Do I need an email address to fill out the form?Yes. A parent or guardian email is required. The school district will use your email address to deliver important news and information to you during the school year.If you do not have an email address, you can sign up for a free account through Gmail, Yahoo and other providers.

What is the form for?This form is completed every year and allows parents and guardians to say yes or no for permission for their children for the following items:

● Media Release● Technology and Internet Use Agreement● 11 Device Agreement

You will find detailed information for each of these items on the form. Every family is required to fill out the form for all of their children in the school district.

I don’t know my child’s Student ID #. What should I do?You can leave that box blank on the form. Student ID numbers are helpful but are not required. This information is found on your child’s schedule or within ParentVue. Older children should be able to give you this information.

I have more than one child in the school district. Do I have to complete a new form for each child?No. You will only complete one form for all of your children in the district. Just click on on page 1 of the form to add each additional child.

I need help filling out the form. How can I get help?You can call the school principal, assistant principal, dean of students or secretary at the school building. They are happy to help you!

Does the form come in other languages?Yes. Click on the drop down menu on the top right of the form to choose another language.

What happens if I don’t fill out the form?Your child will not be issued a school district Chromebook computer to use at school or at home.

No sé el número de identificación de estudiante de mi hijo. ¿Qué tengo que hacer?Puede dejar esa casilla en blanco en el formulario. Los números de identificación de los estudiantes son útiles pero no son obligatorios. Esta información se encuentra en el horario de su hijo o en ParentVue. Los niños mayores deberían poder darle esta información.

Tengo más de un hijo en el distrito escolar.¿Tengo que completar un formulario nuevo para cada niño?No. Solo completará un formulario para todos sus hijos en el distrito. Simplemente haga clic en
