Surveys
• There are four types of surveys: Commodity Information, Demand Ordering, Direct Diversion and Hold/ Recall.
• Commodity Information and Demand Ordering Surveys allow the Commodities Team to gather information regarding needs and future orders of various commodities
• Direct Diversion Surveys order surveyed commodities and divert these commodities to a processor chosen by the sponsor.
• Hold/Recall Surveys allow you to communicate how much of a recalled product you have and when it is destroyed per instructions.

• Verify that the Program Year and Program values are correct. Use the drop down arrows to select the correct program year and program.
• Click the Continue>> button.

• CATS displays the Survey Entry – NSLP page.• Click on the number in the Survey column to access the
survey.

• After clicking Continue>> , CATS displays the survey with the Submit Survey button on the bottom of the screen.
• Use the Submit Survey button to submit the completed survey.

Respond with the amounts of held or recalled commodity items that are in your inventory and verify that you have disposed of them as the USDA Disposition as instructed.

• Read the Special Instructions.
• If you do not have any of the recalled foods, click the checkbox next to the statement, “We DO NOT have any donated foods identified by the Can/Package Codes below on hand.”
• If you have any of the recalled foods listed, check the Can/Package code.
• If the Can/Package code on the items match, enter the number of items in the Cans/Units field.

Destroying Held or Recalled Items
• After you have been notified, watch for an email from the Commodities Team specifying the USDA’s Disposition for the recalled items.
• After you have executed the directions in the Commodities Team’s email, return to the Hold/Recall Survey.
