New Report Formats

This module will explain the new report formats for the 353 and758.

Reports in new format

• 353 – Number of Services by Cost Center by Billing Code

• 758 – Radiology/Pathology/Lab Services by Cost Center by Billing Code

• 439 – Positive Pregnancy log – coming soon.

Why the new format?

• The 353 & 758 are by payor code. With all the new Medicaid payor codes, there is not enough room on a piece of paper.

How are they named in E-reports
