Improvement of the health care service to the citizen through a tool that makes health professionals work easier by allowing the shared use of the available information about patients among healthcare centers
The Shared Clinical History (HC3) should make easy and potentiate the care continuity and improve the computer resources

Pilot project during 2008
Objective: Validate the model of interoperability among different electronic records existing in other autonomous communities in Spain.

Involvement of 26 partners from 12 European countries.
