MOTION: After discussion, Ms. McKenzie made a motion to approve Mr. Glaz’s application for licensure. The motion was seconded by Mr. Spafford and carried with a 2/0 vote. The motion was seconded by Ms. McKenzie and carried with a 2/0 vote. TAB 8 Great Lakes Seminars, CE Provider #50-8423 MOTION: After discussion, Ms. McKenzie made a motion to approve Great Lakes Seminars’ application for licensure. The motion was seconded by Mr. Spafford and carried with a 2/0 vote. APPLICANT RATIFICATION LISTS TAB 9 Endorsement Candidates: Occupational Therapists & Assistants MOTION: Mr. Spafford moved to approve the Endorsement ratification list of 394 applicants for licensure (APPENDIX 1). The motion was seconded by Ms. McKenzie and carried with a 2/0 vote. ACTION TAKEN: 394 names ratified for licensure by endorsement. TAB 10 Examination Candidates: Occupational Therapists & Assistants MOTION: Ms. McKenzie moved to approve for licensure, pending the successful passage of the NBCOT examination, Exam ratification list of applicants for 60 names (APPENDIX 2). The motion was seconded by Mr. Spafford and carried with a 2/0 vote. ACTION TAKEN: 60 names ratified for licensure pending successful exam results. TAB 11 Examination with Waiver Candidates: Occupational Therapists & Assistants MOTION: Ms. McKenzie moved to approve the Examination with Waiver ratification list of 68 applicants for licensure (APPENDIX 3). The motion was seconded by Mr. Spafford and carried with a 2/0 vote. ACTION TAKEN: 68 names ratified for licensure by Examination with Waiver.


TAB 12 Continuing Education Provider Applications MOTION: Mr. Spafford moved to approve 7 continuing education providers, 4 Prevention of Medical Errors course providers, -0- Florida Laws and Rules course providers, and 3 AOTA approved providers, as approved by Board Rule. (APPENDIX 4) The motion was seconded by Ms. McKenzie and carried with a 2/0 vote. TAB 13 Exemption for Disqualification of Employment Applications MOTION: Ms. McKenzie moved to approve 4 applications on the Exemption Application from Disqualification of Employment list granted per Board Staff review, as permissible within the Board Delegations (See APPENDIX 5). The motion was seconded by Mr. Spafford and carried with a 2/0 vote. ACTION TAKEN: 4 Exemption for Disqualification of Employment application approved. TAB 14 VALOR Military Application List MOTION: Mr. Spafford moved to approve 2 applications on the VALOR Military Application list of applicants for licensure (APPENDIX 6). vacant MOTION: After discussion, Mr. Spafford moved to nominate Ms. McKenzie as the Temporary CE Committee Person. The motion was seconded by Ms. McKenzie and carried with a 2/0 vote.


Ms. McKenzie discussed to come to the board office on a Wednesday afternoon after 2:00 to train in CE Broker and review CE applications for the Board with Ms. Boutwell. MOTION: Ms. McKenzie moved to nominate Mr. Spafford as Board Chair. The motion was seconded by Mr. Spafford and carried with a 2/0 vote. MOTION: After discussion, Mr. Spafford moved to nominate Ms. McKenzie as Vice Chair. The motion was seconded by Ms. McKenzie and carried with a 2/0 vote.

Mr. Spafford suggested and Ms. McKenzie agreed to delay in filling the remaining Board liaison vacancies until the October Board meeting.

TAB 16 Review of International Sports & Fitness Trainers Association (ISFTA) Continuing Education Course

The Board office received a complaint from a licensed occupational therapist regarding the suitability of the provider’s “ISFTA Enriching Alternative Medicine with Essential Oils Course”. ISFTA was originally approved as a continuing education provider by the Board in July 2009. Upon receipt of the licensee’s concerns, Board staff sent inquiry to ISFTA requesting review and response to the key concerns expressed by the licensee. Staff provided for the Board’s review the original complaint, Board staff’s follow-up inquiry to the continuing education provider, the provider’s subsequent response, and a copy of the Board’s Rule 64B11-6.001, F.A.C., Continuing Education Program Approval. MOTION: After discussion, Ms. McKenzie made a motion to request a formal audit of the provider’s course registered in the complaint, per Rule 64B11-6.001(3), F.A.C. and requested the specific materials listed in Rule 64B11-6.001(1) and (3), F.A.C. for submission to the Board for review. The motion was seconded by Mr. Spafford and carried with a 2/0 vote. The motion was seconded by Ms. McKenzie and carried with a 2/0 vote. MOTION: After discussion, Mr. Spafford made a motion to ratify the annual regulatory plan as presented by Ms. McNulty. TAB 17B Rule 64B11-4.003, Standards of Practice; Discipline MOTION: After discussion, Mr. Spafford made a motion to grant authority to open up Rule 64B11-4.003 for development and directed counsel to proceed with the rule making process, prior to October 31, 2016. REPORTS, IF ANY TAB 18 Board Chair, Acting – Jim Spafford Mr. Spafford remarked it was his honor to serve as Acting Board Chair, and was planning to attend the September 28th, 2016 Board Chair meeting in Tallahassee.

James Spafford, NBCOT Conference Mr. Spafford provided a brief explanation to the board and staff of his attendance at the NBCOT 2016 Inaugural State Regulatory Leadership Forum held May 11-13, 2016 in New Orleans, Louisiana, as the Board delegate. Mr. Spafford stated he was honored to attend as he was the only Consumer member that was in attendance of the states represented. Mr. Spafford stated that Florida seemed to be well notable among the state participants, and ranked number one in number of licensed OTA’s, and number 5 in OT licensures in the US. Mr. Spafford included he observed that Florida seemed to be very fortunate to have qualified and efficient Board staff in comparison to many other states circumstances, and therefore he expressed his admiration of the efficient Florida Board and staff. He stated that Telemedicine was a big topic on the agenda, as well as NBCOT’s Navigator system. He concluded he was able to make many contacts among the state representatives and that the Leadership Forum proposed to meet biannually which he agreed to be appropriate.

OLD BUISNESS TAB 20 May 2, 2016 General Business Meeting Minutes MOTION: Ms. McKenzie made a motion to approve the May 2, 2016 minutes as presented, The motion was seconded by Mr. Spafford and carried 2/0 vote. OTHER BUSINESS AND INFORMATION (items that do not require Board action) TAB 21 Staff Recognition Presented as informational items, however, Mr. Spafford stated his appreciation and praise to the Board Staff. Mr. Hall also expressed his appreciation for the dedication of staff especially to Mr. Spafford, for stepping in as Acting Board Chair and to Ms. McKenzie today in a time of membership leniency to complete all that is needed to be done for the Boards business needs.
