    Welcome to the fourth edition of the Clarence Get Going Guide.

    Community feedback continues to show that this guide is a valuable resource in helping to find out what is going on for older people in Clarence.

    With every new version, the guide continues to respond to the various requests for new and relevant information, and this edition is our biggest yet.

    As the first Tasmanian Council to be named as an Age Friendly City and Community by the World Health Organisation (WHO), it is a timely recognition of Council’s efforts to involve and cater for people at all stages of life, regardless of their ‘age’.

    The Clarence Positive Ageing Advisory Committee (CPAAC), comprised of older community residents from all areas of Clarence, and representatives from key service providers, supports Council in its commitment to produce this resource every two years. 

    On behalf of CPAAC, we hope that the Get Going Guide continues to enlighten you as to what activities are available across the City.  And remember, there are many other opportunities in Clarence just waiting for you to discover them!

    Clarence City Council joined the World Health Organisation (WHO), Global Network of Age friendly Cities and Communities in 2014, as the first and currently only Tasmanian city to do so.

    The network currently includes: 210 cities and communities in 26 countries, covering over 88 million people worldwide.

    Council has a number of wonderful products, programs and facilities to help support its growing older population across the City.

    You will find all contact details and further information in the ‘Get Going in Clarence’ section, from page 44.

    Council provides: Get Going Guide – this information resource is produced every two years.

    Spotlight on Seniors – a newsletter produced six times a year to provide key information; convey positive messages and celebrate the achievements of older people; and provide the opportunity for older people to give advice and ideas on issues that are important to them locally.

    Community Grants Program – providing access to resources including older person groups and organisations in Clarence.

    Council supports: Clarence Positive Ageing Advisory Committee – a special committee of Council consisting of older community residents and service providers who are active in assisting Council to implement the actions of its Positive Ageing Plan.

    Access and Inclusion – while Council does not provide direct services to people with disabilities, Council is committed to addressing the access and inclusion needs of people who have a disability or mobility issue.

    Disability Access Advisory Committee – a special committee of Council consisting of residents with a disability, or who care for a person with a disability, and Council and service provider representatives, who are active in assisting Council to implement the actions of its Access Plan.

    Clarence Planting Ahead Program – assisting clients of the service to transform their gardens to more low maintenance and easy access, and working with the Clarence Positive Ageing Advisory Committee on community education.

    Council operates and manages with local committees:

    Clarence Seniors and Citizens Centre and Alma’s Lounge 17 Alma Street, Bellerive Phone 6244 4264 Located in Bellerive, the centre has plenty of off-street parking, great facilities and a variety of activities for people over 50 years old. New members are always welcome. The hall is available for hire and seats over 200 people with full bar and facilities, catering for birthdays, anniversaries etc.

    Howrah Recreation Centre 11 Howrah Road, Howrah Phone 6247 7711 Located in Howrah near the Shoreline Shopping Centre, the centre provides a quality facility for use by many community groups, including many groups and activities catering to older people. Facilities include meeting rooms, a large multi-purpose hall accommodating over 200 people, a fully equipped bar and large dance floor.

    Lindisfarne Community Activities Centre 37A Lincoln Street, Lindisfarne Phone 6243 5810 Located in the heart of Lindisfarne Village, the centre provides an ideal location for many community groups and activities that cater to the needs of older people. Facilities include a library, a large multi-purpose hall, flexible meeting spaces and kitchen facilities.

    Selecting a new home or planning changes to an existing home?Purchasing or planning changes to an existing home is something that needs careful consideration .

    Some things to think about:

    Choosing a location Access to services: is the home close to transport, shops, and support services? Is it a busy street?

    Finance Are the purchase/rent/alterations and maintenance costs within your financial capabilities? Enquire about support services.

    Maintenance Is the maintenance within your physical capabilities? Are community-based services available?

    Outside Slope of land: preferably flat; ground surfaces: slip-resistant preferable; level car parking space and unrestricted access to door; maintenance required: painting, gardening, repairs; access: width of doorways, paths, steps etc; utilities: garbage disposal etc; security: locks, alarms, visual access, lighting.

