Go! Fit! Win! Wellness LISD has partnered with iaWellness & Covenant Health to offer a program designed to educate and motivate you towards a healthy lifestyle. We will provide free health screenings, educations, and health guidance for you, as well as access to a new wellness portal to help you track your points and activities. At LISD, our staff is our most valuable asset, and your time and health are important to us. We have streamlined the wellness process to make it easier for you. You will be able to complete your health screening via finger stick instead of blood draw, answer a brief health risk assessment on an iPad, and have your results reviewed, all in the convenience of your 20 minute appointment. Quick and Easy! You will note that the program requirements for this year are very similar to what you've had in the past. How Do I Meet The Standard To Earn My Medical Plan Premium Incentive for 2016?

Welcome to LISD's 2015-2016 Wellness Program!LISD would like to introduce the NEW Go! Fit! Win! Wellness Program. Don't miss out on this great opportunity to

save up to $900 on your 2016 medical plan premium and to improve your health at the same time! We look

forward to a fun and successful year. Please review the information in this guide to help you complete the

requirements and get the most out of your wellness program!

You may schedule your health screening, access your screening results, review your risk factors, log points, participate in challenges and view other important information in your secure wellness portal.

Click "I’m A First Time User”

Complete the registration

process by using your Wellness ID

(your Wellness ID is the same as

your employee number*).

View the 2015-2016 Go! Fit! Win! Kick-Off Video View the video at gofitwinwellness.com no later than August 31, 2015. It will provide you with important information to help you achieve success in the program. Viewing will be tracked on the wellness portal.

Complete a Wellness Screening You and your spouse* (if they are on the medical plan) must screen by November 13, 2015. Screenings will be held on-site at each campus and location.

Complete a Health Risk Assessment (HRA) You and your spouse* (if they are on the medical plan) must complete an HRA by November 13, 2015. The HRA will be available for completion at your wellness screening appointment.

Health screenings are a benefit funded by LISD. They

provide important information toward the early detection

and prevention of disease. If you are unable to schedule

at your home campus, contact iaWellness at 806-765-

7265 or email at [email protected] for alternate

scheduling.

What Are My Risk Factors and What is Progress? We use 11 core measurements for the Go! Fit! Win! Wellness Program. Each value listed

below has an optimal value to shoot for, but we count progress as a step in the right

direction! The chart below has each value listed along with the optimal value and the

progress guideline.

RISK FACTORS PROGRESS GUIDELINES OPTIMAL VALUE

Systolic Blood Pressure by at least one point <120

Diastolic Blood Pressure by at least one point <80

Total Cholesterol by at least one point <200

If you viewed the kick-off video, completed your health screening and your HRA, it’s time to start working

on improving your health screening results. Begin making progress now to earn the medical plan premium

incentive for 2017. You have until your health screening in the fall of 2016 to meet the improvement

requirements outlined below. In an effort to meet the needs of a variety of health and wellness levels, we are

offering 2 options for achieving your improvement requirements.

OPTION A: (points and progress*)1. Earn 500 wellness points by November 15, 2016.

Your wellness plan is committed to helping you achieve your best health. Rewards for participating in a wellness program are available to all

employees. If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity

If your spouse is on the medical plan, they are

required to complete the steps below in order for you

to maintain your medical plan premium incentive.

Complete Wellness Screening by

November 13, 2015. Spouses can sign up online

using the instructions listed inside.

Complete an HRA by November 13, 2015. (HRAs will

be available at screenings.)

Spouse Wellness Portal Login

Spouses will follow the login instructions in this guide

using the LISD Employee's ID and date of birth. They

will need to complete their personal information

and then will be given their own Wellness ID

for future logins.

Spouse Participation

Spouses are not required to earn points or make

improvements, but we encourage them to improve

their health based on their screening results. Spouses

may track their points using the online portal, but can

not participate in online challenges.

LIFESTYLE COACHING & disease management

You have access to your very own registered dietitian,

personal trainer, and registered nurse. Working with

a qualified health professional to meet your nutrition,

fitness, or condition management goals is just a free

phone call away for you or your spouse! Coaches

may also contact you. Take advantage of this

opportunity and start improving your health!
