Reduce Travel Lane Blockages Improve Peak Hour Travel
Reduce Congestion Pilot New Curbside Regulations
NYC DOT will pilot curbside access restrictions on Roosevelt Ave. from Broadway to 90th St.:
• DOT will restrict curbside parking and loading on both sides of the street
during weekday peak hours (7 - 10 a.m. and 4 - 7 p.m.)
• Regulations allow for expeditious pick-up and drop-off of passengers
• Regulations allow for deliveries to off-street loading docks
• NYPD will assign additional staff to enforce curb regulations
• DOT will monitor operations and reassess program after six-month pilot
Implementation starts in mid-March

NYC DOT pondrá a prueba restricciones de acceso a las aceras de Roosevelt Ave., desde Broadway hasta 90th St.:
• DOT restringirá el estacionamiento y la carga junto a las aceras, a ambos lados de la calle, durante las horas pico de los días de semana (de 7 a 10 a.m. y de 4 a 7 p.m.)
• Las regulaciones permitirán recoger y desmontar pasajeros de manera rápida
• Las regulaciones permitirán las entregas en las zonas de carga fuera de las calles

To have your tax returns prepared at a VITA (Volunteer Income Tax Assistance) tax site,
please bring the following information with you:
• Proof of identification - Government issued photo ID (State ID, Driver’s License, or Passport)
• Social Security cards for you, your spouse and dependents or a Social Security number verification letter issued by the Social Security Administration
• An Individual Taxpayer Identification Number (ITIN) assignment letter may be substituted for you, your spouse and your dependents if you do not have a Social Security number
• Proof of foreign status, if applying for an ITIN
• Birth dates for you, your spouse and dependents on the tax return
• Wage and earning statements (Form W-2, W-2G, 1099-R,1099-Misc) from all employers
• Interest and dividend statements from banks (Forms 1099)
• A copy of last year’s federal and state returns, if available
• Proof of bank account routing and account numbers for direct deposit such as a blank check
• To file taxes electronically on a married-filing-joint tax return, both spouses must be present to sign the required forms
• Total paid for daycare provider and the daycare provider’s tax identifying number such as their Social Security number or business Employer Identification Number
• Forms 1095-A, B or C, Affordable Health Care Statements
• Copies of income transcripts from IRS and state, if applicable

Catholic Migration Services, a non-profit organization associated with the Roman Catholic Diocese of Brooklyn, with offices in Queens and Brooklyn, has provided free legal advice and representation since 1971. Eligibility: Women aged 40 and older with health insurance
Women aged 50 and older without health insurance
Currently living in New York City No Mammogram in the past 12 months
