Sts. Joachim & Ann Care Service is the largest social service provider for individuals, families and children in the Tri-county area of St. Charles, Lincoln, and Warren. Our motto is... planting the seed of hope, breaking the chain of despair. We work with a purpose to protect families, especially children, the elderly and disabled, regardless of race, age, gender or religion from the despair, hopelessness and social injustice caused by poverty.

My Dear Friends,

The mission of Sts. Joachim and Ann Care Service, to serve those in crisis and to prevent homelessness and hunger, has been met with open arms by the many companies, agencies, and volunteers that continually respond to the needs throughout the community at large.

This past year the Care Service continued building our Street Outreach program. Many thanks to both the Missouri Housing Development Commission and the Missouri Foundation for Health for recognizing the need for this service and providing the funding to launch this program in 2016. Social Service Workers are able to meet clients where they are and provide emergency services. In addition, because of our relationship with SSM Health Care Systems, health assessments are performed by qualified health workers. This year we added a second team, allowing a deeper reach into the Tri-County area and a speedier response to the needs in the community. Last Spring, the Care Service undertook a project entitled “In Plain Sight” to bring awareness of the homeless living in the Tri-County area. This project opened the eyes of the community to recognize that homelessness exists--in our plain sight.

Our transitional housing program expanded with the addition of two houses. As part of our growth, the Care Service hopes to continually add housing inventory that can be used to serve our clients. We are also always in need of landlords who will assist in housing clients that are below 50% of the area median income level. Housing is a major key in family stability so if you can lend a hand in assistance please keep the Care Service in mind.

The Children and Family Development (CFD) Program continues to effectively assist very vulnerable clients with comprehensive case management. In 2017, 552 children within 308 households were served within the CFD program. Many thanks to the Community and Children Resource Boards of St. Charles and Lincoln counties for providing the funding to make this possible.

Our Food Pantry continues to build on its client choice program. We are discussing the opportunity for nutritionists to periodically be onsite in the pantry to assist our clients in understanding the importance of good eating habits and food choices. We are excited about the community garden to be added in 2018 that will provide fresh garden vegetables to our pantry.

Additionally, we were fortunate to receive a program sponsorship from Mercy Health Systems for our Adopt-A-Family program. In serving the 533 families who benefited from this program, the sponsorship, as well as the many in-kind services from the community, helped assure that once again this program was a huge holiday success.

In 2017, there were no disaster declarations in the tri-county area which enabled our Long Term Recovery team to wind down and bring closure for the 400 families affected by the December 2015 flooding. The Care Service and the other agencies making up the Long Term Recovery committee are always prepared to serve those in need when and where crisis occurs.

This coming year, the Care Service will be under re-certification by the Council on Accreditation (COA). This certification indicates that the Care Service operates using the best practice principles in our industry, which makes the Care Service a stronger agency for our clients and stakeholders. Please keep us in your thoughts and prayers as the accreditation process progresses.

The Care Service addresses the comprehensive and complex needs of families and individuals facing crisis, such as homelessness, hunger, mental and physical health disorders, lack of basic necessities, untimely death, and multiple issues that confront families who are dealing with poverty. Utilizing integrated services, we work with our clients to achieve stabilization and systemic change in their lives, which also impacts the overall health and preservation of our local communities. Disaster Recovery - We are a first responder, frequently called to assist in the immediate and long-term coordination of community-wide disaster efforts, and we are a member of Community Organizations Active in Disaster (COAD).

Did You Know?Our Street Outreach Program served 114

individuals leading to 321 encounters on the streets in 2017. As a result of the program’s efforts,

21 of those individuals were moved intopermanent housing.

• 86% of clients surveyed were still residing at the address where assistance was last received (this is a slight increase from 83% last year).

• 95% of total clients surveyed stated they were currently living in a safe, secure, and stable home environment (consistent with last year’s result).

• 52% indicated they were able to keep up with their utility payments since assistance was last provided. This is a significant improvement over last year’s 34%.

• 75% of clients surveyed were able to keep up with their rent/mortgage payments since assistance was last provided. This is another significant increase over last year’s 64%.

• 89% of those surveyed stated that the housing assistance provided to them by the Care Service had a great impact on their household.

