Welcome to the Primary Care Pharmacy Association Conference 2018. We really value your participation and hope you will take the opportunity to make the most of the networking opportunities with each other and with our speakers today.

PCPA is here as a professional network for all of us working across the breadth of primary care pharmacy - sharing best practice and practical ideas and solutions; encouraging and developing links with our specialist colleagues in the acute, community and mental health services to help ensure best care and outcomes for our patients.

The focus of today is Patient Safety which is a priority for the NHS and for global healthcare systems. Patient Safety is fundamental to quality systems of care delivery and we have a range of expert, excellent speakers and workshop leaders who will inspire, inform and challenge your thinking and knowledge. Our keynote speaker today is Richard Cattell, Deputy Chief Pharmaceutical Officer, NHS Improvement, who is leading on patient safety improvements including reduction of medication errors.

PCPA continues to go from strength to strength and has a growing membership who share member resources via the website at www.pcpa.org.uk . Last year we were proud to announce that our virtual support network had nearly 700 participants – it now has over 1,100 members – pharmacy professionals new to primary care seeking support, specialists sharing their valued insights, practice pharmacists reaching out to improve care and share practical solutions and community pharmacists linking in to inform and contribute their expertise.

Please join as a full member so that we can continue to grow the support we can provide to you. In the past year our Practice Pharmacy Group chaired by Ravi Sharma has continued to grow in influence and popularity as have our PCPA Care Homes Group chaired by Dr Graham Stretch FRPharmS, PCPA Community Pharmacy Group chaired by Luvjit Kandula FRPharmS, PCPA Clinical Specialist Group chaired by Lelly Oboh and the PCPA Pharmacy Technicians Group chaired by Helen BirchallMy grateful thanks go to all of our speakers and workshop leaders today and to our exhibitors who make these events possible.

Osama Ammar - Head of Continuing Fitness to PractiseOsama Ammar is Head of Revalidation at the GPhC. He led the work to pilot and consult upon revalidation and is now working to implement the model in phases over the next few years. Osama has thirteen years of experience working in professional regulation; holding posts at HCPC and NMC before joining the GPhC.

Liz Butterfield – PCPA PresidentLiz has extensive experience as a primary care pharmacist and hospital pharmacist in senior and consultancy positions. Liz is qualified as an independent prescriber and encourages PCPA members in the development of this role. Liz is an elected member of the Royal Pharmaceutical Society’s English Pharmacy Board and was the EPB Board Lead for Medicines Optimisation. She represents the RPS as a member of the National NHS England/ABPI Steering group for Medicines Optimisation. Liz was awarded Fellowship of the RPS in 2014 for services to the profession of pharmacy. Liz is raising the profile of members of the PCPA by lobbying with other professional and patient groups to highlight the unique contribution that primary care pharmacists can make to better outcomes from medicines by working alongside GP’s and nurses in general practice.

Liz is currently managing a care home medication review service in Brighton and Hove for 1,250 elderly residents with a focus on reducing problematic polypharmacy and improving outcomes. Liz is also Project Lead for a Polypharmacy Project across Kent Surrey and Sussex for the Academic Health Science Network, working closely with NHS Specialist Pharmacy Services.

Richard Cattell – Deputy Chief Pharmaceutical Officer, NHS ImprovementRichard is the Deputy Chief Pharmaceutical Officer at NHS Improvement, working to the Medical Director of NHSI and Chief Pharmaceutical Officer. His key objectives include supporting trusts who are struggling with the medicines elements of care quality, driving the delivery of 7 Day Services Clinical Standards and the delivery of the Medicine Safety Programme (the NHS response to the WHO Global Safety Challenge – Medication without Harm). His role is also a key part of the system wide senior pharmacy leadership team.

Richards career has primarily been in acute hospital pharmacy, culminating in being Chief Pharmacist at two different acute and community provider Trusts, and Chief Operating Officer at two similar provider Trusts. He has previously been President of the Guild of Healthcare Pharmacists and is currently Chair of Trustees of UKCPA and a Fellow of the Royal Pharmaceutical Society.

