The screen and well casing were welded together and lowered into the well shaft, section by section. This was done with the drilling rig. After the screen and casing were in place we checked to see if it was plumb then started to backfill. Plumb measurement taken did not deviate not more than 118 inch.

A. Volume Used: 4.4 cubic yards
B. Composition: Water/cement ratio 1: 1 with approximately 2 percent of bentonite.
C. Method of Installation: A 112" pvc pipe was lowered to the grout section. Grout mix was then pumped and slowly raised to 20 foot section. It was then left to be cured for two days before another 20 foot section was grouted.

Test was done by a deep turbine well rated at 8 H.P. We pumped at a rate of 100 gallons per minute.

Soil Type - Kealia Silt Loam - This soil is poorly drair.ed and has a high content of salt. The surface layer is dark reddish brown silt loam about 3 inches thick. Below this are stratified layers of silt loam and fine sandy loam. The subsurface layers are dark reddish brown to dark reddish gray in the upper pan and dark grayish brown to black near the zone of the water table.

The well construction/pump installation permit shall be for construction, testing, and installation of a 200 gpm capacity, or less, pum,P in the well, as determined by the pumping test results. The applicant shall subInlt to the Commission the test results and proposed permanent pump information, based on the test, for approval by the Chai..rperson.
The proposed use(s) shall not adversely affect existing or future legal uses of water in the area,includin~ any surface water or established instream flow standards. This permit or the authonzation to construct and pump water from a well shall not constitute a determination of correlative water rights. The permittee is notified and by this provision understands that the quantity of water taken from the well could be reduced by the Commission in the future. This permit is not a commitment that the pump capacity permitted here or even some lesser amount is guaranteed in the future.
The applicant shall provide and maintain an approved meter or other appropriate device or means for measuring and reporting total water usage. Water usage shall be measured on a monthly basis and reported to the Commission.
The following shall be submitted to the Commission within thirty (30) days after completion of work:

Action Requested: Permission to construct, test, and install a 180 gallons per minute (gpm) pump in Maalaea Power Plant M-17 Well (Well No. 4829-04) for industrial use. The proposed amount of use is 200,000 gallons per day (0.200 mgd).

Agency Review: The application has been sent to the Maui Department of Water Supply, the State Historic Preservation Division, the Office of Hawaiian Mairs, and to the State Departments of Health and Hawaiian Home Lands for review. There have been no objections to the project.
Analysis: The well is expected to develop brackish to salt water. No adverse effects are expected.
Water Availability: The well will be located in the alluvial aquifer of the Central Sector, Kahului Aquifer System of Maui. The alluvial aquifer is a separate aquifer from the basal aquifer. Since the well will develop water from the alluvial aquifer, it will have no adverse effects on the basal aquifer.
RECOMMENDATION:
That the Commission on Water Resource Management (Commission) approve the issuance of a well construction/pump installation permit for the Maalaea Power Plant M-17 Well, subject to the conditions listed below.

Historic Preservation Review of a W Construction and Pump Installation Permit for Maui Electric Company Well No. 4829-04 Waikapu, Wailuku District, Maui TMK: 3-8-05: 25
Maui Electric Company proposes to construct. a new well and pump within the Ma'alaea power generating plant. The well site is adjacent to the proposed power plant expansion area.
An archaeological and paleoenvironmental study of the proposed power plant expansion area has been completed by the applicant (Archaeological Inventory Survey and Paleoenvironmental Investigations at Ma'alaea Power Plant, Maui Electric Co., Ltd., Waikapu Ahupua ' a, Ma ' alaea, Maui, J. S . Athens, J . V . Ward, M. Tomonari-Tuggle 1994) . A draft of the report has been reviewed by our office and the work was found to be acceptable.
No evidence of subsurface historic sites was identified during the inventory survey. The surface area at the proposed well site and in the immediate vicinity of the well has been previously impacted by the existing power plant construction.
Based on the present land use and on subsurface findings of the archaeological survey, it appears unlikely that historic sites will be impacted by the proposed well. We believe the construction activities for this project will have "no effect" on historic sites.

Briefly describe the proposed work:
The well bore will be advanced with the appropriate drilling method to a total depth of at least 130 feet. The well will be screened over the bottom 30 feet of the well bore. The well will be developed and tested at a withdrawal rate of at least 180 gallons per minute (gpm). Solid stainless steel "tasing and cement grout will be extended to the top of the gravel pack. Water samples will be collected during the well test.

The well bore will be advanced with the appropriate drilling method to a total depth of at least 130 feet. The well will be screened over the bottom 30 feet of the well bore. The well will be developed and tested at a withdrawal rate of at least 180 gallons per minute (gpm). Solid stainless steel'iasing and cement grout will be extended to the top of the gravel pack. Water samples will be collected during the well test.

Units 17, 18, and 19 will be a 58 - MW dual-trained combined-cycle combustion electric generating power plant. A list of permits is attached.

Note: Signing below indicates that the applicant understands that, if the permit requested is granted by the Commission on Water Resource Management, the proposed work is to be completed within two (2) years of the approval date. In addition, the contractor shall submit to the Commission a well completion report, well abandonment report, or both, within 30 days after the completion date of the permitted work. The applicant also understands that monthly water use data shall be submitted to the Commission. The applicant further understands that approval of the proposed permit shall not constitute a determination of correlative water rights and shall not guarantee the pump capacity or future use up to the permitted pump capacity.

Briefly describe the proposed work:
The well bore will be advanced with the appropriate drilling method to a total depth of at least 130 feet. The well will be screened over the bottom 30 feet of the well bore. The well will be developed and tested at a withdrawal rate of at least 180 gallons per minute (gpm). Solid stainless steel '\:asing and cement grout will be extended to the top of the gravel pack. Water samples will be collected during the well test.
