FROM THE PRESIDENT

John Whyte, MD, PhDRecipient of the 2018 Physician Leadership Award from the Einstein Society

This year, the Founda�on con�nued in its efforts to iden�fy new cre-a�ve funding priori�es to advance the field of PM&R. For many years we have invested produc�vely in the development of early career physiatric researchers and provided pilot funding to established inves-�gators for new areas of study as a way to elevate the standing of our field within medical educa�on as well as to contribute new knowledge to our field. Given the broad spectrum of disabling condi�ons and focus areas treated by physiatry, there is no one line of inves�ga�on that will provide all the answers for our field. Instead, we need a pipe-line of inves�gators across the breadth of the field to address the issues faced by each subspecialty area. Our Awards and Grants program has been designed to address that vital need.

More recently we have explored other ways Founda�on investments can advance our specialty, in the view that there are many different kinds of leaders and leadership that could benefit physiatry. As part of this explora�on we have undertaken several surveys of our donors as well as of physiatrists who are not currently donors to the Founda�on. In these surveys, we asked for importance ra�ngs of a number of possible research and non-research topics from both donors and nondonors. In addi�on, we provided a list of addi�onal topics specifically to nondonors to learn which, if any, would encourage new support of the Founda�on.

Our current donors expressed high levels of enthusiasm for a number of research topics. Those not currently contribu�ng to the Founda�on, not surprisingly, expressed lower levels of interest in suppor�ng research topics across the board. Both groups expressed the greatest research interest in topics such as compara�ve effec�veness research and research involving pa�ent cen-tered outcomes, which involve real world treatments of importance to pa�ents, in contrast to early mechanis�c research that may lead to enhance treatments in the future. Among the new topics presented to current nondonors, the two that generated the greatest enthusiasm were “Funding analysis of large data sets to examine the impact of physiatric involvement in coordi-nated acute and post-acute care programs on quality, cost, and clinical outcomes” and “Support-ing analysis of other large data sets to examine the impact of physiatric care on pa�ent out-comes.” There’s probably li�le disagreement about the importance of these topics to our field. So it’s worth reflec�ng on what makes them challenging to address, par�cularly as we consider the most strategic pilot investments from the Founda�on’s modest resources.‘

To study the compara�ve effec�veness of any treatment or medical specialty, one needs to have a well-characterized pa�ent popula�on, two or more clearly defined treatments (treatment A/B or physiatrist/non-physiatrist), and an appropriate and sensi�ve outcome measure. In PM&R all 3 of these axes are challenging. For many medical condi�ons, the severity of the disease or dis-ability is the strongest predictor of outcome irrespec�ve of treatment. Consequently comparing outcomes of pa�ents treated by physiatrists and non-physiatrists only makes sense if we treat very similar pa�ents. Moreover, some important pa�ent factors (e.g., family support) are chal-lenging to measure, so demonstra�ng “comparability” of two groups of pa�ents is daun�ng.

Next we come to the treatment. Do we really want to compare “physiatrists” to “non-physiatrists”? That’s a good idea if we think that most physiatrists can deliver be�er outcomes than most non-physiatrists for most clinical condi�ons, but is that a realis�c hypothesis? Or might the rela�ve superiority of physiatrists depend on the complexity of the condi�on being treated. If the par�cular benefit of physiatric care applies only to certain subgroups of pa�ents, then such a generic study risks showing no superiority overall for physiatric care.

Finally there is the challenge of outcome measurement. Given the heterogeneity of the pa�ents we treat, there are few outcome measures that would be relevant to the whole popula�on. We could study things like FIM or length of rehabilita�on hospital stay, which would be relevant to large segments of our pa�ent popula�on, but which are affected by many other factors. Once we focus on more specific clinical measures such as gait speed, or pain level, then we must con-sider much more focused pa�ent groups as well.

