We look forward with greatanticipation to another spirit-filled, knowledge building, and exciting YPD AnnualConvention. We will culminate this year with various workshops, leadership training,mission outreach, the election of Conference Officers for the 2012 – 2013 year, and aspecial Youth Night Worship Service.

REGISTRATION:The registration fee for this year’s Annual Convention will be $75.00 per person (youth and any adultsattending). Registration will include snack on Friday, 2 meals & snack on Saturday, T-shirt, conventionmaterials, and YPD Connectional Newsletter. The deadline for registration payment is August 25, 2012. Wewould prefer no on-site registration (unless it is has been previously confirmed). The registration form isincluded in this package. Please complete a form for all attendees. PERMISSION FORMS:Registration Forms, Permission to Travel, Photo Release, Code of Ethics, and Medical Authorization formsmust be complete for all YPDer’s 26 and under. Medical Authorization forms are also required for allAdults. These forms must be submitted with the registration forms by August 25, 2012, and all LocalDirectors must have a copy with them at all times during the Convention.
CREDENTIALS:Please complete the credential form included in this package and make sure all eligible YPD members bringtheir own properly signed credential form. They will be certified by the credential committee. Credentialforms must be completed prior to arrival IN BLUE INK. NO EXCEPTIONS. Note: Only distribute andcomplete credential forms to those YPDers who are at least 13 years old by August 9, 2012.

Midwest Conference YPD Code of EthicsEVENT: 2012 Annual Conference, September 7-9, 2012.
Name: ________________________________________ Age:_____1. The participants MUST attend all sessions, so as to benefit from their attendance, unless excused by the Conference YPD Director or
a member of the Conference Triumvirate.
2. Participants will abide by rules required by the Director’s Council (Conference YPD Director, Conference Triumvirate and Local YPDDirectors).
3. Participants become the responsibility of and under the direct supervision of the Director’s Council upon their arrival at the point ofdeparture of any activity or upon arrival at the place of the planned event. Disrespect of any Director or Adult Advisor will not betolerated. Violators will be referred to the appropriate Director for disciplinary action or possible removal from the planned event.
4. Each Director will be responsible for the activities of the participants in their charge.
5. Any infraction of rules, unbecoming conduct or unauthorized acts will result in the offending participant being brought before theDirector’s Council or their designated representative.
6. Possessing, dispensing or using cigarettes, tobacco of any kind, alcohol, barbiturates, tranquilizers, hallucinogenic drugs or marijuana isstrictly PROHIBITED. If proven that any participant has indulged in the above misconduct at any time during activities, theparticipant will be suspended from all activities.
7. Malicious mischief resulting in damage to property or the discomfort of participant will not be tolerated, and may result in beingbanned from participating in activities above the local church level.
8. Unauthorized absence from any planned meetings or activities will result in referral to the Director’s Council for appropriate action.
9. All property belonging to others (hotel, church, etc.) must be respected.10. THE ABOVE RULES APPLY TO EVERYONE.

SECTION C - YPD DIRECTOR LEADERSHIP AWARD-YPD DIRECTOR ON ANY LEVELThe Executive Board (any level) will prepare nomination letter (not to exceed one typewrittenpage) along with a color 3x5 photo explaining reason for nomination along withactivities/events. Also include leadership qualities exercised/exhibited. YPD First Vice-President (any level) shall sign letter.**Each Local YPD is to secure a minimum of 25 $15.00 sponsors for their YPD Director(any level). ($375)
SECTION D - CONFERENCE YPD DIRECTOR AWARD
Episcopal Supervisor, Episcopal WMS President, and Episcopal YPD Director shall select aConference YPD Director that exercised/exhibited leadership skills and positively conveyedand executed vision of Episcopal Leadership to her Annual Conference.
