NKF’s Strategic PlanGoals that Focus on Program:

1. To Empower Patients and Their Families

2008 Executive Leadership Conference

NKF’s Strategic PlanGoals that Focus on

Program:

• Building on the impact of KDOQI and KEEP• Addressing Risk Factors like diabetes and

hypertension• Pursuing the knowledge that CKD is a

multiplier of risk, cost and poor outcomes• Utilizing new strategies, targeting new

audiences, and interacting on a higher level with public health and treatment communities

2. To Increase Identification of, and Improve Treatment for Those With or at Risk of Kidney Disease

2008 Executive Leadership Conference

NKF’s Strategic PlanGoals that Focus on

• Focus on the late or “no” referral of our patients to transplantation

• Increase living donation and concepts like Paired Exchange

• Focus more on the “transition” of our patients from Stages 3 to 4 and then from Stages 4 to 5

• Ensure that transplant recipients keep the transplant they have

3. To Improve Patient Care Through Early Transplantation

Expanding NKF’s Programs: Scope and Reach

The Challenges:• 26 million people have CKD, another

20 million are at risk• Not aware, not identified, not

treated• Not seen by a nephrologist• Patients and their physicians have

no real connection with NKF

2008 Executive Leadership Conference

Expanding NKF’s Programs: Scope and Reach

The Opportunities: • Finds and attracts new patient and

professional constituents• Allows NKF to move into a new space, a

bigger space, and across other disease states

• Expands NKF’s message and builds more brand equity for NKF

• Serves as a catalyst for new programs based on new constituent needs

• Attracts new corporate partners

Saves Lives

2008 Executive Leadership Conference

Distribution of Medicare Patients & Costs

CKD represents 6% of Medicare enrollees and they account for 18% of the expenditures

2008 Executive Leadership Conference

Distribution of EGHP Patients & Costs:

CKD represents 1% of the population age 50-64 and account for 9% of the expenditures in EGHPs

2008 Executive Leadership Conference

CKD is a Risk and Cost Multiplier

CKD subpopulation is very expensive and has high event rates, making it a major public health issue

2008 Executive Leadership Conference

Cardiovascular – Kidney – Diabetes

Core Messages• Cardiovascular disease is the number one

killer of people with CKD

• Chronic Kidney Disease multiplies CVD risk

• For every CVD risk factor (Diabetes, Hypertension, Dyslipidemia, Anemia), clinicians must test for CKD

2008 Executive Leadership Conference

Cardiovascular – Kidney – Diabetes

Core Messages• CKD is the disease multiplier:

1. The patients are sicker 2. Risk factor levels are higher3. Control is worse and treatment is more

complex4. Outcomes are worse5. Costs are higher

• MDs don’t need to find new patients • With CKD, patient management is different• Treatments are available and effective today

2008 Executive Leadership Conference

NKF’s Message in Stage 4 CKD

Stage 4 CKD is an asymptomatic clinical event!

It’s a “Kidney Attack”• We can slow progression in Stage 4• We can reduce the incidence of bad events that

occur in Stage 4• We can reduce the severity of Stage 4 co-

morbidities• We can improve the coordination of care in Stage 4 • We can prevent:

– Cardiovascular events– Death

2008 Executive Leadership Conference

Launching The New CKD Paradigm

• NKF, the authoritative voice on CKD pulls disease states together

• Global KDOQI conference October, 2007– George Bakris, MD (Nephrology)– Mark Molitch, MD (Diabetes)– Richard Nesto, MD (Cardiology)

• 80 key opinion leaders including primary care and family practice MDs

• 2 ½ days

2008 Executive Leadership Conference

Goals• Investigate and understand the

relationship between CVD, diabetes and CKD

• Explore the impact of CKD as a disease multiplier

• Discuss barriers to optimal care (early identification, diagnosis of co-morbid conditions, specialist care/primary care)

• Evaluate available treatment options • Provide recommendations on what you

do with what we know

2008 Executive Leadership Conference

Cardio Kidney Diabetes (CKD) Conference

• Chairs: George Bakris, Mark Molitch, Richard Nesto• By invitation-only: Nephrology, Cardiology,

Endocrinology, Epidemiology, Primary Care community leaders

• Goal: to identify practical approaches to early identification and management of risk factors in:* Cardiovascular disease* Chronic kidney disease, and/or * Diabetes

• Need for collaboration between specialties to improve care of patients with overlapping disease processes.

October 2007 in Chicago

2008 Executive Leadership Conference

Cardio Kidney Diabetes (CKD) Conference

• Recommendations about:* Influence of Blood Pressure Targets on Vascular