    Inside Doors and windows: width, type; circulation space; floor surfaces; lighting; electrical wiring; heating and ventilation; plumbing; kitchen and bathroom: accessibility, space, layout, heights of benches etc.

    Accessing InformationSometimes we don’t know what information we need, until we need it . Now, if we have access to a telephone and a computer we have information available ‘at our fingertips’.

    My Aged Care was introduced on 1 July 2013 to assist older people and their families and carers to access aged care information and services.

    The My Aged Care website was established by the Australian Government to help navigate the aged care system.

    My Aged Care is part of the Australian Government’s changes to the aged care system which have been designed to give people more choice, more control and easier access to a full range of aged care services.

    My Aged Care is made up of a website and a contact centre. Together they can provide you with information on aged care for yourself, a family member, friend or someone you’re caring for.

    You can call the My Aged Care contact centre on 1800 200 422 between 8am and 8pm on weekdays and between 10am and 2pm on Saturdays. The My Aged Care contact centre is closed on Sundays and national public holidays.

    Independent Living Centre (ILC) This is a free statewide service with information about thousands of items of equipment on its database including:

    ✦ scooters, wheelchairs and electric recliners

    ✦ things to help you do household tasks (e.g. cooking, washing and shopping)

    ✦ items to make bathing, dressing and personal care easier and safer

    ✦ office seating and ergonomics

    ✦ specialised products for recreation and leisure

    Product information includes a detailed description, suppliers and prices, and a picture. They do not sell equipment.

    The ILC can advise on building modifications for access and safety.

    They also have a list of hire agencies and people who are selling equipment second hand.

    The display centre holds examples of many items you can buy in Tasmania. You can see or try them at the centre (please make an appointment).

    Groups can visit the centre, or centre staff or they can come to you for education about products or specific conditions or equipment tailored to your requirements.

    Home Care ServicesBeing able to stay living in your home and being as independent as possible is what most people want as they age .

    Our needs change as we get older and sometimes we require some additional support to remain living at home.

    What is the Age Care Assessment Team (ACAT)?If you need some help at home or are considering moving into an aged care home, you may first need a free assessment by an Aged Care Assessment Team (ACAT). A member of an ACAT will talk to you about your current situation and work out if you are eligible to receive government-subsidised aged care services.

    You will need an ACAT assessment and approval if you have complex aged care needs and want to:

    ✦ access aged care services through any level of Home Care Package

    ✦ receive services through transition care

    ✦ receive respite care in an aged care home (sometimes known as nursing home)

    ✦ move into an aged care home

    You will need a Home Support Assessment if you have entry-level aged care needs and want to access services through the Commonwealth Home Support Program, such as meals or transport.

    You won’t need a Home Support Assessment or an ACAT assessment for aged care services that are not subsidised by the Australian Government such as services offered by volunteer groups and charitable organisations.

    What is a Home Care Package?The Home Care Packages Program is part of the Australian Government’s continuum of care for older Australians. It provide older people who want to stay at home with access to a range of ongoing personal services, support services and clinical care that help them with their day-to-day activities.

    The Program sits in between the Commonwealth Home Support Program (formerly known as the Home and Community Care (HACC) Program) and Residential Aged Care.

    What levels of care are available?There are four levels of Home Care Package:

    ✦ Level 1 – to support people with basic care needs

    ✦ Level 2 – to support people with low level care needs

    ✦ Level 3 – to support people with intermediate care needs

    ✦ Level 4 – to support people with high care needs

    What types of services are provided?The services that can be provided in a Home Care Package include, but are not limited to:

    Support services – such as help with washing and ironing, house cleaning, gardening, basic home maintenance, home modifications related to your care needs, and transport to help you with shopping, visit your doctor or attend social activities

    Personal care – such as help with showering or bathing, dressing and mobility

    Nursing, allied health and other clinical services – hearing services and vision services

    Care coordination and case management – helping to make sure you receive holistic services tailored to your needs

    What is consumer directed care?

