    S U N T A N N I N G AND SUN PROTECTION: A REVIEW OFTHE PSYCHOLOGICAL LITERATURE

    Ab stract--Ex cessiv e sun exposure has been l inked to skin cancer an d to p rema ture aging, drying andwrinkling of the skin, pred om inantly among Caucasians. This review examines the psychological l i teratureon suntanning and sun protec tion behaviours am ong Caucasians. The research is examined in relationto: methods o f study; att i tud e and n orm ative beliefs towa rds suntanning and sun protection; differencesin suntanning and sun protectio n knowledge and beh aviour as a function of age and gender; and, attemp tsto change sun related behaviours. A consistent finding across studies is that many people show a highlevel of knowledge o f the dangers of excessive sun exposure a nd the need fo r sun protection, however,this know ledge often does no t transfer into behaviour, with ma ny people, particu larly adolescents, st il ldesiring and actively seeking a suntan. The implications of these findings for intervention studies arediscussed.Key words--review, suntanning, sun protection, health promotion

T h e r e f o r e , d e c r e a s i n g t h e a m o u n t o fs u n e x p o s u r e i s th e p r i m a r y o b j e c t i v e o f b e h a v i o u r a lr e s e a r c h i n s k i n c a n c e r p r e v e n t i o n . S u c h p r e v e n t i o nh a s g e n e r a l l y t a k e n t h e f o r m o f i n c r e a s i n g p u b l i ca w a r e n e s s a n d k n o w l e d g e o f t h e d a n g e r s o f ex c e ss iv es u n l i g h t e x p o s u r e a n d e d u c a t i n g t h e p u b l i c a s t ot h e b e s t w a y s t o a v o i d o v e r - e x p o s u r e . Howeve r , i t ha s a l so been found tha t l eve l s o fa t t i t u d e t o s u n t a n n i n g [ 9 , 1 0 ] a n d a c t u a l s u n r e l a t e db e h a v i o u r [1 1, 12 ] a r e n o t a l w a y s c o n g r u e n t w i t h t h ek n o w l e d g e o f r i s k s p e o p l e p o s s e s s.

    METHODS OF STUDYR e s e a r c h h a s m o s t l y u s e d s u r v e y / q u e s t i o n n a i r ed e s i g n s a n d h a s e x a m i n e d a n u m b e r o f v a r ia b l e s

    i n c l u d i n g p e o p l e ' s l e v e l o f k n o w l e d g e o f t h e d a n g e r so f o v e r - e x p o s u r e [ 9, 1 3 ] , t h e p e r c e i v e d b e n e f i t s o fh a v i n g a s u n t a n [ 9, 1 0 ], a n d u s u a l b e h a v i o u r a n d l e v e lo f s u n p r o t e c t i o n w h e n o u t d o o r s [ 1 2] . H o w e v e r , t h e r e h a v e b e e n a v a r i e t y o f q u a s i -e x p e r i m e n t a l , i n t e r v e n t i o n d e s i g n s a t t e m p t i n g t oi m p r o v e s u n p r o t e c t i o n k n o w l e d g e , a t t i t u d e s a n db e h a v i o u r . CLARKEpersonal risk and motivation to change behaviourneed to be assessed before implementat ion of anintervention and if perceptions of risk and m otivationto change are low, other strategies need to be incor-porated into the in terven tion to help personalize therisk and increase motivation to change.

    Interventions directed at specific high risk groupsmay be more effective at reducing sun exposure thanthe more general public education campaigns. Forexample, the Telecom outdoor worker 'Cover your-self against skin cancer' campaign and the modellingof appropriate sun protection behaviour by lifeguardsat two U.S. pools both successfully increased skinprotection behaviour in the environments at whichthe campaigns were aimed. Specific group focusedinterventions are capable of providing situationspecific information, can teach skills necessary toincrease self-efficacy [66] to deal with peer and familypressure to get a suntan and can model appropriatesun protection behaviour that is relevant to themembers of the group in their immediate environ-ment, which the more general campaigns are unableto do.

    General community-based campaigns are stillrequired. However, their emphasis may change.Rather than being seen as a major source ofinformation on sun protection they could take asupport role; acting as reminders, complement ing andenhancing the focused interventions.

    Skills training has been shown to be important inreducing other problem behaviours such as smoking[e.g. 67] and unsafe sex [e.g. 68] . To date therehas been no published research assessing what skillspeople need to successfully engage in sun protect ion.Nor has there been any research evaluating people'sconfidence in their ability to increase their sun protec-tive behaviour and resist pressure from peers andfamily to expose themselves to the sun. Otherresearch necessary for developing successful interven-tions includes an identification of the stages of change[69] from seeking a suntan to long term maintenanceof sun protection behaviours, the rates of progressthrough the stages, an unde rsta ndin g of the reasonsfor relapse or discon tinua nce of sun protection,means of minimizing relapse and means of changingthe behaviour of those who do relapse.

    CONCLUSIONSAt present there appears to be a belief in public

    education campaigns and intervention studies thatraising the public's level of knowledge of the dangersof over-exposure to the sun will result in more peopleusing appropriate protection when they are inthe sun. However, this belief appears to have onlybeen partially supported by people's behaviour withmany still valuing a suntan despite their high levelof knowledge of the dangers of over exposure.Knowledge gains must precede any attempts to alterbehaviour, but knowledge alone will not necessarily

    result in changes to behaviour [70]. Similarly,Atk in [71] reports that health education programswhich rely solely on increasing the public's level ofknowledge are likely to have only limited success inchanging behaviour. Knowledge gains should be seenas a first step toward behaviour change, rather thanas the main tool to alter behaviou r [70, 71]. Theresearch discussed here indicates that the majori ty ofthe public have a high level of knowledge of thedangers of over-exposure to the sun, suggesting thatnow is an appropriate time to begin looking at waysto directly influence people's behaviour.
