• Ensure to fill out entirely and legibly

• Can Give by Payroll Allotment, Cash or Check

• Blue Copy goes to Payroll Office

• White Copy to CFC Office

• Pink Copy to Donor

Ensure Pledge Form is Completely Filled Out

Ensure These Amounts Match

Ensure They Select Whether they are Civilian or Military

Ensure has SS# (or Employee ID for USPS) and signature for payroll allotments

Thank donor and give them Pink copy for their records
