Who are we? Big Lottery Fulfilling Lives programme One of 12 projects in England Funded for 8 years Working with multiple complex needs ON Nottingham is a Partnership of 17 agencies

Multiple complex needs?

At least 3 out of 4 of the following needs:

Substance misuse

Mental health

Homelessness

Offending

Nottingham City based

The most chaotic people in the city

Case co-ordination

Long term approach

Aims:

To empower people with multiple complex needs and support and enable them to take control of their lives.

Change front line services and make them more effective by listening to what services users want and need

To deliver change at strategic and commissioning level

Why system change? People with multiple complex needs can’t access many front line services

Estimated 60,000 people in UK with multiple complex needs

Massive use of front line services at great cost

Many services are geared up to only deal with one problem

Are not accessible for the client group

How do we change the system?

Service shaped by beneficiaries by consulting, listening and acting

Expert citizens at the heart of the process

Buy in from providers, commissioners, politicians and everyone!