Robin Conibere – PCPA Practice Pharmacy Group Southern LeadRobin Conibere is a Practice Pharmacist at Beacon Medical Group, a large Group Practice in Plymouth and Devon. He conducts clinics as part of the Urgent Care Team, with a GP, an Advanced Paramedic Practitioner and a Nurse Practitioner to help man-age “on the day workload” for the Group. He is an independent prescriber and helps manage patients with long term conditions and Polypharmacy issues. Robin has a background in Community Pharmacy and is keen to integrate with all pharmacy sectors to improve patient care. Beacon

SPEAKERS PROFILESMedical Group are a large Innovative Group of Practices who are looking to explore new ways of working in General Practice to provide better patient care and have recently been announced as a Primary Care Home Rapid Test Site. Tess Fenn – President, APTUKDuring her career Tess has been an active member of the Association of Pharmacy Technicians UK. She has held the posts as Education Officer (2004) and Vice President (2011) before being elected as President in June 2014. In her role as President she leads the Board of Officers and the Executive in setting the strategic direction of the association. She is also responsible for the overall organisation and management of the Association. Tess began her career as a pharmacy technician in 1970 and has worked in a variety of pharmacy sectors; hospital, academia, community and PCT. Tess currently practises as Pharmacy NVQ & Technical Staff Development Manager at Guy’s and St Thomas NHS Foundation Trust, London where she leads on support staff development, CPD and quality assurance of NVQ/QCF provision. Tess is the APTUK representative on the Health Education England Advisory Group for Pharmacy and an observer member of the London Pharmacy Workforce Group.

Anna Hogkinson – PCPA Diabetes ChairAnna has recently been appointed as Consultant Pharmacist for Diabetes and works in a multidisciplinary intermediate care team to improve outcomes for people with Type 2 diabetes. Anna’s day to day role includes development of local and sector wide guidelines as well as undertaking community diabetes clinics, focusing on those with complex medication regimes, multi-morbidity and problems with adherence. Anna has worked in primary care for 15 years and has undertaken a number of roles including practice based pharmacist, service redesign manager and senior commissioning pharmacist. Anna has also worked as a clinical lead for local and sector wide diabetes transformation programmes developing a number of resources to support colleagues in primary and secondary care support people with diabetes. Anna completed her Masters in Diabetes in 2016, focusing on GLP-1 analogue use in her final dissertation. Anna is keen to work with, and support colleagues working in primary care and share learning to improve outcomes for people with diabetes.

Salmia Khan – PCPA Practice Pharmacy Group – Technician South East LeadSalmia started her pharmacy career at the age of 17 in a local pharmacy, and worked her way up from there. She worked for Boots for 7 years, completing the NVQ3 and ACPT training and was a store manager for 3 years before moving on to work for North West Surrey CCG. Salmia is currently a GP Pharmacy Technician based in Woking. In this role she has trained as a phlebotomist, completed training to administer vaccines and runs an anticoagulant clinic once a week. Salmia leads on medicines reconciliation for the practice and is developing CQC/Practice policies to help implement medicines management processes.

Helen Kilminster – PCPA Practice Primary Group Chair & GP PharmacistHelen is a GP Pharmacist based in Malvern, South Worcestershire under the NHSE pilot scheme. Previously working as a GP Federation pharmacist for SW Healthcare where she was supporting lead pharmacist for all 32 practices in South Worcestershire CCG. Helen has experience in both hospital and community pharmacy. She attained Postgraduate Clinical Pharmacy Diploma at Keele University in 2007. She is an Independent Prescriber with specialist interest in palliative care and cancer care services. Helen is currently undertaking a MSc. in Advance Clinical Practice at Worcester University

