What is BenefitsPlus?• BenefitsPlus is the opportunity for every benefit-eligible employee to

meet one on one with a Benefits Educator to gain a better understanding of all of the benefits offered by Fresenius Medical Care North America (FMCNA).

• Several new initiatives have been implemented for this upcoming plan year:• New e-Benefits enrollment on PeopleSoft for the upcoming company-wide

annual open enrollment

• “Benefits at a Glance” and the education around saving money on medical expenses through utilization of the Telemedicine offered to all employees enrolled in one of the FMCNA medical plans

• Continued education via the BestFit Tool around the FMCNA Consumer-Driven Health Plan (CDHP)/Health Reimbursement Account (HRA)

Who is Enrollment Advisors?• Enrollment Advisors (EA) is a benefits communication company

specializing in benefits education, enrollment and administration.

• Headquartered in Birmingham, Alabama.

• EA Benefits Educators are licensed, non-commissioned advisors who assist with raising awareness of the value of employer benefits programs by hosting individualized benefit education sessions.

• EA encourages healthcare engagement by helping employees understand their benefit options and assisting them with making informed/educated decisions.

Who is eligible?• All employees scheduled to work at least 30 hours per week are eligible

to participate in the 2015 BenefitsPlus Enrollment.• Enrollment in Medical, Dental, Vision etc. is not required for employees to enroll

in the voluntary benefits. The voluntary benefits are stand-alone, supplemental plans.

• Employees may also enroll their legal spouse or domestic partner.

• Employees may also enroll their dependent children and grandchildren.• A dependent child may be the natural child, stepchild, legally adopted child, child

placed for adoption, or other child for whom the employee has permanent legal custody.

• Coverage for dependent children ends at the end of the month in which they attain age 26 with the exception of the Trustmark Universal Life, which covers dependents up to age 23.

Hospital Indemnity – MetLife• Group Hospital Indemnity Insurance can complement your medical

coverage by helping ease the financial impact of a hospitalization. It provides a lump sum payment that can be used as the policy holder sees fit for hospital admission, accident-related inpatient rehabilitation and hospital stays.

Critical Illness Insurance – UNUM

Critical Illness insurance is designed to help employees offset the financial effects of a catastrophic illness with a lump sum benefit if an insured is diagnosed with a covered critical illness. The Critical Illness benefit is based on the amount of coverage in effect on the date of diagnosis of a critical illness or the date treatment is received.

Employee Coverage: $5,000-$50,000 Guaranteed Issue: $30,000

Spouse/Domestic Partner Coverage: $5,000-$30,000 Guaranteed Issue: $15,000

Child(ren) Coverage: Automatically receive 25% of employee amount

Accident Insurance – UNUM

Accident Insurance provided by Unum can pay benefits based on the participant’s injury and the treatment received, including emergency-room care and related surgery. The benefit can help offset the out-of-pocket expenses that medical insurance does not pay, including deductibles and copays. • On and off the job coverage

• Sickness Hospital Confinement Benefit*- pays employee, spouse or child a daily benefit if hospitalized for a covered sickness. $100/day, to a maximum of 30 days, for employees and spouse; $75/day for children.

• Family coverage options are available**: • Employee & spouse

• Employee & dependent children (up to age 26)

• Employee, spouse & dependent children

Injuries include: Expenses include:- broken bones - emergency room treatment- burns - outpatient surgery facility- torn ligaments - doctor office visit- cuts repaired by stitches - hospitalization- coma due to a covered injury - occupational therapy- eye injuries - speech therapy- ruptured discs - chiropractic visit- concussion - physical therapy

* 12/12 Pre-existing condition on the Sickness Hospital Confinement Benefit

Universal Life Insurance – TrustmarkUniversal Life – offers flexible, comprehensive benefits and enables employees to adjust their death benefits, cash value and premiums as their financial needs change.

Universal LifeEvents® – matches the needs of insured throughout their lifetime. Pays a higher death benefit during working years. At age 70, the death benefit reduces. Living benefits do not reduce.

What is your responsibility?• As the primary point of contact for your location, you should

communicate to all benefit-eligible employees the following:

• The importance of attending the enrollment meeting with a Benefits Educator, whether that be face-to-face or via the EA Benefits Service Center

• Verify they have received and read the Benefits Guide

• Remind employees to bring their dependent and beneficiary information (date of birth and SSN) to their enrollment meeting

What is your responsibility prior to on-site enrollment?

• Answer basic questions about the enrollment process

• Schedule your employees prior to the date for your on-site meeting day using the sign-up sheet provided in the Manager Booklet. If you need a new sign-up sheet, please contact Jonathan Morgan with Enrollment Advisors: [email protected].

• Inform the employees of their designated meeting times and locations within the facility/clinic