• 89% were very satisfied with the quality of care they received from the Care Service case management staff.

The Care Service conducted follow-up surveys with its clients approximately 3-6 months after they have received financial housing assistance. Approximately 56% of clients who received this assistance were at or below the 25% area median income level. Calls were made to 492 households that received housing-related financial services during this time period. We were able to reach and survey 54%, or 264 households, of the 492 total. This report represents clients who were served from April 1, 2016, to March 31, 2017, covering 12 months of service.

Chad & DawnChad and Dawn were both working full-time in the fast food industry, but still struggling to make ends meet. The couple, who are parents to three children under the age of six, picked up as many extra shifts as possible, but came to the Care Service facing an eviction.

Both Dawn and Chad struggled with past drug addiction and each survived traumatic childhoods. After finding each other, the couple was able to overcome their addictions and have been married for several years. Dawn was pregnant with their fourth child, and late in the pregnancy she was put on bedrest. The already struggling family lost an important source of income. The Care Service was able to step in and assist with rental arrears and several months of rent assistance. The Social Service Worker (SSW) was also able to advocate for the family with local agencies and churches to obtain funding to help with utilities. In addition, the family was signed up for ‘No Hunger Holiday,’ and the Adopt-A-Family program.

The couple’s oldest child Sam was struggling with school, and during a home visit the SSW was able to educate Dawn that she could request testing to see if Sam had a learning disability. Dawn had Sam tested at school, and found that he did learn differently. An Individual Education Program (IEP) was put into place to help him catch up and enjoy learning.

The new baby made his appearance before Christmas, and the family was finally beginning to feel as though things were falling into place. A week after Christmas, Dawn found several small lumps in her breast. What doctors thought was mastitis from the birth, turned out to be cancer. Further testing showed that the lumps were Stage 3 breast cancer and that Dawn carried the BRCA gene. She opted to have a double mastectomy with chemo and radiation. The SSW was able to provide resources for her regarding her cancer diagnosis and for help with the bills as she went through her treatment.

The family stopped in after January to let the SSW know that they had moved in with a family member so that they had support and assistance with child care. The family reported that after Dawn’s treatment that they would apply for an USDA loan to buy their first home. Through the course of working with the couple they never asked “why me?” They would smile, and tell the SSW that the “Lord has us.” Their faith and ability to maintain a positive spirit was a blessing.

SATISFACTION

The Care Service conducted a Food Pantry survey to give clients the opportunity to share their opinion about the program, and to provide the Care Service with information to make improvements to service delivery. Additionally, the 2017 survey provided insight into how changes implemented in 2016 (Client Choice and expanded hours) were viewed by clients. Those surveyed were asked about their Food Pantry experience over the previous 12 weeks.

Did You Know?In Missouri, 947,900 people (including 258,610 children) are struggling with hunger. In St. Charles County 10.4% of the population faces food insecurity. In Lincoln County, 13.5% of people wonder where the next meal will come from. In Warren County, 12.8% of the people face a daily hunger struggle.

Kelly Kelly had just lost her job when she came to the Care Service for assistance with rent and utilities. The single mom of a nine-year-old daughter had been fired from her position after missing too much work caring for her daughter who had been sick several times this year.

Without a job, Kelly’s only income was about $300 a month in SSI for her daughter. Her ex had not been paying his court-ordered child support. Her rent was past due and accumulating late fees daily, and her electric was about to be shut off. Her mother had provided Kelly with financial assistance, but was unable to assist any further. The Care Service was able to pay her utility bill to avoid disconnection and get her rent caught up to avoid the eviction.

Her Social Service Worker (SSW) signed the family up for the food pantry to stretch the budget, and Adopt-A-Family for the upcoming holiday. Kelly applied for numerous jobs daily and had several interviews. Within a month of losing her job, Kelly secured new employment making $18 an hour.

Her SSW helped Kelly create a budget with her new income, and assisted with finding alternative child care if her child became ill. In addition, the SSW provided clothing vouchers for new work clothes and signed Kelly up for Dress for Success. Kelly was also provided with a gas gift card and referred for a free oil change.

