Dear Mr. Jacobs:
On July 9, 2010, I conducted a NPDES Compliance Evaluation inspection of the SnowI-lilt Country Club wastewater treatment works (WWTW). Mr. Keith Collins;who is thecontract operator of record for the WWTW, represented Snow Hill Country Club duringthe inspection. The purpose- of the inspection was to evaluate compliance with theterms and conditions of the NPDES Permit. A copy of the report is provided within.
Please pay special attention to the "items requiring correction" shown in boldtype, for there are compliance schedules associated, with these items. Snow HillCountry Club will need to assign an employee to take daily readings at theWWTW when Mr. Keith Collins is unavailable. These readings include color, odorand turbidity observations, as well as daily flow readings.

Several items were changed/added after the previous inspection in 2009including, but not limited to: elapsed time flow meters were added to the sandfilter dosing pumps, operator of record log book and the burned out light bulb forthe sand filter dosing pumps alarm was replaced.

Non-compliance notification forms must be submitted to Ohio EPA forviolations of the NPDES permit (such as those shown above). The purposeof these forms is to meet the requirements of the NPDES permit and tocommunicate with Ohio EPA as to the cause of the violations and the steps beingtaken to prevent similar violations from occurring in the future. Operator of Record Notification
The attached Operator of Record Notification form shall be completed andsubmitted to Ohio EPA. Within three days of any future changes in an operatorof record, the permittee shall notify the Director of the Ohio EPA of any suchchange on a form acceptable to Ohio EPA. Daily Readings
The NPDES permit requires daily monitoring of color, odor, turbidity anddischarge flow. Currently, Snow Hill Country Club does not have a maintenanceperson assigned to take these daily readings. The operator of record is only on-site once or twice per week; therefore, arrangements must be made to meetthe daily monitoring requirements of the discharge permit immediately.
*Readings are not required on weekends and appropriate holidays (utilize thecode "AN" on the e-DMR).
Operator of Record Logbook
The operator of record logbook contained the prescribed information requiredthrough Ohio Administrative Code 3745-7-09; with one exception: the logbookis required to be a hardbound book (the logbook for Mr. Collins was spiralbound). Please make the appropriate correction without delay.
The overall condition of the WWTW was good and the effluent from thedechlorination chamber was clear with no observable solids.