Funding one large study to solve a problem is appealing, but considering the complexi�es of physiatric care, it is unlikely that a single large study will ever address the ques�ons as they are phrased above. More likely, it will take a sustained program of research to understand the con-tribu�on of physiatric care to specific problems experienced by specific pa�ent popula�ons, as measured by specific outcome measures. Accordingly, we must answer many smaller ques�ons over �me to build the big picture of the overall impact of physiatric care in our healthcare sys-tem. We mustn’t let the complexity of the problem overwhelm us into a sense of helplessness. Rather, we’ll need to adopt the approach required to solve most highly complex problems: The Founda�on for PM&R will con�nue to chip away at the problem over �me with judicious invest-ments that help develop experts that are up to the task!

ADVANCING PHYSIATRIC SCIENCEProviding pilot funding and career development grants are the core of the Founda�on’s mission. In 2018, the Founda�on granted over $160,000 in pilot funding to inves�gators who could demonstrate they have the resources and exper�se available to com-plete innova�ve, impac�ul research and likely to lead to addi�onal studies.

PROOF OF THE PUDDING DEBATESupported by a gi� from Dr. C. George Kevorkian, the Proof of the Pudding Debate is a lively and informa�ve evening dessert and discussion session on a controversial topic in physiatric medicine held each year during the AAPM&R Annual Assembly. The 2018 program featured Chris�na Greiss, DO and Sco� Laker, MD (pictured below) on the topic of “Football Par�cipa�on and Chronic Trauma�c Encephalopathy.” The debate was based on a Point/Counterpoint column from the PMR Journal (PMR 2018; 10: 655-660).

The Founda�on provides valuable career development grants for many rehabilita�on medicine scien�sts early in their career. Many of these individuals have gone on to have very successful research careers. I have especially sup-ported the Justus Lehmann Research Grant in honor of Dr. Lehmann’s vision for research. Dr. Lehmann was a mentor of mine in my early career and made a significant impact on my career development. He recognized the importance of research to establish the future of rehabilita�on medi-cine. Given the changes in our health care system, research is even more important today, so please contribute to the Founda�on. The Founda�on needs your support to elevate physiatry to serve others.

OUR MISSION:ELEVATING PHYSIATRY TO SERVE OTHERS

FOUNDATION SPEAKS OUT ON CANCER REHABOn March 12-13, 2018, Dr. Lynn Gerber represented the Founda� on and the AAPM&R at the American College of Sports Medicine (ACSM) roundtable on Exercise in Cancer Rehabilita� on. The mee� ng was convened to review and update the ACSM’s guidelines on exercise in cancer survivors, which are used by 70% of all cancer programs in the US. The roundtable addressed ques� ons about the role of exercise in reducing mortality and promo� ng func� on for cancer survivors. Dr. Gerber said, “It is crucial that the Founda� on have a seat at the table at events like this where important recommenda� ons for rehabilita� on treatments are discussed and made. Our par� cipa� on also raises the profi le of the fi eld by having our name on the publica� ons and guidelines, which will be coming out in 2019.”

PRESIDENTS FUND UNDERWRITES PAL EVENTThe Founda� on is commi� ed to suppor� ng leadership development to prepare the next gen-era� on of physiatric leaders for important roles in PM&R and beyond. For the fourth year in a row, the Founda� on’s President Fund supported a mentoring event for par� cipants in the AAP’s Program for Academic Leadership (PAL) during the AAP Annual Mee� ng. This produces a relaxed opportunity for candid and suppor� ve mentoring. The emphasis this year was on academic departmental opera� ons, but the discussion was unstructured and focused on PAL a� endees’ ques� ons. Many of the past presidents who par� cipated have a connec� on to the Founda� on, including board members Bruce Gans, Kurt Hoppe, John Melvin, John Whyte and Friends member Gary Clark.

AMERICANS WITH DISABILITIES FUND: RX FOR EXERCISERecognizing that exercise is even more important to maintain health and improve func� on in the popula� on with disabili� es, the Founda� on for PM&R has partnered with a number of other organiza� ons including the Na� onal Center for Health, Physical Ac� vity and Disability (NCHPAD) and the ACSM’s Exercise is Medicine program to provide resources for physiatrists and their pa� ents. In 2018, we introduced a sec� on dedicated to the needs of individuals with spinal cord injury.
