How It Helps patIeNtsWhat would a patient-centered medical home mean for the girl with asthma and her family? A nurse might call her parents after their first ER visit to discuss how to prevent flare-ups. Her grandfather’s primary care doctor could coordinate with the grandfather’s specialists. And a scheduler could contact the girl’s grandmother when it’s time for her mammogram.

“Dr. Reedy and her team at the former Paramount Medical Group received recognition from the National Commit-tee for Quality Assurance for using this model in adult and pediatric care,” explains Pegeen Butterfield, director of care management. “Now that they’re integrated into Summit Medical Group, we’re working to expand elements of this model throughout the group.”

wHat Is It?“A patient-centered medical home isn’t a place,” explains Jamie Reedy, MD, MPH. “It’s a model for how to provide comprehensive, coordinated primary care.

“This model says that when you walk in our door, we address all your needs, not just your acute problem. We get all the information we need about you and any care you receive elsewhere. After we order tests, prescribe medicines, or recommend treatments, we follow up with you about what needs to happen next. If you haven’t been in for a while, we reach out to you,” says Dr. Reedy. Medical assistants, nurses, reception-ists, registered nurse care coordinators, and others work with doctors to make this happen.

Worried parents whisk their daugh-ter to the emergency room for a

serious asthma flare-up. Doctors control the symptoms and send them home, relieved. But three weeks later, the girl returns, wheezing badly.

Meanwhile, her grandfather with diabetes and heart failure can’t control his blood sugar. His problem isn’t a lack of health care but the opposite. He sees so many specialists, he can’t keep track of how to manage his conditions. His wife hopes he’s not headed for another hospital stay—which reminds her: Isn’t she due for a mammogram soon?

Scenes like this are not uncommon in health care. But Summit Medical Group is developing an approach to care that moves beyond your office visit or test, based on the concept of a “patient-centered medical home,” or PCMH.

Did you know that you can be at risk for heart attack without ever

having noticeable symptoms? If you’re like most people, you probably answered no to this question.

Although chest pain (or angina) is a common symptom of heart attack, a recent study shows that many people have no chest pain during a heart attack. Women ages 55 and younger in the study who had heart attacks with no chest pain were two to three times more likely to die in the hospital compared with men the same age who had classic heart attack symptoms.

Researchers believe the higher death rates in the women are probably because people without traditional heart attack symptoms are more likely to ignore their symptoms until it’s too late for lifesaving interventions. In addition, patients without classic heart attack symptoms are probably treated less aggressively when they reach the hospital.

There’s no cure for this chronic lung condition, but taking recommended

self-care steps can help you manage it.Chronic obstructive pulmonary disease

(COPD) refers to a group of illnesses, such as emphysema and chronic bronchitis, which cause breathing problems and stop air from reaching your lungs.

Tobacco use is the main cause of COPD, but asthma, air pollution, respiratory infections, and genetic factors can also lead to the condition.

Medication and supplemental oxygen can treat COPD symptoms. However, self-care plays a significant role in managing the disease. The following steps can help you control the symptoms and slow the progress of COPD.• Stop smoking. This is the most

important thing you can do to stop or slow the damage to your lungs. If you’re having trouble quitting, talk with your doctor.

• Get vaccinations. Have the flu vac-cine on a yearly basis and a pneumo-nia vaccine as recommended by your doctor. Preventing flu and pneumonia can minimize COPD complications.

• Join in rehabilitation and exercise programs designed for people with COPD. Doing so can help you stay fit.

• Avoid lung irritants. Limit your exposure to secondhand smoke, air pollution, chemical fumes, and dust. Keep your windows closed when air pollution or outside dust lev-els are high. Stay away from your home if you’re having it painted or sprayed for insects.

• Take all your medications as pre-scribed. Refill your prescriptions before they run out so you don’t come up short. Bring all your medicines to your doctor appointments so the dos-ages and frequency can be checked.

• Conserve your energy. Find easy ways to cook, clean, and complete other daily tasks. Ask friends and family for help if you can’t get things done on your own.

• Seek emotional support. COPD can cause fear, anxiety, depression, and stress. Talking about your fears with your friends, family, health care team, and a mental health professional can help you feel better. Joining a COPD support group can also help you cope with your feelings and condition.

PulmonologyBefore joining Summit Medical Group, Dr. Wilt was associate medical director of the New York Presbyterian Hospital/Columbia University Medical Center Adult Lung Transplant Program.

She also has been assistant professor of clinical medicine at Columbia University College of Physicians and Surgeons and medical director of the Inpatient Multi-organ Transplant Unit at New York Presbyterian Hospital/Columbia University Medical Center.

