NCCC FOUNDATION
Scholarship Program
Many scholarships have been established for North
Country Community College students by generous
donors who are passionate about education and helping
others to achieve their goals in life. Each year, NCCC
students benefit from the generous underwriting
donated by these friends of the college and champions
of education.
The NCCC Foundation makes a variety of scholarships
available for full-time and part-time students based on
academic performance, degree program and/or financial
need.
This NCCC Scholarship Booklet describes all the
scholarships available for NCCC students for 2014-
2015 academic year.
Please read the descriptions for each scholarship
carefully to determine individual eligibility based on
the established criteria. Note the deadlines
vary. Scholarship awards will be applied to your college
expenditures in equal amounts each semester.

website/Foundation/Scholarships.

Recipients must meet the following criteria:
Be a child, grandchild, or member of the immediate family of a North Country alumnus/a
Be nominated by said alumnus/a
Be a full-time matriculated freshman or sophomore in Fall 2015
Maintain a cumulative average of 3.0 to continue the scholarship award
Agree to make academic credentials available for review by the Scholarship Committee
Agree to release name for College and/or alumni-initiated publicity
A complete scholarship application must include:
A completed NCCC Foundation Scholarship Application Cover Page with this scholarship listed
High school transcript with graduation date (academic average must be 85% or higher) or college GPA of 3.0
A resume (list all school/community activities, including offices held, awards received, etc.) and 2 letters of recommendation
An essay from applicant describing his/her qualifications and relationship to the North Country alumnus/a
A written statement by the nominating alumnus/a with his/her name, address, NCCC graduation date, telephone number, relationship to nominee and a few sentences about the nominee’s qualifications

Be a returning full-time student
Have a minimum GPA of 2.75 and be maintained to receive funding for the 2nd semester
Indicate current college and/or community service
State academic major
List career goals
Have a commitment to succeed academically at NCCC
How to apply:
Submit a typed two-page personal statement addressing the merit qualifications listed above and that answers the question of how the scholarship will benefit you
Provide permission to the Scholarship Committee to access academic records
Provide three NCCC faculty mentor/instructors for reference
Complete an Association of Professionals Scholarship Application form available from the NCCC scholarship web page

These scholarships are not administered by the NCCC Foundation
and applications must be sent directly to the sponsoring group or
organizations.

ALICE HYDE MEDICAL CENTER AUXILIARY
SCHOLARSHIP
Applicants must be planning to pursue a course of study in a
HEALTH CARE PROFESSION and be a RESIDENT of the
area that the AHMC serves (Brushton-Moira, Chateaugay,
Malone, St. Regis Falls, Salmon River).
There are two types of scholarships:
1. Students who are starting their first year of study in a health
care profession should apply. This scholarship will be $300
and will be awarded in June at Franklin Academy.
2. Students who have successfully completed one or more
years of study and are continuing their education in a health
care profession should apply. This scholarship will be $800
and will also be presented in June at F.A..
Completed applications must include an Alice Hyde Medical
Center Auxiliary Scholarship form (found on the NCCC schol-
arship web site) plus the following:
1. Cover letter describing financial need, reason for choosing
this health care profession, career goals and current extra
curricular activities
2. One recent letter of reference, other than your New Vision
H.C. Teacher
3. Current official school or college transcript

Saranac Lake Voluntary Health Association Scholarship
Opportunity for North Country Community College Students
Enrolled in the Nursing or Radiology Tech Programs
Recipients must follow the following criteria:
Be enrolled full time and in good academic standing
Be a resident of Essex, Franklin or Hamilton County
Have been accepted to the Registered Nursing program or second year
of the Radiology Tech Program at NCCC for the 2015-16
Show care, compassion and overall excellence in dealing with patients
A Complete Application will consist of:
A brief essay describing your educational/life journey thus far
Two letters of recommendation, with at least one from a health care
professional or instructor in your academic program
Transcript of current year’s academic studies
Several $1,500 awards will be made; preference will be given to non-
traditional students going in to the RN or Rad Tech programs.

Going Back to School?
Need some help?
Three North Country foundations announce the continuation of a joint
effort to support non-traditional area students. The program targets
older students who wish to begin or continue their college education.
The Bruce L. Crary Foundation, the Deo B. Colburn Education
Foundation and the Lake Placid Education Foundation will accept
applications for the 2015-2016 academic year until August 15, 2015.
Students who wish to begin their undergraduate program, or who were
unable to complete the requirements of their undergraduate college
degree, and now plan to return to school are encouraged to apply.
Applicants must be area residents, 25 years of age
or older, and have been accepted for full-time
studies at a regional college for the Fall Semester.

The Edna A. Lauterbach Scholarship
This scholarship was established in memory of Edna Lauterbach, RN, former President of Com-
petent Care, Inc. and American Care, Inc. of Newburgh, New York. The fund is sponsored by
Community Health Care Services Foundation, Inc. (CHC), an affiliate of the New York State
Association of Health Care Providers, Inc. (HCP).
Candidate Eligibility Requirements (all requirements below must be met for
consideration):
1. Applicant must be a NYS Registered Nurse or Licensed Practical Nurse.
2. Applicant must reside in New York State.
3. Applicant must be planning or engaged in an entrepreneurial home care or commu-
nity based venture that would benefit from advanced nursing training, education or
support; or from support for other home care or community based services/products.
4. Applicant must show that proposed class/training will improve candidate’s ability to
manage their organization or venture.
5. Applicant must demonstrated financial need.
6. Applicant must submit a 200-700 word original essay including educational history,
background information and reason why this scholarship would be important to their
future success. (Not to exceed one page, typed and double-spaced, if possible.)
Selection Process:
1. Applications must be complete or they will not be reviewed by reviewing
committee.
2. The Community Foundation shares applications with an HCP committee, which
reviews applications and selects recipients, based on qualification requirements,
financial need and a well-written essay of 200 – 700 words.
3. Award winners will be notified in late August in time for the fall semester and
publicly announced at HCP’s Annual Management Conference & Exhibition
Meeting in the fall.
Scholarship Fund Disbursement:
1. Number and size of scholarships vary based on applications submitted.
2. Recipient is required to submit a letter to The Community Foundation for the Capital
Region at the end of the funded program describing the effect the class/training had on
the recipient’s business or career in home care or community-based service.
3. Payments are made directly to the college or university in which recipient is en-
rolled. If recipient attends another type of institution (i.e., computer training company),
recipient will be reimbursed the scholarship amount after submitting a paid receipt.

Scholarship Guidelines
This scholarship is for:
Adults returning to school after an absence from formal education of a
year or more
And who are residents of the Saranac Lake Central School District.
1. Applicants must have a high school diploma or G.E.D.
2. Applicants must provide financial information.
3. Applicants must fill out a short application.
4. A short history and goals statement must be submitted in a
personal letter with the application (application is separate).
5. Applicants must submit 2 letters of recommendation from
non-family members.
6. You may receive a phone call if the committee has questions.
7. Three $1,000 scholarships are available.
8. Applications and letters are due no later than May 11, 2015.
9. Scholarships will be awarded by check to the winners once the
club reviews a copy of proof of enrollment.
