In compliance with Reuter-Stokes, Inc. Byproduct Material LicenseQ No. 34-18156-01, License Condition No. 19, we are submitting the

results of a radiation survey for areas around and above theirradiator in the shielded and unshielded positions. The gammaradiation survey was performed using a calibrated Reuter-StokesInstruments Environmental Monitor (pressurized ion chamber) ModelNumber RSS-111-100. A copy of the instrument calibrationcertificate is enclosed as Attachment 1.

Prior to loading the 30 curie Cobalt-60 source, we reshieldedthe room in which the irradiator is located to provideapproximately four inches of lead with additional concretesupport to the walls and a lead filled door. The 30 curiesource was loaded by Amersham Corporation who is licensed toperform the service under NRC License No. 20-12836-01. Shortlyafter loading, the radiation survey was performed before anyrestart of irradiator operations began.

The radiation levels of areas around and above the facility areshown on Diagrams 1-7. All areas above the facility are close tobackground levels when the source is in both the shielded andunshielded position. The closest unrestricted pathway around theirradiator had readings of less than 0.3 mR/hr when the source isin the unshielded position.
