Prosthetic Management / Paediatric

presentation• Brief about prosthetic in paediatric • Case study• Short video(case study)

Prosthetic Management / Paediatric

Background – important statistics

• Around 4,500 children suffer from limb deficiency yearly• 1,500 babies born with limb reduction every year• Congenital limb reduction twice as prevalent as traumatic amputation• Ratio of upper amputation to lower limb amputation 1:4

Prosthetic Management / Paediatric

What is prosthesis• An artificial substitute or replacement of a part of the body such as a

tooth, eye, the leg, an arm, etc. • A prosthesis is classified into functional and aesthetic reasons or both.

Prosthetic Management / Paediatric

History

• First recorded illustration of prosthetic replacement appears in the Rig-Veda, a religious text written in Sanskrit, compiled between 3,500 and 1,800 B.C. in India.

• The 5th Egyptian Dynasty (2750-2625 B.C.); archaeologists have unearthed the oldest known splint from that period.

• In the early 1500’s, French military doctor Ambroise Paré invented a hinged mechanical hand and prosthetic legs with locking.

Prosthetic Management / Paediatric

Prosthetic management depends on…• Pre prosthetic fitment • Post prosthetic fitment

When & why pediatric prosthetic start ?

At the age of 4 months prosthetic intervention can be started….1. Ideal age (developmental milestone)

2. Adaptive (part of his body to help in crawl, grasp, and hold)

Prosthetic Management / Paediatric

3. Not to develop alternative skills.

Prosthetic Management / Paediatric

Different levels of prosthetic management in Pediatric

• Partial hand• Wrist disarticulation• Below elbow• Elbow disarticulation• Transhumeral (above elbow)• Shoulder disarticulation

Prosthetic Management / Paediatric

Type of prosthetic managementa) Passive Cosmetic prosthesesb) Body powered prosthesesc) Myoelectric prosthesesd) Bionic arme) 3D printed prostheses

Prosthetic Management / Paediatric

a) Passive cosmetic prostheses• Mitts• Gloves• Fingers• Modified terminal device

Prosthetic Management / Paediatric

b) Body powered• Functional hand(3 jaw chuck)• Hooks• CAPP( Child Amputee Prosthetics Project)

Prosthetic Management / Paediatric

c) Myoelectric Prostheses• The myoelectric prostheses are controlled by contraction of specific muscles triggering prosthesis

movement through electromyographic (EMG) signals. • These prostheses have a stronger pinch force, better grip, and are more flexible and easier to use

than conventional hooks.• After 2years

3D printed prosthesis:• New tech In 3D printing, successive layers of material are formed under computer control to create 3D object like hand , arm shell etc

Prosthetic Management / Paediatric

d) Bionic arm: electrodes are directly connected with nerve supply ( by surgical procedure)

• Advanced• Rare

Prosthetic Management / Paediatric

What Mobility India does…• Drawing a balance between expectations and logical

achievable results• Artificial substitutes can never replace a real segment or

feature.