Dr. Wilt earned her BS in mo-lecular biology with high honors at Lehigh University and her MD at the University of Medicine and Dentistry of New Jersey-Robert Wood Johnson Medical School. After graduating, she completed her residency at Columbia Pres-byterian Medical Center.

Health Screenings Can Be lifesaving

These tests can serve as the wake-up call that persuades you to make

lifestyle changes a reality, the American Heart Association notes. Learning that your cholesterol is out of whack, for instance, may drive home the point that it’s time to start eating better and become more active physical-ly. Problematic test results make it per-sonal—a matter of your health and perhaps even your life—not just an abstract ideal.

Keeping track of the details, though, can be a challenge. Here are some of the health screenings most highly recom-mended by the U.S. Preventive Services Task Force:• Blood pressure screening:

Screening for high blood pressure for adults 18 and older

• Breast cancer screening: Screening every other year using mammography for women ages 50 to 74

• Cervical cancer screening: Pap tests every three years for women ages 21 to 65 who have had vaginal intercourse and have a cervix

Break Free from SmokingSummit Medical Group’s Behav-ioral Health and Cognitive Therapy Center is pleased to introduce its Freedom from Smoking program.

Developed and proven effective by the American Lung Association, patients are taught skills to quit smoking and stay smoke-free.

• Cholesterol blood test: Screening to determine cholesterol abnormali-ties is strongly recommended for men 35 and older and women 45 and older if they’re at increased risk for coronary heart disease. Screening is also recommended for men 20 to 35 and women 20 to 45 if they’re at increased risk for coronary artery disease.

• Colorectal cancer screening: Screening using fecal occult blood testing, sigmoidoscopy, or colonoscopy beginning at age 50 until age 75. As the task force notes, the risks and benefits of these three methods vary.

• Type 2 diabetes screening: Recommended for adults without any diabetes symptoms who have sus-tained blood pressure (either treated or untreated) higher than 135/80.

In terms of preventive health, regular screening tests are just part of the puzzle. You should also develop a relationship with a health care pro-vider. Your doctor can keep an eye on

your well-being, talk with you about a healthier lifestyle, discuss risky habits like smoking and excessive alcohol use, keep your immunizations up-to-date—and help you control illnesses when they do develop.

In some ways, diabetes doesn’t dis-criminate. Men and women are

equally likely to develop the disease. Both must work closely with their doc-tors to manage it.

But in other important aspects, diabetes isn’t an equal opportunity offender. Women often—but not always—bear the greater burden. Understanding the unique ways the condition affects each gender is impor-tant. Discussing them with your doctor can help you take control of your health.

womeN caN Get GestatIoNal dIabetesAs many as one in 10 women who didn’t have diabetes before will develop it during pregnancy. Often, your blood glucose returns to normal soon after the baby is born. However, your risk for type 2 diabetes remains higher for 10 to 20 years afterward.

more meN uNderGo amputatIoNA combination of poor blood flow and nerve damage contributes to foot infections in people with diabetes. As a result, they’re about 20 times more likely to lose a limb than the general population. Researchers recently reported that amputations are more frequent among men. However, more women die from complications related to the surgery.

Diabetes Strikes Men and Women Differently

I went to see neurologist Komal Naik, DO, at Summit Medical Group after suffering from debili-tating, daily headaches for more than four weeks, so I was not a very patient patient.

I told Dr. Naik my whole story. She listened intently and assured me she wouldn’t cause me any unnecessary pain, but that she had to try some things to see if her diagnosis and course of treat-ment was the right way to go.

I went for an MRI with contrast a few days later. A day or two afterward, I received a call from Dr. Naik telling me I didn’t have enough fluid going to my brain. She mentioned a cerebrospinal fluid leak, which is not uncommon and is treatable. I was scheduled for two not-so-fun procedures.

womeN more proNe to depressIoNIn general, depression strikes women twice as often as men. All too often, mood problems go hand in hand with diabetes, each fueling the other. A study in the Archives of General Psychiatry found the combination of diabetes and depression may be deadly: having both may significantly shorten women’s lives.

Heart rIsks dIffer, tooOverall, women have lower odds of getting heart disease than men. But diabetes turns the tables. Women with diabetes have almost the same risk for heart disease as men without diabetes. Even more concerning, if a woman with diabetes develops heart disease, she has a higher risk for death than a man in that situation would.

Regardless of gender, most people with diabetes die as a result of heart disease or stroke. Healthy habits such as eating right, exercising, and keeping your blood glucose, cholesterol, and blood pressure under control can help you live a longer, healthier life.
