Reporting Method: Measures Groups

• You can select the measures group of

your choice from the listing on the

reporting details page.

• When you click on the title of any one

of these measure modules to view

Measures Group information• Measure Descriptions

• Patient age range

• Diagnosis, encounter, or procedure

codes which qualify a patient for that

measure’s denominator

Measures Group Reporting Requirements

Eligible individual providers can select Measures Group reporting to satisfy 2016 PQRS requirements

• Select just ONE Measures Group per provider to satisfy PQRS requirements

• 25 Measures Groups Available for 2015

• Report on 20 of your patients seen in 2016

• 11 of which must be Medicare part B Fee-For Service (FFS) patients

• Satisfy each measure within your selected Measures Group

• CMS guidelines indicate that each measure you report must have a performance rate greater than 0%

Complete your login or new registration and then complete the Premier Solution Services Agreement to begin your 2016 PQRSwizard reporting!

Registration should be completed for each individual provider submitting a report through PQRSwizard.

If you are an administrator completing this for a provider, use the reporting provider’s information for registration.

Email Submission

• The last step for your report setup is the submission of an email address.

• This email address will be used for the purposes of distributing the PQRS feedback report, which will come directly from CMS. Feedback reporting will become available pending CMS’s timelines.

Populate Registry With Your Practice Data

• Once your account registration is complete, you progress to the data entry screen.

• You will be able to click the “Enter Data” tab to enter patients one by one.

One Form for Each Patient

• An online form will be completed for each of

the patients that you’re entering. The

questions appearing on this form collect all

pertinent information for your selected PQRS

Measures Group report.

• We validate on data points such as the

patient age and the encounter date as there

are certain measures that only apply for visits

during a certain periods of time in the year,

like the influenza vaccination measure.

• This logic is built into the PQRSwizard to

assist you in submitting a complete report.

Finished! Submit Your Report to CMS

• Once all steps are complete you are finished! The registry will submit the data to CMS on your behalf during a certain period of time in the year when CMS permits registries to submit data.

• You will have into the first quarter of 2017 to complete your PQRSwizard data entry for the 2016 reporting period.

• Check the PQRSwizard homepage for the reporting deadline for the 2016 reporting period.
