Your Enrollment ChecklistUnderstand your benefit options for 2017 – we encourage you to take the time to review the enrollment materials made available to you so that as you consider your healthcare and financial needs, you can make the best decisions for yourself and your family. All enrollment materials can be found at ChemoursBenefits.com.

Consider your coverage needs. Don’t select more coverage than you need or overlook benefits that provide you with cost saving opportunities.

Compare your medical options. The two 2017 medical options, Choice Plan and the Choice Plus Plan, work similarly, but there are some differences – you can either pay more in premiums (pay more out of each paycheck) and less when you get healthcare services (lower deductibles and out-of-pocket maximums apply) or you can pay less in premiums and pay more when you get healthcare services (higher deductibles and higher out-of-pocket expenses).

* "Never lose it" refers solely to the portability of the account. Investing involves risk, including possible loss of the principal value invested.

Health Savings Account Make a smart investment in a Health Savings Account (HSA) – Save it. Use it. Never lose it.*

When you enroll in a Chemours medical plan, you become eligible for a Health Savings Account.

When you open an HSA, it is your personal savings account, the money stays with you for the rest of your life, and you can use the HSA Visa debit card that comes with your account to pay for qualified Healthcare expenses, such as doctors’ office visits and annual physicals, deductibles and coinsurance.

An HSA offers the “triple tax advantage”: pretax contributions, tax–free gains on any money your HSA savings earn and tax–free withdrawals when you use the money for qualified medical expenses.

After you verify your eligibility when you enroll in an HSA, Chemours will make deposits to your HSA up to $600 if you cover yourself, or up to $1,200 for other coverage levels. The company HSA deposit will be prorated based on your date of hire. You too can contribute to your HSA up to the 2017 IRS limit by way of payroll deductions – up to $3,400 for individual coverage and up to $6,750 for family coverage. The IRS HSA Contribution Limits include your contributions and company contributions. HSA catch–up contributions for those who are age 55 or older is $1,000.

You can use the money in your HSA for eligible healthcare expenses as they occur or you can choose to save the money in your HSA for future eligible healthcare expenses, such as when you retire. Remember any unused HSA funds roll over year after year. Keep in mind that you can change your HSA contribution amount at any time during the plan year and you can also start and stop your HSA contributions at any time during the plan year.

Flexible Spending AccountsFlexible Spending Accounts allow you to pay for certain out–of–pocket expenses with pre–tax dollars. You choose the amount that you want deducted from each paycheck.

Eligible Limited Purpose Health Care FSA expenses include:

Prescribed dental and vision products

Non–cosmetic dental care, orthodontia, eyeglasses, contact lenses and laser eye surgery

Co–payments, coinsurance, and deductibles under dental and vision plans

Special Eligibility Rules Apply to the HSA

If you are covered in 2017 by another health plan that does not qualify as a high-deductible health plan or by Medicare, the IRS requires that you decline enrollment in an HSA.

Limited Purpose Health Care Flexible Spending Account (FSA) The Limited Purpose Healthcare FSA can only be used for eligible dental and vision expenses. The IRS 2017 annual contribution limit for a Limited Purpose FSA is $2,600. If you are enrolled in a Chemours high–deductible medical plan, you cannot use your Limited Purpose Healthcare FSA for medical expenses. Remember to plan carefully – the Limited Purpose Healthcare FSA is a “use it or lose it” benefit.

Dependent Care FSA A Dependent Care FSA allows you to set aside pre–tax money to pay for eligible day care expenses for your dependent child under the age of age 13 or for a spouse/dependent age 13 or older who is not able to take care of him/herself. These day care expenses must be incurred because you have to work and are not available to care for your dependent child yourself. The Dependent Care contribution maximum for 2017 is $5,000.

For more information about the HSA or FSA visit MyChemoursBenefits.com or bankofamerica.com/benefitslogin.

