1.0 direCTor’s messaGe
Greetings to all & Salam 1 Malaysia.
As a leader in establishing traditional and complementary medicine(T&CM) services in Malaysia, the Traditional & Complementary Medicine Division (T&CMD) is committed towards realising the vision and mission of improvingthehealthandwell-beingofallMalaysians.
The Division has continued its efforts in strengthening the incorporation of T&CMservicesinthenationalhealthcaresystemwiththeestablishmentof two T&CM units in 2011 at Hospital Sultanah Hajah Kalsom, Cameron Highlands and Hospital Raja Perempuan Zainab II, Kelantan. To date,there are 10 hospitals offering T&CM modalities for the management of chronicpainandpost-stroke.Besidesthat,HospitalPortDicksonhasbeenselected toprovide Shirodhara, the firstmodality under the traditionalsystemof Indianmedicine tobe introduced inagovernmenthospital.MoreT&CMunitsareexpectedtobeestablishedinMalaysiainlinewithT&CMD’s target of excellence.
It is hopeful that the introduction of T&CM into the mainstream health care system would elevate T&CM practices to the next level. The incorporation of T&CM into the mainstream health care system is one of the strategies under the World Health Organisation (WHO) and we are proud to announce that Malaysia is in line with WHO’s calling.

2.0 orGanisaTion pHilosopHy
2.1 vision
Traditional and Complementary Medicine Division aspires tobecentreof excellence for traditionaland complementary health care practices.
2.2 mission
Traditional and Complementary Medicine Division will ensure that the practice of traditional and complementary healthcare is safe and effective forthebenefitofcitizens.
2.3 sTraTeGies
To achieve the above vision and mission, theTraditional and Complementary Medicine Division (T&CMD) has planned the following strategies:
i. To increase the competency and efficiencyof the organisation through development of effective human capital, moderation and automation of the office function, and toconstantly improvise the services;
ii. To develop practice guidelines which ensure quality services provided and conform to standard practice;
iii. To strengthen mapping activities of premises providing T&CM services;
iv. To ensure good understanding and working relationship between T&CM practitionerbodies and T&CMD through meetings anddialogues;
v. To enhance T&CMD staffs’ expertise;
vi. To create a dedicated and fully motivated staff through appropriate remuneration;
vii. To accelerate activities in research, development and innovation;
viii. To create a conducive environment which encourages the staff to work as a team and to achieve the assigned task professionally;
ix. To strengthen working network with other government agencies, private sector and also international organisations.
2.4 QUaliTy poliCy
We are committed to implement regulatory and standardised system of health care practices of traditional and complementary medicine that is efficient and effective towards enhancing levelandqualityofpublicwellness.
3.0 ClienT’s CHarTer
i. Issuance of support letter for foreign practitionerswillbeprocessedwithin14days.
ii. Assist in developing standard and criteria for the field of Traditional and ComplementaryMedicine practices within 6 months.
iii. Ensuring Traditional and Complementary Medicine Division staff at all levels undergo a minimum of 7 training days in a year.
iv. Inspecting no less than 750 premises of Traditional and Complementary Medicine health care practices according to planned schedule.
v. Traditional and Complementary Medicine Unitstobedevelopedwithin9months.
vi. Paymentforsuppliesandservicestobemadewithin 14 days upon receiving complete invoice.

A total of 118 applications for support letter for foreign Traditional and Complementary Medicine (T&CM) practitionerswereprocessedin2011(Table1).Assuch,24applicationsforsupportletterthatcoveradministrativeandmanagementpositionswereprocessed(Table2).

5.4 inspection of premises
In terms of inspectorate activities, a total of 824 premises in Malaysia were inspected throughout the year 2011 with the Central zone (Klang Valley area) leading the statistic with a total of 381 premises. These inspectorate activitiesweredividedaccordingtozonesnamelyNorth,South,Central,East,SabahandSarawak.Figure4showsamarkedimprovementinthetotalnumberofpremisesinspectedin2011.ThisisduetoanincreaseinthenumberofstaffbeingpostedintheCentralI&ESectionaswellascollaborativeeffortswiththebranchofficefromeachzone. From Figure 5, the highest practice area inspected from the year 2007 to 2011 is Complementary Medicine with a total of 630 premises.

In-service training were conducted with a mission to enhance the knowledge, skills and competency of the staff membersofT&CMD.Long-termgoalsaretoproducecompetenthumancapitalintheaspectsofknowledge,qualityandpositivevalueswhileenhancingtheefficiencyandeffectivenessofthepublicservicedeliverysystem.AcomprehensivetrainingprogrammehavebeenpreparedanddesignedbytheDivisionthroughtheTrainingOperational Plan for 2011.

5.6.3 Continuous Medical Education (CME)
18CMEsessionshadbeenconductedatseveralgovernmenthospitals.Therewasanincrementof5CMEsessionsas compared to year 2010. The main purpose of the CME programme was to provide exposure and disseminate informationonthetrackdevelopmentofT&CMtothestaffmembersoftheMinistryofHealth.TheCMEparticipantswereencouragingwithatotalof1449peopleconsistingofnurses,medicalassistantsandmedicalofficers.
2011 also saw two television interviews in Selamat Pagi Malaysia (Radio Televisyen Malaysia (RTM1)) and Hello on Two(RadioTelevisyenMalaysia(RTM2))representedbyMr.JaafarbinLassaandDr.ShamsainibintiShamsudin,respectively.

In2011,T&CMDreceivedabudgetofRM2,185,460.00foroperationanddevelopment.90.75%ofthebudgetwasspent on managerial services such as payment to T&CM practitioners and acquisition suppliers for T&CM Units. Figure12showsthedistributionofthebudgetandexpenditureinyear2011.

Out-patient oncology clinic patients were interviewed using validated questionnaires.