Kelly was awarded back child support which she used to pay back her mother. She is now working full-time and managing to stay on track with her budget—and saving along the way for any unexpected expenses. 9

SONNET

Sonnet has lived on the street for the last three years, travelling across the country with friends. Most recently she took shelter in a tent popped up under a bridge in St. Charles County.

“I wish the general public understood that it could be you. One bad choice can change your life,” said Sonnet. “Just acknowledge the fact that we are human.”

Sonnet took photos of her friends which she considers family.

“Family isn’t always blood,” said Sonnet.

She also took photos of her living space—under the bridge—and the places she travels for food and showers. Her bright orange tent tucked into the corner of the bridge shows the reality of her only shelter. Taking a shower is not a simple task, but requires Sonnet to travel to a park restroom or truck stop to clean up.

Her favorite photos, however, are of her friends sleeping. Sleep is a brief respite for those living on the streets.

“It’s the most peaceful time—for a short time,” said Sonnet. “It’s a break from reality; especially when you are sober—that’s why a lot of people turn to drugs and alcohol.”

Sonnet hopes to one day be able to see her kids again and have a relationship with them.

“We all make mistakes and one wrong decision could leave you in the same situation,” said Struckhoff. “If we are able to raise awareness of homelessness in our community and start conversations about the real issues we see in the community then I think we’ve done our job.”

The In Plain Sight project raised more than $30,000 that will be directly used for families and individuals who are homeless in St. Charles, Lincoln and Warren counties.

To expose the reality of homelessness in our community, Sts. Joachim & Ann Care Service unveiled its In Plain Sight project in April 2017. The project provided 100 homeless members of the community with a disposable camera and a mission—to tell their story from the street.

“People pass them every day, but one of the biggest questions I am asked is- are there really homeless people in our area? We want to shine bright light exposing homelessness in our area, and provide an outlet for members of our community who are not hiding, but are In Plain Sight,” said Pam Struckhoff, director of program services.

Struckhoff said the goal of the In Plain Sight project was to bring about awareness of homelessness in our neighborhoods, and provide the homeless with a voice through a camera lens.

“These men and women, who volunteered as photographers, are an integral part of providing a visual of the harsh reality of homelessness in St. Charles, Lincoln and Warren counties,” said Struckhoff.

Photographers had seven days to tell their story through pictures before returning the camera to the Care Service. The photographers received a t-shirt with ‘photographer’

emblazoned on the back, a $10 fast food gift card and a backpack stocked with toiletries for their participation in this innovative and eye-opening project.

The photos were developed and a team of community judges chose the top 20 photos which were on public display at the St. Peters Cultural Arts Centre from June 29-August 20. Visitors of the display also had the opportunity to vote for their favorite photo by way of donation while taking a self-guided auditory tour that explains the backstory of the photo. The photos were also available to view online at InPlainSight.live starting on June 29.

In Plain Sight culminated with an evening event on August 19 that included a live auction of the portraits at the St. Peters Cultural Arts Centre. The top three winning portraits received a beneficial prize based on their individual needs. Additional photos from the homeless photographers are available as a travelling display to local venues, churches and community agencies following the auction.

THOMAS

Two years ago, Thomas was running his own business and supporting himself without any problems. Unfortunately, the business failed which left him without any income. Thomas is now homeless for the first time in his life. He has been living in his van for nearly two years.

Thomas has captured pictures that tell the story of his life on the street. His photos show some of the local bars where he parks his van to sleep at night, the Salvation Army where he can get a hot meal and the United Methodist Church where he can take a shower and eat breakfast. He also visits a weekly food pantry to supplement the two hot meals from Salvation Army, and spends time at local libraries utilizing the computers to look for jobs.

Thomas hopes people learn from his photos that “anything can happen.” He wants his photos to bring awareness to St. Charles County that homelessness is right here. His shot of his van—with the window duct-taped—in the empty parking lot shows one of the places he parks to sleep at night.

“It’s not all about the people living under the bridge and holding up signs,” said Thomas.

Thomas dreams about stabilization and getting back to work. He wants to be able to make it to August so he can get Social Security.

“I’m used to living on so little,” said Thomas.

The Food Pantry Program served 500 unduplicated households resulting in 6,703 fooddistributions throughout 2017.
