RE: CROWN CASTLE WIRELESS COMMUNICATIONS FACILITIES IN PUBLIC RIGHT-OF-WAY Crown Site: CA-PHS07m2 Address: 355 Jerome Ave. Piedmont, CA Applicant: Crown Castle NG West LLC / SureSite Consulting ENGINEER: Mountain LTD Mike Rivera 19 Yarmouth Dr. Ste. 301 New Gloucester, Maine 04260 Project Description and Statement Crown Castle NG West LLC ("Crown Castle") is submitting the accompanying complete application to install its telecommunications network facilities in accordance with your code, ordinances and regulations. Crown Castle is proposing to install a Small Cell wireless facility on an existing utility pole in the PROW. Install strand mount kit #006d on existing utility pole. Install (1) db362nxd3s-m db spectra antenna. Install (2) ak009a shroud kits with (4) ericsson 2203 radios inside at 9' 6". Install 2" schedule 80 power feed riser (flush mounted to pole). Install 4" schedule 80 comm riser (flush mounted to pole). Iinstall smart meter at 7'-0". Install power disconnect at 8'-6". Install vertical ground rod. Crown castle fiber at 24'-0"· Install (1) 16m down guy 1'' anchor rod 15' lead west. There is no noise producing equipment being proposed at this pole location. Top of pole will remain at 38’-6” after installation. All proposed equipment on wood pole will be painted to match the existing color of the wood pole. Paint finishes are non-reflective to minimize visual impact and durable for outside installation. Crown has made all efforts necessary to make all equipment stealth as possible. A telecommunications facility mounted to existing poles, whether streetlight or utility poles, qualifies as a collocation, regardless of whether the pole hosts an existing facility. As the FCC recently declared, “for the purposes of the Section 332 shot clocks, attachment of facilities to existing structures constitutes collocation, regardless whether the structure or the location has previously been zoned for wireless facilities. Crown considers this application a collocation. Crown Castle has agreed to allow co-locations at this facility for future carriers. The current equipment design is based off a single carrier. Per Public Works Excavation permit, our traffic control plans provide details regarding traffic coordination during construction. Crown will aim to minimize the impact to traffic and circulation by scheduling work during low traffic times and days. Once approval is obtained, a construction schedule can be provided. Per EME Study provided two sectors of the antenna will be used. The third sector will not be used at this location. Crown Castle is a public utility that is authorized by the FCC and the California Public Utilities Code § 7901 that grants a statewide franchise to telephone corporations to place telephone equipment in the public rights of way. This site is in compliance with FCC standards. Crown Castle provided an EME study which complies with the prevailing standards for limiting public exposure to radio frequency energy and will not for this reason cause a significant impact on the environment. The highest calculated level in publicly accessible areas is much less than the prevailing standards allow for exposures of unlimited duration. Crown Castle’s finding are consistent with measurements of actual exposure conditions taken at other operating nodes. All construction activities comply with the recommendations of the arborist report as it pertains to protection of the existing trees, shrubs, and root systems related. Construction crew to follow procedures entailed within report to ensure no damage will occur per the arborist notes. All efforts will be made to restore landscaping to the original condition.
The site of the proposed facility is located in a public right-of-way. This particular location falls within Zone A. We are utilizing existing infrastructure that is designed specifically to a "minimum functional height”. These small cell facilities designs are only intended to cover a small area, and presents the least visual impact possible to the community. This project will provide improved wireless telecommunication service to the surrounding Piedmont area. Please be advised the Federal Communications Commission (FCC) has adopted Rules and Regulations that impact how you must process this application. In addition, state law also limits your regulation of Crown Castle's access to the public rights of way. Pursuant to the California Public Utility Commission, Crown Castle has been granted a certificate of public convenience and necessity ("CPCN"). As a result, Crown Castle must be granted access to the public rights of way in the same manner and on the same terms applicable to other certificated telecommunications providers and utilities. The PUC is the lead agency under CEQA, not the City. As the PUC determined, and as the courts have repeatedly held, these facilities are exempt from CEQA under the Class 3 Exemption. Subject: Arborist Report – Location CA-PHS07m3
Small Cell Wireless project, Piedmont CA
Dear Mr. Miller:
Crown Castle is proposing to install small cell wireless communications devices at 19
locations in Piedmont. HortScience | Bartlett Consulting (Divisions of the F. A. Bartlett
Tree Expert Co.) was asked to prepare Arborist Reports for each of the 19 locations to
meet the City of Piedmont requirements. This letter responds to that request.
Overview
The proposed new antenna installations would either use existing city street light poles or
power poles or install new poles that will support the antenna. Some sights require
installation of an underground vault that will hold equipment. Trenching between the
pole/antenna and the vault, as well as between the vault and nearby utility poles, may be
required where a vault is installed. In this case, the antenna measures 2’ tall by 10”
across.
Potential impacts to trees (including both City street trees and trees on private property)
would be associated with 1) trenching between the vault and the pole, and 2) installation
of new light/utility poles. Trenching may sever roots and equipment working in close
proximity to trees may damage trunks and/or require pruning of tree crowns to provide
clearance. New poles must be lifted from a horizontal to vertical position which may also
damage tree trunks and require pruning ahead of pole installation and/or following pole
installation, if branches are damaged.
Description of Site and Trees
Site CA-PHS07m3 is located across from 344 Jerome Ave., near the corner with Hill
Lane. An existing utility pole was located near the curb on the southwest side of Jerome
Ave. will be used to support the wireless antenna and equipment.
Two trees were located on either side of the existing utility pole, both of which were
London plane trees (Platanus x hispanica). Tree #1 was on the southeast side of the
pole and #2 was on the northwest side. Approximate locations of trees are shown on the
Tree Assessment Plan (see attachments).