3. The data is collected at the Traditional and Complementary Medicine Units in three integrated hospitals: a. Kepala Batas Hospitalb.PutrajayaHospitalc. Sultan Ismail Hospital.
The study involved the data of cancer patients receiving ChineseherbaltreatmentfromOctober2007untilFebruary2010. Data was extracted from from patients’ records.
The data extraction form was categorised into 4 sections, namely socio-demographic, type of cancer, the symptoms associated with treatment principles of Chinese Medicine and herbaltherapyuse.
To identify the patternofherbalusage according to the concept of treatment as stated in the T&CM Practice Guidelines on HerbalTherapyas an adjunct treatment for cancer.

In addition to training courses, study monitoring in accordance to GCPs, regulatory requirements and other protocols had been initiated and conducted at Kepala Batas Hospital, Putrajaya Hospital and Sultan IsmailHospital startingDecember 2011. This initiative shall affirm thecredibility, accuracyandqualityof the studiesconducted.Mistakeshadbeenidentifiedandimprovementsweresuggestedtothesiteinvestigators.Ithasbeenplannedthatstudymonitoringbeconductedmorefrequentlynextyearfollowedbystudyaudits.

UntilDecember2011,atotalof4910localT&CMpractitionershadvoluntarilyregisteredwiththee-PENGAMALsinceNovember2008(Figure13).Thelargestincrementfortheregistrationoflocalpractitionerswasnotedin2010withatotalof1198.Thiswasattributedtoaseriesofroadshowsanddialoguesheldthroughoutthecountry.
Traditional ChineseMedicine practitioners (66%) recorded the highest percentage from the total number ofpractitioners.ThiswasfollowedbyComplementaryMedicinepractitioners(13%)andTraditionalMalayMedicinepractitioners (11%) (Figure14). The number of Traditional ChineseMedicine practitioners and IslamicMedicalpractitionersprogressivelyincreasedin2011duetoinitiativesbeingtakenbytherespectivePractitionerBodies(Table14).

6.4.1 RecertificationoftheMSISO9001:2008
Theaudit for recertificationof theMS ISO 9001:2008was conducted from 29 to 30 November 2011 bySIRIM. This audit is an important milestone for T&CMD asitreplacesthepreviousMSISO9001:2008certificatewhichexpires inDecember 2011. Theauditwas ledbyateamofauditorsfromSIRIM,namelyMs.MariamBtMohamad ZinandMs. Rafidah BtMohdYusuf.Anote of gratitude goes to the commitment and dedicationfromallthestaffmembersforithasbeenannounced that T&CMD have successfully acquired the recertificationofMS ISO9001:2008, valid for thenext three years (2012 – 2015).

The journal database project was aimed atproviding quicker access to existing journals bybuilding an online database (http://tcmd03/JournalDirectory/)thatcouldbeaccessedbyallstaffmembers.Thisprojectpotentiallysavestime,reduce printing and usage of paper.
3R Project themed “Save the Earth” was inaugurated by Dr. Ramli Abdul Ghani, formerdirector of T&CMD on 14th September 2011(Figure 24 & 25). The project serves to educate andbuildawarenessonadoptinggreenvaluesinour daily lives and to incorporate these values at the workplace as well.

After the successful development of the e-Pengamal online application for the registration of practitioners, the Division had ventured into developing another online application namely the Inspectorate and Enforcement (I&E) Module, Figure 26.
TheI&Emodulewasdesignedinamannerthatwouldprovideenforcementofficerseasieraccesstodetailedinformation on inspectorate and enforcement activities, practitioners and their practices as well as publiccomplaints.TransmissionofinformationisexpectedtobefasterandsaferwhileencouragingcoordinationandconsistencybetweentheDivisionanditsbranchofficesintermsofsystematicdocumentationandminimisationof red tape procedures.
More importantly, the module would strengthen the roles and functions of the I&E section and help ensure that the provisionofT&CMservicesarewellmonitoredforbothsafetyandefficacy,inlinewiththemissionoftheDivision.

The Division had upgraded its websitewith a new interface and addedmore information. It caters to bothpractitioners and the public who wish to obtain information relating to traditional and complementarymedicine.
InJuly2011,theT&CMwebsitereceivedafourstarratingundertheMinistryofHealthMalaysia(Figure27).ThewebsitehasalsobeenlistedintheMyGovernmentwebsiteforevaluationbyMultimediaDevelopmentCorporation(MDeC) in 2012. 8.1.1 1sT inTernaTional ConferenCe on HomeopaTHy 2011 (iCHom 2011)
On 15th to 17th July 2011, the 1st International Conference on Homeopathy 2011 (ICHOM 2011) was held at the Putra World Trade Centre. The conference was a successful collaboration between the Traditional andComplementary Medicine Division (T&CMD), Ministry of Health (MOH) and the Malaysian Homeopathic Medical Council (MPHM).
The theme of the conference was “Is Homeopathy Evidence-based Medicine?” It was organised with theobjectiveofopeningupvastopportunities for researchers to realize thedesire to improve thecompilationofscientificevidencethatreflecttheeffectivenessofhomeopathypractice.
The response was encouraging with a total of 300 participants - 250 T&CM practitioners’ largely consisting of homeopathicpractitioners(includingfiveinternationaldelegates),50studentsandthegeneralpublicbothlocaland international.Bookvendorsandhomeopathicproductdistributorsalsotookpart inpromotingtheir latestproductsattheirrespectivebooths.

In the month of Ramadan, the T&CMD SocialandWelfareClubhadarrangeda breaking fast ceremony at the AlRawsha restaurant in Kampung Pandan which served Arab cuisine. The feastwasattendedby20clubmembersandmost of theexpenseswere fundedbythegenerousofficersofT&CMD.