Yinka Kuye – Education Supervisor CPPEYinka is a highly experienced and dynamic educator, supporting the development of professional teams and leaders, with a reputation for delivering world class training interventions. With extensive leadership experience and excellent interpersonal and social skills Yinka is constantly championing a culture of leadership at all levels.Currently working with the Centre of Pharmacy Postgraduate Education at Manchester University as a learning and development pharmacist and as an Education Supervisor on the NHS England clinical pharmacist in general practice programme. Yinka qualified as an Independent prescriber in 2007 with a specialism in Diabetes and is currently also working as a Diabetes clinical pharmacist in general practice.Yinka constantly looks for ways to stimulate personal growth and development; she is a qualified MBTI practitioner and is currently completing her PGCert in Clinical Education at the University of Leeds.

Jan MacDonald - Group Manager, MHRAJan MacDonald is a pharmacist by profession and is currently Group Manager for the Access, Information for Medicines & Standards Group in the Vigilance and Risk Management of Medicines Division at the MHRA. A key aspect of her role is safe packaging and quality information for healthcare professionals and patients. Jan has undertaken number of assessment and management roles within the agency. Prior to this she was principal pharmacist for quality assurance and quality control in the NHS.

Michael McGuire – Community Pharmacist & PCPA CPG CommitteeAs MD/Community Pharmacist at Marton Pharmacy in Middlesbrough, Michael has worked to transform the health and wellbeing of his community. He campaigned for, and subsequently worked with the Public Health Team, to bring Healthy Living Pharmacies to Tees. In 2009, Michael won the UK Community Pharmacist of the Year Award, and has since won national awards for Excellence in Training, Innovation and Excellence in Business, and an award from Mayor Ray Mallon for Services to the Community. Michael is an Honorary Lecturer at Durham University School of Pharmacy, and Chair of the Local Professional Network for Durham, Darlington, and Tees on behalf of NHS England. Michael has inspired audiences from the UK to Dubai, has mentored and coached Pharmacists, Nurses and GPs, and is currently delivering Leadership Training to Healthy Living Pharmacies and LPCs. Michael has recently been instrumental in devising and launching the NHS111 Community Pharmacy Referral Service.

Dr Anna Murphy - Consultant Respiratory Pharmacist & PCPA Respiratory ChairConsultant Respiratory Pharmacist, University Hospitals of Leicester NHS TrustAnna Murphy is a consultant respiratory pharmacist at University Hospitals of Leicester NHS Trust and honorary visiting professor at DeMontfort University. The clinical aspect of her post offers assessment, medicines optimisation, monitoring and advice to people with respiratory disease, working both in hospital and primary care.

Lelly Oboh – PCPA Clinical Specialist Group Chair & Consultant PharmacistLelly Oboh is a Consultant Pharmacist (care of older people) with over 20 years of experience in community settings within and outside the NHS. She has considerable experience working within general practice and integrated care teams to optimise medicines use as part of routine patient care.Her leadership role involves championing innovative pharmacy service developments, showcasing and driving uptake at local and national levels. She actively promotes best practice through facilitating effective partnerships between the NHS, Social care, statutory and professional bodies using a whole systems approach to drive change and equip wider workforce to deliver medicines optimisation. She is keen to develop extended roles for pharmacists within integrated care teams for frail older people in community and care home settings and has pioneered a model of “community clinical pharmacy teams”, which has been replicated by other NHS organisations. These models focus on active engagement with older people (carers), developing personalised solutions to improve adherence, better care co-ordination at handover points and improving medicines use in social care settings.

