Filtek Z350 Universal Restorative is the result of a revolutionary combination of two fillers - nanoparticles and nanoclusters. A nanometer is just 1/1000 of a micron. These nanosized filler particles abrade below the visible spectrum of light, allowing the polish retention typical of a microfill.
Further, both nanoparticles and nanoclusters fillers are combined directly with our proprietary, reduced-shrinkage resin to produce a material that duplicates the physical properties of a hybrid composite.

The Polish of a MicrofillThe initial polish of many composites is quite good. Wear, toothbrush abrasion, and other factors reduce this initial gloss.
Microfills are known for excellent polish retention, but, until now, universal composites have tended to lose much of their initial shine. Filtek™ Z350 Universal Restorative addresses this using a novel approach.
The nanoparticles of Filtek Z350 restorative are added both individually and in clusters, termed nanoclusters.
These nanoclusters are lightly sintered so that they can break apart during the wear process. By doing so, the optical effect is the loss of a nanometer-sized particle rather than the loss of a micron (e.g. hybrid composite) sized particle, which is the reason for the long term polish retention.
The nanoparticles of FiltekZ350 Restorative are added both individually and in clusters, termed nanoclusters.

What does this mean to a clinical practice ?It means that Filtek Z350 restorative is truly a universal composite, with the esthetics required for anterior restorations and the strength needed for posterior restorations.
Diastema Closure - Filtek Z350 Single Shade
20 yr old female presented with midline diastema .A high frenal attachment was diagnosed. Treatment involved surgical correction of frenal attachment, followed by correction of diastema, using Z350 composite and Adper Prompt adhesive
Primary incision for correction of frenal attachment.
Sutures should be placed carefully to prevent the incision from reopening and to allow healing.
4 weeks after surgery good healing is seen, and case is ready for closure of diastema.
1. A mock restoration is done after shade matching to evaluate shade match and expected contours.
Z 350 shade A3 is used as the restorative material .Since the diastema is small a single shade will suffice .It is important to minimize excess and have as smooth a finish as possible during placement. This helps in ensuring good surface texture and decreases time required for polishing.
Pr imary f in ish ing and pol ish ing completed. Final finishing and polishing is delayed for 24 hrs. This allows the tooth to rehydrate, and evaluation of colour and contours is much superior at the second appointment . (Gross finishing with 32 fluted carbide burs (Kerr Corp) ,Soflex discs (3M ESPE) and Astropol finishing kit ( Ivoclar Vivadent)
Restoration after final polishing. One can clearly appretiate the superion polish afforded by Z 350 .Such finish ensures long lasting esthetics and eliminates the need for repeated polishing.(Final polish using Astropol ,Soflex and Jiffy brushes)
The patient also had a small class III on the distal of 21 which has now been restored.

Dr. Arvind Shenoy completed his BDS from Mumbai University in 1988 and MDS from Mumbai University in 1991, both from Govt Dental College and Mumbai Hospital. His specialisation was in Conservative Dentistry and Endodontics. Class II Composite Restoration using Filtek Z350
Cavity Preperation with removal of caries. Old amalgam and secondary caries removed.
Band placed Disposable Omni Matrix (Ultradent Products) and Wedge (Hawe Neos). Tight contact area developed by positive wedging using wooden anatomic wedge. Ultra thin band (preferably 0.001”) contoured to correct contact location.
A 18 year old male patient. Mesial lesion in relation to 26.Mesial and distal amalgam restorations with marginal deterioration. Discoloured Palatal groove.
Planned Restoration: MO and Distal pit with FiltekZ350 (Nanofilled Dental Restorative) Shade A 3.5 and A 3 E (Filtek supreme) and Palatal groove with Filtek Flow Shade A3.
After etching with 35% Phosphoric acid and bonding with Adper single bond, Filtek flow (Shade A3) was placed as a liner. This helps in close adaptation of initial increment of resin, as well as reduces incidence of post op sensitivity. In deep cavities Vitrebond Liner (Resin modified glass ionomer) is preferred.
Composite built up in increments ,contouring done using Duckhead instrument (Hu Friedy) for achieving anatomic contours. Shades used A3.5 (Filtek Z350)and A3 Enamel (Filtek Supreme). No transluscent shades were used in this case. Palatal groove restored with Filtek Flow Shade A 3.
Band removed and gross excess removed using No 12 BP Blade. Occlusion verified.
Contact built up using Optra Contact instrument ( Ivoclar Vivadent)
Gross finishing completed using Finishing Carbide burs and Diamond Points (Brassler ) ,Polishing cups (Diatech), Soflex finishing discs (3MESPE).
Restoration after polishing with Soflex Brushes (3M ESPE) and Diamond polishing Paste (Ultradent ).
Final restoration in harmony with adjacent tissues. Contact size and shape duplicates original anatomy as s e e n i n p r e o p i m a g e . Z 350 contains nanoclusters with nanofillers which gives the restoration superior strength and excellent polishability.
Filtek Z350 in Decidous Teeth
An intra oral radiograph was taken which showed that the carious lesion was near the pulp tissue. It was decided to remove the decay, do an indirect pulp capping and fill the tooth with Z350 composite.
Before starting with the removal of decay the shade of the tooth was matched with the Vitapan shade guide, which was B2.
Caries was removed and finally checked with caries detection dye.
Paleodent Matrix with a wooden Hawe neos wedge was applied.
Contouring an Occlusal carving was done with P1 instrument. The top layer of Filtek Z350 B2 was applied and cured.
Final polishing was done using the Sof Lex system (3M ESPE).
A tight contact, good occlusal morphology with excellent esthetics was obtained. Filtek Z350 contains nano clusters with silinated and etched nano fillers, which give the restoration superior strength, esthetics and excellent polishability.
Child age 7 years came to the dental center with a complaint of pain. On examination it was observed that the child had a carious lesion in tooth no.74 with food impaction.
Indirect pulp capping was done with Vitrebond resin modified glass ionomer liner.
Pushing the composite towards the matrix with a suitable instrument and curing established a tight proximal contact.
Matrix and wedge were removed and the high points checked using a thin articulating paper and removed.

