Subj: Official Passport Application (DS-82 form)

1. This form is used when the Traveler already has a tourist passport and can submit the passport with the DS-82 application.

2. Access the US Department of State Travel website http://travel.state.gov

3. Click on Passports

4. On the left side click on Applications and Forms then select Application for Passport By Mail (Renewals Only): DS-82

5. Click on COMPLETE FORM DS-82 Online

6. Click that you have read the Privacy and Computer Fraud notices. Then click Submit.

7. You are applying for a Passport Book. Click Continue.

8. Enter in information about you. DATES are in MM/DD/YYYY format. Click Next.

9. The passport is to be mailed to International Programs Office (IPO) at this address: 117 Decatur Rd, Stop 13F Annapolis, MD 21402

10. Enter email address and telephone number. Click Next.

11. Enter travel plan information. Date format is MM/DD/YYYY Click Next.

•For all trips, enter 5/9/2015, 3 Weeks, China. This doesn’t mean you’re going to China, we just need to enter information in these blocks.

Click Add button if you want to add additional telephone numbers (OPTIONAL)

12.Enter your emergency contact information. This should be someone who is not traveling with you such as your mother, father,

or your spouse.

13. Enter your passport history. You should already have a tourist passport if you are filling in a DS-82 form. Select PASSPORT BOOK.

Enter information about the passport .

In most cases, you will enter your tourist passport (blue cover) information.

If you are renewing an official passport (red cover), then enter the official passport information here.

Click Next.

16. Review all your information for accuracy and completeness. Make corrections using the Edit buttons as needed.

17. Once your review is complete, scroll to the bottom and click Next button.

18. You are applying for a “no fee” passport, also known as an “official” passport. You will not have to pay any fees. Click SUBMIT.

20.Click Open or Save. Either way you will get a 6 page document.

Only print Pages 5 and 6 of the application. The pages can be printed in black & white. They do not need to be in color.

Bring the hardcopy to International Programs Office.

21.Open the document and go to page 5. A successfully completed DS-82 application will have a bar code in the upper left corner and will show DS-82 in the lower left corner of the pages. The Form will be blue in color.

You can print pages 5 and 6 in black & white.
