CLINICAL TRIALS
- Studies have shown that the use of a flutter valve as opposed
to an Under Water Drain (UWD) and an emphasis on earlier
mobilization reduces the hospital stay of patients:
- Niemi et al. (1999) proved a reduction in mean drainage time
(1.2 days), mean length of hospitalization (1.7 days) and number
of x-rays (1.6) when flutter valves were used for chest drainage
(simple pneumothorax) as opposed to UWD’s (76 patients).
- Graham et al. (1992) compared the use of chest drainage bags
with flutter valves to UWD's in 119 patients undergoing elective
thoracotomy. Patient with flutter valves were fully mobile 23
hours earlier, able to sit in a chair 7 hours earlier and discharged on
average 1 day earlier.
- Knottenbelt & vd Spuy (1990) showed a significant reduction in
hospital stay with a protocol of greater patient mobility and
physiotherapy used on patients undergoing chest drainage (after
penetrating chest trauma).
- Bernstein et al. found that 66% of patients fitted with a Heimlich
flutter valve showed full expansion of the lung within one hour
(pneumothorax).
- The first clinical trial (Cooper & Hardcastle, 2006)
comparing the Sinapi Chest Drain with UWD’s showed a
24% reduction in drainage time (penetrating chest
trauma).
- Further studies supporting patient mobility: Bar-el et al.,
Baumann, Carroll, Cerfolio et al., Conces et al., Crocker & Ruffin,
Driver et al., Heimlich, Horowitz & Oliva, Lodi et al., Mckenna et al.,
Metersky et al., Ponn et al., Taubert et.al., Vricella & Trachiotis.
Waller et al (1999) found that a flutter valve provides a
physiologically more effective alternative to the underwater seal
drainage system when managing post-operative air leaks.

- The water seal is an inefficient ‘one-way
valve’: the water column in the tubing moves back to the lung (tidal
action) allowing air to re-enter the pleural space. This air-buffer
reduces the ability of the lung to ‘push air out’ of the pleural space -
thus reducing the rate of recovery (Miller & Sahn, 1987).
- The bottle can be knocked over = seal and closed system is broken.
- Health care staff requires a good understanding of the physics of
"water pressure" to manage the UWD effectively.

OTHER ADVANTAGES OF THE SINAPI CHEST DRAIN
- Manual suction can be applied by depressing the bulb to assist with
drainage.
- An angulated valve and depressed bulb indicate expanded lung.
- Regular patient movement is easier, helping trapped fluids to exit
the thoracic space.
- When patients mobilize, breathing volume increases. This
increases pressure inside the pleural space forcing air/liquid out via
the catheter.
- Patient posture improves allowing full use of the chest muscles and
diaphragm.
- Patient mood improves with increased independence.
- The speed of pleural drainage and promptness of pulmonary re-
expansion are the most effective measures against pleural
infections (Aguilar et al. & Millikan et al.).