    Since 1 July 2015, all Home Care Packages are being delivered on a Consumer Directed Care (CDC) basis. If you are already receiving a Home Care Package, your service provider will discuss what this means with you at the next review of your care plan.

    CDC means:

    ✦ you get more say in the care and package you access, how it is delivered and who delivers it to you

    ✦ you will have conversations about your needs and goals

    ✦ you will work in partnership with your service provider to develop your care plan

    ✦ you agree to the level of involvement you will have in managing your care package

    ✦ you will have a greater understanding about how your package is funded and how those funds are spent through your individualised budget and monthly income and expense statement

    ✦ your service provider will monitor and provide you with ongoing formal reviews to ensure that your package still meets your needs

    Care Assess is a private healthcare organisation, specialising in health assessment and coordination of community care. They employ registered nursing health professionals as care coordinators in order to provide clinical care for clients who need complex care in the community (living at home).

    Community Based Support (CBS) provides in-home, centre-based and community-based support to frail older people, people with a disability, people with mental illness and their carers to enable them to remain living independently in the community.

    Members of the veteran community are able to access Home Care Packages in the same way as other members of the ageing community.

    Additionally, Day Clubs provide opportunities for the aged, the isolated, people with disabilities, carers and volunteers, to develop and maintain social contact outside the home through activities such as games, sports, fitness, information sessions and arts and crafts.

    The Day Clubs program is run nationally and is administered and operated by DVA and ex-service organisations. Each club tailors its activities to the interests and skills of its members.

    The service offers a range of in-home services to support the frail aged and people with disability throughout Hobart and surrounds, providing individualised care to assist people to remain at home.

    They provide assistance to access packages that help support quality care at home including: home help; home care assistants; respite; veterans home care; and handyman services.

    The Home Care Packages are designed to give older people with complex care needs the help they need to stay living in their own homes.  Packages can provide a range of services at times to suit the individual.

    In order to be eligible for a Package, you must have been assessed by the Aged Care Assessment Team (ACAT). 

    The Dementia Respite service is available to the carers of people with dementia who exhibit challenging behaviour.  Priority is given to carers living in remote locations and to people from culturally and linguistically diverse backgrounds.

    The Aged Care Team offer clients a variety of social outings throughout the year.

    Uniting AgeWell offers a range of services to enable older people to remain active and independent, and living in their own communities, including Home Care Packages, Centre-based Respite for Home and Community and National Respite for Carers. Services and programs include referrals to physiotherapy, occupational therapy, falls prevention, health promotion education and social activities.

    Lillian Martin Community is a peaceful residential home, located just a few minutes from Hobart. It offers 93 residential places, including dementia-specific care and couples accommodation.

    There are also eight Independent Living Units and seven Social Housing Units located onsite, as well as a Day Respite Centre for leisure and social; and a Men’s Shed.

    South Eastern Community Care (SECC) is a not-for-profit organisation providing services to frail aged and younger people with disabilities and their carers. SECC is committed to providing a range of flexible home care services in rural and regional Tasmania, predominately in the south of Tasmania, including Clarence.

    They offer in-home and centre-based care, including assistance, support and education to eligible clients and carers, to assist them to remain in their own homes and communities.

    Advance Care PlanningAdvance care planning offers people the opportunity to discuss with family members, carers, friends and health professionals the choices for care at the end of life. Through age, illness or accident, people may lose their capacity to understand the nature and consequences of proposed medical treatment. Healthcare decisions then fall to families, friends and health professionals who may not be aware of your wishes.

    You can create an advance care plan that may include:

    ✦ an Advance (Health or Care) Directive (or other similar document)

    ✦ an enduring power of attorney (or other similar document)

    ✦ a letter to the person who will be responsible for your decision-making

    ✦ an entry in your medical record

    ✦ instructions which clearly voice your view

    The advance care planning process encourages you to reflect on what is important to you, your beliefs, values, goals and preferences in life. It explains how you want to be cared for if you reach a point where you cannot communicate decisions about your medical care. It keeps you involved in your medical decisions, both now and in the future, whether you are healthy or have an illness.