Patient came with a complaint of visible black fillings done some time back. She also complained of sensitivity to hot and cold. She wanted something more cosmetic in the mouth. On examination it was observed that the patient had multiple amalgam fillings in the molars. There were dark areas seen around the fillings, which suggested that either there was secondary decay around the fillings or discoloration of enamel had taken place due to the amalgam.
It was decided to remove the amalgam fillings and look into the tooth. When the amalgam fillings were removed it was found that there was secondary decay below the fillings.
This decay was removed and the teeth were checked with caries detection dye
Any further decay was removed where the stains were present. There was a substantial loss of tooth structure present. It was decided to fill the teeth with Filtek Z350 because of the high strength of the material, which was needed in this type of a cavity.
Also, FILTEK Z350 could give the best esthetics due to Nano-technology.The shade of the tooth was recorded before the etching, but after the removal of amalgam filling and the secondary caries. The shade is selected using the Vitapan shade guide. Pulp was protected using vitrebond glass ionomer liner. and the fluoride leaching effect was utilized.
After etching and bonding, the selected shade of Filtek Z350 composite was used.
The build-up was done in increments using Filtek Z350 shade A3 and each increment cured for 20secs.
Filtek Z350 Composite Of 21st Century
The finished restoration after carving and polishing gave excellent results (Fg.14, 15). The strength and polish ability of FILTEK Z350 is much superior to other composites due to the sialination and bonding of the nano filler particles

Over the opaque shade Filtek Z350 A3.5 (3M ESPE) was used as the body of the restoration.A thin layer of Filtek Supreme YT - yellow transcluscent (3M ESPE) was applied and cured.

After application of Adper Prompt Self-etch Adhesive (3M ESPE) and curing for 10 secs, the first layer of Z350 OA3 - Opaque A3 (3M ESPE) was applied in the cervical and mesial and distal cavities.
Calcium Hydroxide applied only to the exposed area and a layer of Vitrebond liner (3M ESPE) is applied to the entire pulpal surface.

An independent in vitro study compared the bond strength of Adper Prompt L-Pop self-etch adhesive to Clearfil SE Bond adhesive on prepared and unprepared enamel. Adper Prompt L-Pop adhesive yielded a higher microtensile bond to prepared enamel than Clearfil SE Bond adhesive and an equivalent bond to unprepared enamel—proving its ability to provide an aggressive etch for high bond strengths.

Treating hypersensitive root surfaces
Fluid movement in dentinal tubules is the generally accepted cause of sensitivity. This movement can be stimulated by drying, temperature shifts, as well as by compounds such as sugar (Brannstrom 1986). Studies indicate that hypersensitive root surfaces can have 8 times as many open tubules as non-sensitive dentin.
If open tubules - which allow fluid movement - are a cause of sensitivity, it makes sense that treatments that occlude the tubules should be effective in preventing or treating sensitivity.
