icp triagemanager triages the health interview data by severity and prioritises care delivery through colour coding each patient to enable healthcare professionals to quickly identify those that need immediate intervention.

who is myclinic@home for? The myclinic@home enables patients to monitor their own condition from home and can also support up to 3 additional family members.

how does it work? On a regular basis, the patient will turn on the monitor to conduct their interview. The myclinic@home health interview guides the patient using its clear text display and audio announcements, through a series of measurements such as blood pressure, temperature and asks pre-set health interview questions.

The patient’s health interview information is then automatically transmitted to the icp triagemanager clinical software for review and processing by health professionals. Any deviation outside of pre-set parameters will alert health professionals so that treatment can be modified, stabilising the patient’s health.

The video conferencing capability allows remote patients to have virtual face-to-face contact with their healthcare provider on a regular basis.

This solution provides patients with increased understanding and knowledge of their condition, reducing anxiety levels and promoting self care.

in the comfort of their own homes.

how does it work?Each patient has a unique login, which when activated gives access to their individual health

interview. The health professional then guides the patient

who is myclinic for?myclinic is a multi user vital sign monitoring solution which enables a group of patients within a common location (residential aged care facility or multi purpose clinic for example) to participate in a telehealth program. Patients have individualised monitoring plans, but share the use of a terminal and medical device peripherals.

myclinic provides a cost effective way of delivering telehealth services to a wider group of potential patients, enabling early intervention and preventative care.

Traditionally, residential aged care homes are associated with a high rate of non elective hospital admissions, and so myclinic allows providers to deliver practical and efficient care into a high resource demand community location.

how does it work?Each patient has either a barcode or swipe card, which when activated on the myclinic monitor gives access to their individual health interview. The patient or clinician can then take the patient’s vital signs using the peripherals relevant to them and answer a series of tailored health questions

using the touch screen monitor. This information is then transmitted to the icp triagemanager software, which the clinician can access directly from the myclinic monitor or from any other device with internet access.

This proactive approach helps to reduce the need for visits to and by clinical staff, reducing the demand on the healthcare system and supporting people in a community setting.

Telehealth can help to improve quality of care by anticipating possible exacerbations before they occur, reducing non-elective admissions to hospital.

myclinic is supplied as a kit, which includes medical device peripherals with Bluetooth capability such as – blood pressure monitor, weighing scales, thermometer, pulse oximeter.
