Section 1 – Introduction
This directory provides a list of Moa Health Plan’s network providers. To get detailed
information about your health care coverage, please see your Evidence of Coverage (EOC).
Moda Health HMO plans:
You will have to choose one of our network providers listed in this directory to be your Primary
Care Provider (PCP). The term “PCP” will be used throughout this directory. A list of network
PCPs can be found in the “Primary Care Physicians & Provider” section of this directory. If you
have questions about selecting a PCP or if you would like to change your PCP selection, call our
Member Services department at 1-877-299-9062 from 7 a.m. to 8 p.m. Pacific Time, seven days
a week (TTY users call 711), or you can log into your myModa account to update your PCP
online.
Generally, you must get your health care services from your PCP. If your PCP determines that
specialty care is medically necessary, they will provide you with a referral to a specialist.
Additionally, some medical services are covered only if your provider gets prior authorization
(approval in advance) from our plan. Referrals and prior authorizations from your PCP are not
required for emergency care or urgently needed care. There are some other kinds of care you can
get without approval in advance from your PCP. Covered services that require PCP referral or
prior authorization are listed in Chapter 4 of your Evidence of Coverage booklet.
The network providers listed in this directory have agreed to provide you with your health care
services. You may go to any of our network providers listed in this directory; however, some
services may require a referral. If you have been going to one network provider, you are not
required to continue to go to that same provider. In some cases, you may get covered services
from non-network providers.
You must use network providers except in emergency or urgent care situations or for out-of-area
renal dialysis or other services. If you obtain routine care from out-of-network providers, neither
Medicare nor Moda Health will be responsible for the costs.
There may also be times when you get a bill from a provider for the full cost of medical care you
have received. In many cases, you should send this bill to us instead of paying it. We will look at
the bill and decide whether the services should be covered. If we decide they should be covered,
we will pay the provider directly.

How to get covered services when you have an emergency
What is a “medical emergency” and what should you do if you have one?
A “medical emergency” is when you, or any other prudent layperson with an average
knowledge of health and medicine, believe that you have medical symptoms that require
immediate medical attention to prevent loss of life, loss of a limb, or loss of function of a limb.
The medical symptoms may be an illness, injury, severe pain, or a medical condition that is
quickly getting worse.
If you have a medical emergency:
Get help as quickly as possible. Call 911 for help or go to the nearest emergency room
or hospital. Call for an ambulance if you need it. You do not need to get approval or a
referral first from your PCP.
As soon as possible, make sure that our plan has been told about your emergency. We need to follow up on your emergency care. You or someone else should call to tell us
about your emergency care, usually within 48 hours. Call Moda Health Healthcare
Services at 1-800-592-8283 or fax the information to 1-855-637-2666. Moda Health
Healthcare Services hours of operation are from 7 a.m. to 4:30 p.m. Pacific Time,
Monday through Friday. (TTY users call 711).
What is covered if you have a medical emergency?
You may get covered emergency medical care whenever you need it, anywhere worldwide. Our
plan covers ambulance services in situations where getting to the emergency room in any other
way could endanger your health. For more information, refer to your Evidence of Coverage
booklet.
If you have an emergency, we will talk with the doctors who are giving you emergency care to
help manage and follow up on your care. The doctors who are giving you emergency care will
decide when your condition is stable and the medical emergency is over.
After the emergency is over you are entitled to follow-up care to be sure your condition
continues to be stable. Your follow-up care will be covered by our plan. If your emergency care
is provided by out-of-network providers, we will try to arrange for network providers to take
over your care as soon as your medical condition and the circumstances allow.
Please note: While you can get your care from an out-of-network provider, the provider must be
eligible to participate in Medicare. Except for emergency care, we cannot pay a provider who is
not eligible to participate in Medicare. If the provider is not eligible to participate in Medicare,
you will be responsible for the full cost of the services you receive.

Prior authorization is approval in advance to get services. Some in-network medical services are
covered only if your provider gets prior authorization from our plan. You do not need prior
authorization to obtain emergency services. Covered services that need prior authorization are
listed in Chapter 4 of your Evidence of Coverage booklet.
