Das Traum-Szenario der Personalisierten Medizin
• Fortschritte in der Genomtechnologie ermoeglichen die Bestimmung individueller Genomsequenzen
• Vergleiche genomische Sequenzen (z. B. gesundes und Tumorgewebe) und lese die Unterschiede
• Erkenne daraus den Krankheitstyp und waehle die beste Therapie
Zwischenbilanz
• Fortschritte in der Genomtechnologie ermoeglichen die Bestimmung individueller Genomsequenzen, schnell, billig und zuverlaessig
• Die Erwartungen die die Genomsequenz geweckt hat wurden nicht wirklich erfuellet (in Bezug auf Medizin)
• Die Innovationen der Genomik fanden nicht in der Schweiz statt • Das Konzept der Personalized/precision Medicine wurde entwickelt – sehr
stark basierend auf genomischer Information
• Warum nicht? Zu frueh?, nicht genuegend grosse Studien? Ungenuegende biologische Grundlagen?

BRCA 1: Ein “Gen fuer Brustkrebs”
• Google hat > 3.5 Mio Eintraege ueber BRCA1 und Brustkrebs
• Mutationen im sogenannten “Brustkrebsgen” BRCA1 werden in bis zu 1% der Frauen in bestimmten Populationen gefunden
• Die Praesenz von BRCA1 Mutationen erhoeht das Risiko fuer Brustkrebs bis
zu 5-fach
• Jedoch: “Nur” 60–85% der Frauen mit BRCA1 Mutationen werden Brustkrebs entwickeln (60–85% Penetranz). D.h. fuer 15–40% ist das nicht der Fall.
• Viele Frauen mit Brustkrebs haben kein mutierted BRCA1 Gen
• Kommerzielle Tests fuer BRCA1 Mutationen sind erhaeltlich (und teuer)
• Actionability der Testresultate?

Tonight, I’m launching a new Precision Medicine Ini8a8ve to bring us closer to curing diseases like cancer and diabetes -‐-‐ and to give all of us access to the personalized informa8on we need to keep ourselves and our families healthier."
Was haben wir gelernt?
• Personalized/precision Medicine bezeichnen eine neue Medizin in der Entscheide ausgehend der persoenlichen molekularen Profile gemacht werden
• Personalized Medicine war bisher wesentlich auf der Genomik aukauend, mit maessigem Erfolg
• Wir koennen das Genom lesen aber die Informa8on nicht verstehen. Daten sind nur nuetzlich wenn ein Paradigma fuer ihre Analyse besteht
• Die Umsetzung eines Genotypen in Phaenotypen wird durch Proteine und ihre Interak8onen katalysiert
• Neue Technologien erlauben es uns das Proteome schnell und praezis zu messen und damit den molekularen Phaenotypen zu bes8mmen
• Die Schweiz ist weltweit fuehrend in der Entwicklung der Proteomik • Viel basic Science Forschung ist dringend noe8g • Die oekonomischen Konsequenzen sind unbekannt