Helen Pinney – PCPA Practice Pharmacy Group Technician LeadHelen Pinney is the Lead Pharmacy Technician for twelve GP surgeries in West London. She oversees a team of pharmacy technicians and independent prescribing pharmacists who are responsible for the pharmaceutical care of community patients and twenty nursing homes in the area. She also frequently works as a locum in community pharmacy in order to stay in touch with the challenges faced in the community sector. She is passionate about expanding technicians’ roles to reflect their full potential and to complement the pharmacy profession as a whole, with an aim to achieving better and more patient centred care nationally

Carol Roberts - CEO for PrescQIPPCarol became the CEO for PrescQIPP in July 2016, an organisation that she founded in 2010 at the SHA as part of the QIPP Delivering Prescribing Efficiencies Workstream (PrescQIPP), which, has gone from strength to strength. PrescQIPP was hosted by the Eastern AHSN and has recently become a Community Interest Company (not for profit) providing support to over 95% of the country’s CCGs, Northern Ireland, Wales and Scotland Shared Services who now subscribe to the PrescQIPP Programme.She has over 15 years’ experience working in primary care, 11 years as a Head of Medicines Management in Essex and London, and then 3 years as the Pharmacy and Prescribing Lead for the East of England SHA and NHS Midlands and East.For the past eight years Carol has also been the Eastern Region Lead for the Home Oxygen Service (HOS) successfully re-procuring, implementing and managing the HOS regional contract achieving significant savings.In 2014 the Royal Pharmaceutical Society of Great Britain awarded Carol a fellowship.

Dr Sarah Rodgers - Principal Research Fellow at the University of NottinghamDr Sarah Rodgers is a Principal Research Fellow at the University of Nottingham. Her main research interest is prescribing and patient safety in primary care. Much of her work has focused on the investigation of the prevalence of medication errors in primary care and the evaluation of interventions aimed at reducing medication errors. In October 2012, Sarah was awarded a three year NIHR School for Primary Care Research Fellowship to work on the development of prescribing safety indicators and data extraction methods in UK general practice. During this time she worked with colleagues to secure funding from the Health Foundation for the rollout of a pharmacist-led, IT-based intervention (PINCER) to reduce medication error to 360 GP practices across the East Midlands.

She is currently Programme Manager for an NIHR Programme Grant award of almost £2.5 million (Avoiding patient harm through the application of prescribing safety indicators in English general practices - PRoTeCT) and is also involved in a number of other patient safety studies, including a Department of Health funded study to understand the nature and frequency of avoidable harm in primary care..

Ravi Sharma – PCPA Vice PresidentRavi is the Clinical Lead Pharmacist for the NHS England while holding several honouree and advisory positions. His key role has been focussed on the development of integrated/collaborative and innovative roles for pharmacists.

He has been responsible for developing new roles of pre-registration, junior and senior pharmacist roles and linking this work into local and national workforce development strategies. He has published several articles in reputable journals and is involved in several research projects around the impact of clinical pharmacy within primary care. Ravi continues to work as a clinician in a GP practice on a weekly basis. More recently, he has been sponsored by the NHS to undertake a Masters in Genomic Medicines at Imperial College London.

Sue Smith - Head of Education at PrescQIPPSue has been Head of Education at PrescQIPP since September 2016. Prior to that she was Head of Medicines Management for the two CCGs in Northamptonshire (Nene and Corby) since their inception in 2012, and the predecessor Northamptonshire PCT.

She started her career in community pharmacy before working as a practice pharmacist in the late 1990s and then as a Prescribing Adviser for Northamptonshire Heartlands PCT.

Sue was a trainer for the former National Prescribing Centre until its merger with NICE when she became a Medicines and Prescribing Associate.

Kate Spittle – PCPA Practice Pharmacy Group, All Wales LeadKate has worked in primary care since 2001 following experience in the pharmaceutical industry, community pharmacy and Cardiff university as a teacher practitioner. She qualified as a supplementary prescriber in 2004, completed the RCGP certificate in substance misuse in 2005 and became an independent prescriber in 2008. She completed the Diploma in Therapeutics at the medical school in Cardiff in 2014, winning the prize for the top academic performance.

