Password Scheme:
This is optional, but useful if someone is wary
of checking ID of a caller. If SSEN/ WW are
unable to contact the household by phone,
they have teams who also go door to door to
ensure the household is contacted.
Nominated Contact:
The household can choose to have
someone else notified on their behalf –
useful if they would have difficulty in
managing the telephone call or a caller
at the door.
Other reasons:
If the household needs assistance
but does not fit any of the
categories given other reasons can
be given e.g vulnerable as in rural
location and wholly dependent on
electricity
Signed:
The form MUST
be signed by
someone in the
household