Commuter FSAA Commuter FSA allows you to set aside pre–tax money to pay for eligible commuter expenses. Chemours offers a Commuter FSA administered by Discovery Benefits - you can save money on your commuting costs by having funds deducted from your paycheck pre-tax. You will receive a Visa debit card from Discovery that you can use pre-tax dollars (subject to monthly limits) to pay for your commuting expenses via your Commuter FSA debit card. The IRS maximums for 2017 are $255 per month for mass transit and $255 per month for parking. You can enroll in the Commuter FSA or make changes to your Commuter FSA payroll deductions at any time during the year.

When you call ComPsych, the EAP administrator, you will speak confidentially with an EAP consultant. The EAP consultant will assess your situation and if necessary, refer you to an EAP network provider who will meet your needs. Six (6) free EAP counseling sessions apply to each unique situation that is assessed by the EAP as a short-term counseling need.

If additional care is needed beyond the six (6) free EAP sessions, you may be eligible to continue treatment under the Chemours medical plan. Your medical plan coverage also includes mental health and chemical dependency benefits coverage.

Out–of–pocket maximum example

If you’re a family of four in the Choice Plus Plan option, you can collectively reach your out–of–pocket limit with expenses that total $10,000, or any one family member can reach a $5,000 limit.

If you or one of your dependents is in an active course of treatment you do not need to contact the EAP, simply contact Aetna (800) 417-2386 or Highmark (866) 730-8592 to find out if your current doctor or provider is in the network.

If you need help to find a new doctor or provider, after you get a list of providers from Aetna or Highmark, contact Health Advocate at (866) 695-8622 for help to find the provider that's right for you.

DeductiblesYour deductible is based on your coverage level. Employees with “You only” coverage have a lower deductible than employees who elect to cover eligible family members on their plan.

If you cover a spouse and/or children, the “other coverage” deductible applies and all family members’ claims go towards meeting the deductible.

Out–of–pocket Maximums You’ll have two types of financial protection if you or your dependents expect high medical plan claims next year—an in–network out–of–pocket maximum and an out–of–network out–of–pocket maximum.

The most you will pay out–of–pocket for all of your in–network covered medical, mental health, chemical dependency, and prescription drug expenses for your family will be:

Choice Plan In–network: $12,000, limited to $6,000 for any one family member.

Out–of–network: $15,000 individual and $30,000 other coverage levels

Choice Plus Plan In-network: $10,000, limited to $5,000 for any one family member

Out-of-network: $15,000 individual and $30,000 other coverage levels

How the Prescription Medication Deductible WorksPrescription medication expenses are subject to your annual medical plan deductible. This means that you will need to pay the full cost of your prescription medication at the negotiated discounted rate until you reach your annual medical plan deductible.

Remember, if you are eligible to participate in the HSA you can use the company contribution deposited into your HSA to help pay for your prescription medication expenses. For 2017, company HSA contributions are prorated based on your date of hire, up to $600 for individual or up to $1,200 for other coverage levels (subject to HSA eligibility rules.)

Your medical plan deductible does not apply to the cost of your preventive care prescription drugs. Although no deductible applies to preventive medications, you will still be responsible for coinsurance for brand–name preventive medication. Preventive generic medication is provided at no cost to you.

ID CardsMedical: If you enroll in a Chemours medical plan a new medical insurance ID card will be mailed to your home address. If you need to get healthcare services and you have not yet received your medical ID card, you can print a temporary card by accessing aetna.com or highmarkbcbsde.com.

Health Savings Account: If you enroll in the HSA you will receive a welcome packet from Bank of America and a Visa Debit card that you can use for eligible healthcare expenses for 2017.

Flexible Spending Account: If you enroll in the Limited Purpose Healthcare FSA you will receive a Visa debit card from Bank of America that you can use to make eligible dental and vision expenses (the Limited Purpose Healthcare FSA cannot be used to pay for medical expenses).

Dental: If you enroll in the dental plan you will receive a MetLife Dental ID card.