She is directly employed by two GP surgeries, and as a practice based pharmacist undertakes medication reviews, shared care prescribing and deals with medication queries on a wide variety of topics. Her current focus is the management of co-morbidities and the challenges that this brings for patients and healthcare professionals! Kate is the acting director of the Diploma in Pharmacy Clinical Practice at Cardiff University and is module leader for the primary care module of the Diploma in Clinical Pharmacy. She has recently developed and led on an advanced practice module for primary care pharmacists to support the new cluster / neighbourhood roles in Wales.

Dr Graham Stretch – PCPA Care Homes Group Chair & GP PharmacistGraham is a General Practice Pharmacist and is now the director and prescribing lead of a pharmacy team supporting ten GP practices and 23 nursing homes around Ealing. Graham has overarching responsibility for safe prescribing for more than 75,000 NHS patients and 1,200 nursing home patients. Graham is also Senior for the 11 practices taking part in Ealing GP Federation’s NHSE Clinical Pharmacists in GP pilot and a Clinical Mentor for the CPPE GPPTP pathway.

Graham was awarded Clinical Pharmacist of the Year 2016 by the RPS for the delivery of a pharmacy team approach to managing nursing home patients. He was also given the title of Pharmacist of the year at the GP awards 2017.

Dr Karl Sylvester - Head of Joint Respiratory PhysiologyDr Sylvester is Head of Joint Respiratory Physiology at both Cambridge University Hospitals and Royal Papworth Hospital NHS Foundation Trusts, with over 20 years of experience in the performance and interpretation of respiratory physiology assessments. He is also honorary Chair of the Association for Respiratory Technology & Physiology. He co-Chairs the ARTP/BTS Joint Strategy Board meetings and presents on the BTS Short Course on Respiratory Physiology.

He has just taken up a role as Secretary for Group 9.1 at the European Respiratory Society, a group which represents respiratory and sleep scientists from across Europe. His specialist interest is the performance and interpretation of cardio-pulmonary exercise testing. He is faculty lead for the ARTP’s annual CPET course and faculty member for the UCLA/Cambridge CPET course. Dr Sylvester initially joined Cambridge University Hospitals after completing a PhD investigating respiratory complications in patients with sickle cell disease.

Helen Williams - Consultant Pharmacist & PCPA Cardiovascular ChairAs Consultant Pharmacist for CVD Disease, Helen is involved in a wide range of activities across South London to improve the care of patients with or at risk of cardiovascular disease (CVD). Helen chairs the local cardiovascular medicines working group which develops and supports the implementation of consensus evidence-based guidance for use across South London’s 12 clinical commissioning groups and 7 acute trusts, covering a population of 3.6million.

Helen is the clinical lead for CVD within two South London CCGs, Clinical Director for AF in the local Health Innovation Network, National Clinical Adviser to the collaborative AF programme of the 15 Academic Health Science Networks across England and also co-chair of the Pan-London stroke prevention in Atrial Fibrillation strategy group. In addition, Helen is an editorial board member for the British Journal of Cardiology and the Journal of Medicines Optimisation, has worked with NICE on the development of a number of cardiovascular guidelines and is a member of the NHS England Primary Care Cardiovascular Leadership Forum and the London Cardiovascular and Stroke Strategic Clinical Leadership Groups.

Andre Yeung - Pharmacy Consultant & PCPA CPG CommitteeAndre is a pharmacy consultant at Andre Yeung Ltd where he provides practical advice to Local Authority, NHS and Pharmaceutical Organisations about community pharmacy. He works for Newcastle City Council as a Senior Specialist Advisor within the Public Health Department, providing strategic input into the planning, commissioning and management of key services. Andre is also Chair of the NHS England Local Professional Network for Northumberland, Tyne & Wear (NTW).

Andre has previously worked as a Business Development Manager for Celesio and was responsible for designing, tendering, implementing and managing large service contracts in partnership with NHS organisations. He also worked as Area Manager for Co-Operative Pharmacy and an Independent Pharmacy chain and as part of two Local Pharmaceutical Committees, has represented the interests of hundreds of pharmacies for a number of years across the North East.
