
clinic, which is the first of its kind in Iowa, provides
cleaning, filling and teeth removal services.

HPV May Be Linked To Higher Risk Of Head And Neck Cancer. NBC News (1/21, Fox) reports on its website that research suggests the human papillomavirus (HPV) raises the chances someone will get head and neck cancer by at least sevenfold and maybe much more. The findings were published in JAMA Oncology.
Oncology Nurse Advisor (1/21, Hoffman) reports that the investigators found that detection of oral human papillomavirus 16 (HPV-16) was associated with an approximately 22 times higher risk for developing oropharyngeal squamous cell carcinoma (SCC).
But, Healio (1/21, Kelsall) reports, this association did
not persist for oral cavity squamous cell carcinoma...or for
laryngeal squamous cell carcinoma.

Could Help Dentists,
Doctors Detect Cancer.
Gizmodo (1/26, Condliffe)
reports that University of
Washington mechanical
engineers have designed a
hand-held microscope that
could help dentists and
doctors identify cancer cells
in just a few moments.
According to the article, the
device is said to be capable
of allowing a trained medic
to identify cancerous tissue,
and a dentist, for example,
could use it to quickly check
if an abscess was cancerous
to minimize the concerns of
a patient.

Peer Review
Peer Review ensures fairness to all parties through individual case consideration and a
thorough examination of records, treatment procedures and results. This free dispute resolution
system often results in solutions which are satisfactory for both parties.
ODA member dentists are required to participate in a peer review process for equitable
resolution of patient complaints about dental treatment.
When patients are not satisfied with dental treatment, they should first talk to their dentist to
resolve the problem. If talking to the dentist does not resolve the issue, a patient can contact the
ODA Peer Review Director at peer@oregondental.org or by calling 503-218-2010 or toll free
1-800-452-5628 to learn specifics details about the process and to obtain a Peer Review
Request form.
The association may refer the case to a local committee of dentists. The committee will
respond initially by assigning the case to a dentist who serves as a mediator. If mediation does
not result in resolution, the case then goes to a full peer review committee hearing where the
patient and the dentist are allowed to present evidence or information. The committee
examines the evidence/ information and makes a decision.

ADA Morning Huddle: Mouth Sores
May Indicate Difficult-To-Diagnose
Condition. In an article for the Philadelphia
Inquirer (2/7), Dr. Eric T. Stoopler, an
associate professor of oral medicine, discusses
a patient who suffered from treatment-resistant
mouth sores and skin lesions and died of
respiratory failure about a year after he first
noticed the mouth sores. Before his death,
immunological blood tests suggested the
patient’s mouth sores and skin lesions were
actually symptoms of an condition called
paraneoplastic pemphigus, usually caused by
an underlying cancer, and a biopsy revealed
lymphoma. Dr. Stoopler said that this patient’s
case underscores the importance of regular
dental exams and prompt evaluation of oral
sores that don’t heal quickly on their own,
adding that these lesions may be the first signs
of a possibly life-threatening condition.

Medicare Deadline Approaching Dentists have until June 1, 2016 to enroll in Medicare or opt out; otherwise, Part D prescription
drug claims for Medicare eligible patients will not be covered. If you choose to take no action,
there will be a 90 day grace period the first time you prescribe for a patient covered under Part
D. We recommend that dentists submit enrollment applications or opt-out affidavits 90 days
prior to the enforcement date to allow time for processing.
Dentists who prescribe Part D covered drugs to Medicare beneficiaries must take one of the
following three actions in order for these prescriptions to be covered by Medicare.
1. Enroll as an ordering/referring provider (855O)
2. Enroll as a Medicare provider (855I)
3. Opt out of Medicare
It is important to note that if a dentist chooses to opt out of Medicare, the dentist and/or patient
cannot receive payment from a Medicare Advantage (MA) plan. Many dentists may unknow-
ingly treat MA patients.

People Receiving Care At Dental Clinics In Oregon.
KEZI-TV Eugene, OR (2/4, Richardson) reported on its website and during a broadcast that
nine volunteer dentists along with hygienists at the South Lane Children’s Dental Clinic in
Cottage Grove, Oregon provide dental care to children in kindergarten through 12th grade from
the South Lane, Creswell, Crow Applegate, Lorane and North Douglas school districts. Dr.
Lonn Robertson, a Springfield dentist who volunteers at the clinic, said, “It is very fortunate to
have a clinic like this, and again, have people who will volunteer and do all they can. The
unfortunate part is that the need is so great.”
Free Dental Care Provided At Homeless Health Day In Oregon.
The Portland (OR) Tribune (1/27, Weinberger) reported that Wallace Medical Concern hosted
its first Homeless Health Day on Jan. 19, where medical and dental professionals from Kaiser
Permanente, Medical Teams International, Northwest Mobile Hygiene, Project Access Now,
Hands of Favor and other organizations volunteered their time to provide medical and dental
services. At the event, 18 people received dental work, the article added.