Arborist Report
Crown Castle - Piedmont small cell wireless project
Both trees were semi-mature,
measuring 13” and 12” in trunk
diameter, respectively. Tree #1
was in good health (rated a 4 on
a scale of 0 to 5, where 0=dead
and 5=excellent) and #2 was in
fair condition (rated a 3). Both
trees had been topped for the
overhead utility lines and #2 had
a moderate amount of twig and
branch dieback present in the
crown. Tree #1 had displaced
the adjacent sidewalk by
approximately 4”.
The distances between the trees
and the utility pole were
measured at 6’ for tree #1 and
28’ for tree #2.
Crowns were measured at 15’ to
the northwest for tree #1 and 16’
to the southeast for tree #2. In
this case, the canopy of tree #1
had grown around the existing
utility pole (Photo 1).
Photo 1: Looking south at London plane #1 and the
existing utility pole that will be usd to support the
wireless equipment. Tree #1 was semi-mature and
in good condition. The pole was located ~6’ from the
tree and the crown extended ~15’ in that direction.
Evaluation of Impacts
In this case, the existing utility pole will remain and a 2’ antenna will be mounted at 23’
above the ground, with all the other necessary equipment attached to the pole. The new
antenna will be mounted on a 5’ long arm, which will extend to the northwest, away from
tree #1. A new guy wire will also be added to the existing pole at 23’, extending 14’ to the
northwest. No vaults or trenching are proposed at this site.
Based on my assessment of the plans, impacts to tree #1 would be associated with the
crown pruning required to install the new antenna, associated equipment and guy wire.
Assuming a minimum of 5’ clearance from the pole would be required to install the
antenna and equipment, I would estimate that approximately 10% of the crown of tree #1
would need to be pruned on the northwest side.
Tree #2 would be outside the impacts of the proposed construction and no crown pruning
is anticipated. I recommend both trees be preserved.

Arborist Report
Crown Castle - Piedmont small cell wireless project
Summary
I evaluated the possible impacts to two (2) trees from installing a new 2’ antenna,
associated wireless telecommunications equipment and a guy wire at site CA-PHS07m3,
in Piedmont CA. No vaults or trenching are proposed at this site.
In this case, impacts would primarily involve canopy pruning on the northwest side of tree
#1. Tree #2 is considered outside the impacts of the proposed project and no crown
pruning is anticipated for tree #2. Preservation of trees is predicated on adhering to the
Tree Protection Guidelines that follow.
Tree Preservation Guidelines
The goal of tree preservation is not merely tree survival during development but
maintenance of tree health and beauty for many years. The response of individual trees
will depend on the amount of excavation and grading, the care with which demolition is
undertaken, and the construction methods.
The following recommendations will help reduce impacts to trees from the proposed
construction and maintain and improve their health and vitality through all phases of the
process.
1. The demolition contractor shall meet with the Consulting Arborist before
beginning work to discuss work procedures and tree protection.
2. Where fencing is not an option, protect tree trunks from incidental damage during
demolition by wrapping the trunks of street trees to a height of 8’ with straw
wattle and orange snow fencing to provide a visual cue and protection from
incidental contact. As an alternative, you can stack and secure hay bales around
the trunk to a height of 8 feet.
The TREE PROTECTION ZONE shall be defined by the limit of the dripline in all
directions.
3. Damage to tree(s) or unauthorized removal is subject to replacement or fine
equal to the estimated value of the tree.
4. No materials, equipment, vehicles, spoil, waste or wash-out water may be
deposited, stored, or parked within the TREE PROTECTION ZONE.
5. Where needed, prune trees to be preserved to clean the crown and to provide
clearance. All pruning shall be done by a State of California Licensed Tree
Contractor (C61/D49). All pruning shall be done by Certified Arborist or Certified
Tree Worker in accordance with the Best Management Practices for Pruning
(International Society of Arboriculture, 2002) and adhere to the most recent
editions of the American National Standard for Tree Care Operations (Z133.1)
and Pruning (A300).
6. All tree work shall comply with the Migratory Bird Treaty Act as well as California
Fish and Wildlife code 3503-3513 to not disturb nesting birds. To the extent
feasible tree pruning and removal should be scheduled outside of the breeding
season. Breeding bird surveys should be conducted prior to tree work. Qualified
biologists should be involved in establishing work buffers for active nests.

7. Demolition of existing improvement such as pavement shall use appropriate size
equipment to perform the task and protect the tree from damage. Equipment
shall be sited outside the TREE PROTECTION ZONE. Pull spoil and debris away
from the trees. If necessary, tie back branches and wrap trunks with protective
materials to protect from injury as directed by the Consulting Arborist.
8. Excavation shall not tear or rip tree roots 2 inches or greater in diameter. As the
operator encounters tree roots, excavation should stop while the root is exposed
by hand and cut cleanly at the edge of excavation.
9. Demolition personnel shall not prune trees to provide clearance. If pruning is
needed, a qualified arborist shall perform the task at the direction of the
Consulting Arborist.
